4 Physicals must be dated after May 1, 2015 to qualify for the school year! Physicals must be completely filled out in order to be accepted, including all parent and student signatures. Please make sure the form is dated correctly. Please use the current VHSL physical form, located here:
5 June 15, 2015 at Centreville High School, 6:00-8:00 PM. Register: -- Sports Physicals $50 per athlete pre-registration, $60 walk-in
6 All students and a parent must complete online concussion education prior to participation in team activities. Must be completed for each new school year Concussion Education for Students and Parents
7 A concussions is an injury to the brain Concussions can alter a student s ability to learn Concussions can be life threatening Signs & Symptoms include: Headache, confusion, dizziness, change in personality, difficulty sleeping, difficulty concentrating, loss of appetite, loss of memory, and others. The best treatment for concussions is REST! CVHS Athletic Training Staff has extensive training in concussion evaluation, diagnosis and management.
8 All student-athletes must provide proof of medical insurance. If your child does not have insurance, please visit and click School Accident Insurance K&K Insurance provides K-12 Voluntary Insurance Policies for students, including football and sport coverage.
9 Participation Agreement - must be signed and given to your student-athlete s coach. Emergency Care Card - must be completed and given to your student-athlete s coach.

12 ATCs are Allied Health Professionals and are licensed by the Virginia Board of Medicine. Both CVHS ATCs hold Master of Science in Athletic training degrees (Mrs. McNamara from Shenandoah University, Mr. Francis from Weber State University). We are not personal trainers! We are medical professionals who specialize in emergency care, evaluation, diagnosis, management and rehabilitation of musculoskeletal injuries, including concussions!
13 Once final rosters have been submitted to the Student Activities Office, all students will complete a baseline Impact Test. Why Are We Using It? It is a tool that can help us identify if the brain has recovered to a point that the athlete is safe to begin to return to sport. It also helps us with the safe progression for full return to sport Students complete Impact Testing as Freshman, and Juniors. New Students to Fairfax County or Centreville must also take a baseline test.
14 Report to the Athletic Trainer! They will provide a full exam, diagnosis, and treatment recommendations to the student-athlete and parent. Injury rehabilitation is essential to proper recover from injuries. All injured athletes receive personalized rehabilitation programs to safely return to play. A Parent, Coach, Family Physician, Athlete, Administrator, ATC, or Team Physician can exclude an athlete from participation. All of these individuals must agree that the athlete can and should play The role of the Athletic Trainer is to make sure your athlete is safe to return to play.
15 Please provide documentation of the student s visit to the Athletic Trainer. Documentation should include any restrictions the student may have due to their injury. The student cannot return to play without documentation on file stating it is safe for them to participate, according to FCPS policy.
16 Report to the Athletic Trainer! Athletic Trainers are extensively trained on how to evaluate, diagnose, and manage concussions. Not all concussions need to go to the doctor reporting to the Athletic Trainer following a blow to the head is essential in determining appropriate care. Any student with the following symptoms after a head injury should report to the Athletic Trainer on site IMMEDIATELY:
