Box 489, Winchester, Ontario K0C 2K0 Tel: Fax: REQUEST FOR QUOTATION SUPPLY AND DELIVERY OF RECURRING SERVICES FOR 2016 & 2017 The Township of North Dundas utilizes various services on a recurring/as needed basis for its properties and facilities. Through the submission of quotations for these services, the Township will compile a list of vendors and prices. The Township will utilize the vendor with the lowest price where the work is performed satisfactorily and the vendor is available to perform the work. Where it is not possible to utilize the lowest vendor, the second lowest vendor will be utilized and so on. These quotations will be used when the value of the project is less than $ As per Township of North Dundas Policy (No ): Purchases & Selection of Supplies; Section III: Written Quotations ($2,000 - < $10,000) Where the estimated value of goods and services to be purchased is two thousand dollars ($2,000) or more but less than ten thousand dollars ($10,000), written quotations shall be required. When this method if purchasing is used, at least three (3) written quotations shall be requested. 3 Insurance: The successful bidders shall at their own expense within 10 days of notification of acceptance and prior to the commencement of work, obtain and maintain until the termination of the contract or otherwise stated, provide the Township with evidence of: Commercial General Liability insurance issued on an occurrence basis for an amount of not less then $2,000, annual aggregate for any negligent acts or omissions by the contractor relating to its obligations under this Agreement. Such insurance shall add Corporation of the Township of North Dundas as Additional Insured with respect to the operations of the contractor. Automobile liability insurance with respect to owned or leased vehicles used directly or indirectly in the performance of the services covering liability for bodily injury, death and damage to property with a limit of not less then $5,000,000 inclusive for each and every loss. Proof of workers compensation coverage is a requirement before commencement of any work. The Township of North Dundas reserves the right to review the limit of insurance coverage to reflect the exposures associated with work being carried out. The Policies shown above shall not be cancelled unless the Insurer notifies the Township in writing at least thirty (30) days prior to the effective date of the cancellation. The insurance policy will be in a form and with a company which are, in all respects, acceptable to the Corporation. 3

The successful candidate(s) or company must be trained to handle sewer drains, camera inspection and roto rooting. 8 Derelict Property Clean-up: The ideal candidate(s) or company will handle all property clean-up in terms of junk removal and disposal of grass and yard cleanup for any properties which have been issued a warning to clean their yard and cut their grass and who fail to comply. Commercial mowers may need to be utilized in some instances therefore applicant must be able to handle all grass cutting conditions. This service will be an on call and as needed basis. Please quote per worker and per apprentice per hour for service calls during regular hours.
