2 OPEN ENROLLMENT FOR PLAN YEAR JANUARY 1, 2013 DECEMBER 31, 2013 SEPTEMBER 10, 2012 THROUGH OCTOBER 5, 2012

3 CHANGES Elect a new health plan effective January 1, 2013 Elect a new dental plan effective January 1, 2013 Add eligible dependents not enrolled in 2012 Remove dependents Change of Address (should be done as soon as you move) Qualifying Events during the Plan Year Medicare eligibility

4 Elect A New County Health Plan 1. Verify eligibility for Plan see page 9 of Open Enrollment Guide. Former Local 512 members now eligible for Kaiser Permanente Basic Plan B, Kaiser Permanente Senior Advantage Plan B, Health Net HMO Plan B, Health Seniority Plus Plan B, Health Net PPO Plan B 2. Complete County Enrollment/Change Form. Elect the plan desired and list ALL dependents who should be included.

5 Elect A New Health Plan 3. If electing Kaiser Senior Advantage, you must also complete the Kaiser Senior Advantage enrollment form. 4. If electing Health Net Seniority Plus, you must also complete the Health Net Seniority Plus enrollment form. 5. If dis-enrolling from Kaiser Senior Advantage, you must also complete the Kaiser Senior Advantage dis-enrollment form. 6. I dis-enrolling from Health Net Seniority Plus, you must also complete the Health Net Seniority Plus dis-enrollment form. Forms are available in the Employee Benefits Office Be sure to submit forms to the Employee Benefits Office before 5 PM on October 5, 2012

6 Elect A New CalPERS Health Plan 1. Call CalPERS at and use the Interactive Voice Response System to make your change; or 2. Log on to CalPERS at follow the prompts to make your change 3. Call the Employee Benefits Office at to advise of the change you made so that your December 2012 premium deduction can be amended accordingly.+

7 Elect a New Dental Plan 1. Complete County Enrollment/Change Form. Elect the plan desired and list ALL dependents who should be included. Submit form to the Employee Benefits Office before 5 PM on October 5, 2012

8 Eligible Dependents Eligible dependents for health insurance non- Medicare retirees include: Legal spouse (survivors may not enroll a spouse) Qualified domestic partner Child of retiree or spouse or domestic partner to age 26 Disabled child beyond age 26 who is unmarried, incapable of sustaining employment due to a physical or mental handicap that existed prior to the child s attainment of age 19.

9 Eligible Dependents Eligible dependents for health insurance - Medicare retirees include: Legal spouse (survivors may not enroll a spouse) Qualified domestic partner (survivors may not enroll a domestic partner) Unmarried child who is dependent on you, your spouse of qualified domestic partner for support, who is: Under age 19 Age 19 24, full time student, dependent upon your for at least 50% of their support, unmarried and living with you Disabled child who is over age 19, unmarried, incapable of sustaining employment due to a physical or mental handicap that existed prior to attaining age 19 and is your dependent as defined by the IRC.

10 Eligible Dependents Eligible dependents for dental insurance Legal spouse (survivors may not enroll a spouse) Qualified domestic partner (survivors may not enroll a domestic partner) Unmarried child who is dependent on you, your spouse of qualified domestic partner for support, who is: Under age 19 Age 19 24, full time student, dependent upon your for at least 50% of their support, unmarried and living with you Disabled child who is over age 19, unmarried, incapable of sustaining employment due to a physical or mental handicap that existed prior to attaining age 19 and is your dependent as defined by the IRC.

11 Eligible Dependents The definition of dependent child includes natural child, step-child, adopted child, child of a domestic partner and any child specified in a Qualified Medical Child Support Order or similar court order.

12 Add Eligible Dependents Not Enrolled in Complete County Enrollment/Change Form. Obtain the certified copy (raised seal) of the marriage certificate or birth certificate or court order. The certified birth certificate or court order must include: retiree s name; or, spouse s name; or, domestic partner s name Submit form and documents to the Employee Benefits Office before 5 PM on October 5, 2012

13 Remove Dependents 1. Complete the Open Enrollment change form. 2. Mark the level of coverage you elect for List the dependent to be removed from the medical and or dental plan 4. Be sure to code with a D Submit form to the Employee Benefits Office before 5 PM on October 5, 2012

14 Change of Address When you move, you need to let the Employee Benefits office know your new address. We have received approximately 65 returned open enrollment packets for retirees. We don t have their current addresses. It is very important to let us know when you move so we can update our records. Also, moving may mean that you are eligible for a different health plan. And, you may have moved out of the area for your current benefit election.

17 Medicare Eligibility 90 days before you or your spouse attain age 65 Employee Benefits will send letter You need to contact Social Security No Medicare Part D Plans Changing to Medicare Supplement Plan is allowed with same carrier Changing carriers is not allowed (other than open enrollment)
