You expect the best from your employees while they are on assignment. And they expect the same from you. The U.S. health care system is a mix of private and governmental systems. The government provides health care coverage and benefits for the elderly and poor, while workers and their family members receive private insurance primarily through their employers. In this arrangement, the employer and employee usually share the cost of benefits, also known as cost-sharing. Health Care Reform adds to the complexity of the U.S. health care system. Employees and employers in the U.S. need to understand how the Patient Protection and Affordable Care Act (PPACA) affects their assignment. A participating Cigna network health care professional, means that the doctor, hospital, or clinic has agreed to charge lower fees and bill Cigna directly for services. Customers simply show their ID card to take advantage of cost-saving benefits. Accessing care outside the U.S. Plans may also allow for care to be accessed outside of the U.S., including your employee s home country. Global health solutions Cigna Global Health Benefits has clinical operations teams located around the world. In the United States, we have medical specialist resources to provide quality support. Our clinical operations team is responsible for international case management and medical monitoring with a focus on global consistency and local delivery. We have in-house Medical Directors, including a Chief Medical Officer. We also have in-house Nurse Case Managers and a clinical support staff. All members of our Clinical Operations team work to provide your employees with quality medical support, no matter where their assignments may take them. Additionally, Cigna uses the services of CareAllies within the United States. This case management partner oversees patient care through telephonic assessment, planning, coordination, monitoring, and evaluation of appropriate cost-effective treatments. CareAllies is a full-service offering for U.S. cases including: ymedical case management services for a variety of medical, surgical, or trauma related illnesses, and injuries. ybehavioral Health case management for all mental health and substance abuse diagnoses. 1
4 How we help Case study (The following case study is an example of how the Clinical Staff at Cigna Global Health Benefits helps our customers. This story is based on an actual case. The person involved has given their express permission to portray certain events.) A 28- year-old female Indian national, working in California, was struck by a vehicle while crossing the street. She suffered numerous traumas, including a crushed pelvis, head injuries, multiple fractures of the upper and lower extremities, liver and bladder lacerations. The woman required multiple surgeries and was on a ventilator for several weeks. Case managers maintained close contact with the customer s family during this time. Her brother, the first family member to arrive from India, needed to secure larger living space to accommodate additional family members who would help care for the woman once she was released from the hospital. The woman was later discharged to a rehabilitation facility for six weeks with cognitive and functional impairments, including the inability to walk and feed herself. She was then discharged to intense physical rehabilitation at home for seven-and-a-half weeks. After the completion of rehabilitation, she was moved to an outpatient physical rehabilitation center for 12 weeks. The case manager and medical director collaborated with CareAllies and the rehabilitation specialist, as well as the family, to monitor her care and anticipate her discharge needs. During this collaboration, there was discussion regarding her ability to return to work as a software engineer, or if she should return to India. Many scenarios were taken into consideration including her ability to fly, whether or not the company would keep her job open, and her lack of insurance coverage if she returned to India. After careful review, it was determined that there was no benefit for the woman to return to India. The woman s family members would take turns serving as the primary caregivers once she was discharged home. The woman progressed well with rehabilitation, and contrary to earlier predictions, returned to work full time nine months later. 2
