Larry Cannon GPP Alcoa I ve seen the impact a fatality has on families, work places and communities. There s no greater sense of loss. It puts into perspective everything we do and there is no product or service we can provide that is worth someone s life. There s no greater cause than for us to commit to the elimination of fatalities. We are imperfect humans operating in a very dynamic work environment, contending with factors associated with task demands, human nature and individual capabilities. Janne Sigurðsson Alcoa Fjarðaál Because all other achievements does not matter if we have a fatality We can NEVER relax two minutes! We must ensure 500 people is constantly working with us on this matter!
Ólafur Gauti Sigurðsson Alcoa Fjarðaál We all have someone waiting at home for us. We need everyone and everyone need us,,, to work will be completed,,, each shift,,, each day,, Jan Werner Hansen Alcoa Lista A human life is more important than any aluminium i product To avoid being blind for the risks in your own plant and daily routines
Bjørn Inge Johansen Alcoa Lista If I can save lives or prevent employee injuries I speak up That all employees speak up when they see unsafe conditions or actions John-Egil Marken Alcoa Mosjøen Understand highest risk activities and tasks for our business - identify practical approaches a facility can use to develop a risk profile Identify areas of future safety research Stand Up to Ownership Engage Employees Training and Education
Per Storvik Alcoa Mosjøen Understand highest risk activities and tasks for our business - identify practical approaches a facility can use to develop a risk profile Identify areas of future safety research Stand Up to Ownership Engage Employees Training and Education Ronny Vatland GGP Alcoa Europe Nothing is more tragic to a company then a work related fatality. We are trusted the responsibility not killing anyone at our plants a responsibility we need to honor. Sometimes what is regarded a minor risk can drive the worst possible outcome a fatality. Understanding the true nature of fatalities, especially the human elements, is a challenge.
Olaf Wigstøl Hydro Aluminium Primary Metal Occupational fatalities are preventable. It is the employers responsibility to create a work environment to ensure that they will not happen You might have serious injury potential even if your safety statistics are good Edgar Sagen Hydro Aluminium Primary Metal I want to stay alive I want my colleagues to stay alive Fatalities happens sometimes where least expected Often behaviour based low risk perception and shortcuts
Trygve B. Svendsen Hydro Aluminium Primary Metal A fatality is absolute the worst that can happen in a plant. Have experienced three fatalities at Karmøy plant. We can never be assure that people and machinery never fail Wenche Eldegard Hydro Aluminium Høyanger Our slogan is: Make everyone care about safety All employees can trust that it`s safe to work at the plant We eare working with people and peoples behaviour
Bjørn Ståle Helle Hydro Aluminium Høyanger Our slogan is: Make everyone care about safety No one is going to get hurt on my watch! Safety work is fresh goods, we are working with the inside of the peoples heads!! Jan Sæbø Hydro Aluminium Høyanger It is important that everybody goes home as healthy as they arrived Low manning and high speed in production makes sometimes stress - Stress and safety are not a good combination and can lead to unsafe actions like shortcuts and high speed driving
Are Jaastad Hydro Aluminium Karmøy Its the ultimate failure... Nobody wants to be a part of, or responsible for thatt The balance between the short term objectives and the fundamental work we need to do to prevent disaster Geir Høines Hydro Aluminium Karmøy Because I would have felt that all EHS work would be useless if we should get a fatality Through you have control on the overall area, a detail may cause a fatality
Yngve Eriksen Hydro Aluminium Karmøy Because my colleagues shall not be injured or sick of their work That we are only human and may do mistakes Martin Sagen Hydro Aluminium Sunndal I know by experience how tragic a fatality is for family, colleagues, company and society around People tend to think and act like they are immortal
Arne Magne Johannessen Hydro Aluminium Sunndal Life is valuable Lack of safe behaviour Bjørn Egil Kjørnes Hydro Aluminium Sunndal To eliminate the same thing happening again Make sure that we all know what is dangerous
Arne Werge-Olsen Hydro Aluminium Årdal As manager, people is my main concern Nothing hurts me more than employees not coming home in good health All accidents are evitable Hanne Hoel Pedersen Hydro Aluminium Årdal I don t have anyone to lose, we need to make sure everybody returns home safely every day! This is mostly about behaviour, and peoples mind is the most difficult thing to change
Harald Midtvoll Hydro Aluminium Årdal Accidents do not happen - They are caused Any accident - Try and predict its potential before it may happen Rannveig Rist Rio Tinto Alcan, ISAL It is important to me because with full respect to all our efforts and investments, nothing is more important than life itself and the health - It is not replaceable if lost We have to make sure that our behaviour will keep us safe at all times
Halldór Halldórsson Rio Tinto Alcan, ISAL The company that I work for has to be responsible for the welfare and safety of all employees It is in a human nature to believe that you are safe and not see the hazards. The greatest challenge is to change human behaviour so employees can see the purpose of safety and will change their behaviour voluntarily Davide Garofalo Kubikenborg Aluminium, KUBAL Fatality prevention is a key attitude for any company to increase risk and safety awareness in the organization, to speed up improvement activity implementation and to reduce the level of acceptance of deviations and potential impact on the employee and asset working life. It is a matter of driving each employee to feel owner of the safety standards of the company and to act as a sole organization across all the facets of the business (management, operations, offices, unions, authorities and customer/suppliers) Factors that can influence a successfull fatality prevention program could be a lack of allignment into the top managment level and/or not having the people with the right attitude on board (therefore a key success factor is to have people fully committed to Safety and continuous improvement to have a Fatality program succeding).
Gunnar Nyberg Kubikenborg Aluminium, KUBAL Prevention of occupational fatalities depends on understanding that worker safety is not only the resposibility of the worker, but is the primary responsibility of the employer. Employers must train all employees in the appropriate safety procedures and maintain a safe working environment so that fatalities are less likely to occur. No one should have to inform a bereaved family about a workplace fatality at Kubal! To recognize the most effective leadership styles and organizational attributes necessary for a fatalityprevention effort, including but not limited to training, root-cause analysis, and employee engagement. Finn Are Berntzen Sør-Norge Aluminium, SØRAL No employer, family member or coworker ever wants a fatality to occur in the workplace One of the challenges is to identify where the next fatal exposure might be and to put the right prevention methods into place
Geir Yven Sør-Norge Aluminium, SØRAL The importance of HSE goal every business must have, both from the management side and the employee side, is to avoid all forms of damage, and the most important thing in this context is to prevent deaths. We should go to work with the knowledge that we all should go home afterwards as healthy and whole as when we came to work. All safety depends on each and one. Regardless of the safety rules, we always ends up with how each individual thinks and performs their job and is conscious of their actions. Magne M. Nordvik Vigeland Metal Refinery It s peoples life at stake and it is possible to prevent It s peoples life at stake and it is not OK to take the risk
Freddy Larsson Vigeland Metal Refinery Our responsibility for the employee Behaviour Kåre Andor Arntsen Vigeland Metal Refinery Main cause is to take care of my colleagues To be tip toe all time...
Elly B. Andresen AMS All colleagues are valuable to me and their wellbeeing is important t - We have no one to loose We are not perfect and our behaviour often works against our good intentions
