It puts into perspective everything we do and there is no product or service we can provide that is worth someone s life. There s no greater cause than for us to commit to the elimination of fatalities. We are imperfect humans operating in a very dynamic work environment, contending with factors associated with task demands, human nature and individual capabilities. We are trusted the responsibility not killing anyone at our plants a responsibility we need to honor. Sometimes what is regarded a minor risk can drive the worst possible outcome a fatality. Understanding the true nature of fatalities, especially the human elements, is a challenge.

The greatest challenge is to change human behaviour so employees can see the purpose of safety and will change their behaviour voluntarily Davide Garofalo Kubikenborg Aluminium, KUBAL Fatality prevention is a key attitude for any company to increase risk and safety awareness in the organization, to speed up improvement activity implementation and to reduce the level of acceptance of deviations and potential impact on the employee and asset working life. It is a matter of driving each employee to feel owner of the safety standards of the company and to act as a sole organization across all the facets of the business (management, operations, offices, unions, authorities and customer/suppliers) Factors that can influence a successfull fatality prevention program could be a lack of allignment into the top managment level and/or not having the people with the right attitude on board (therefore a key success factor is to have people fully committed to Safety and continuous improvement to have a Fatality program succeding).

Employers must train all employees in the appropriate safety procedures and maintain a safe working environment so that fatalities are less likely to occur. No one should have to inform a bereaved family about a workplace fatality at Kubal! To recognize the most effective leadership styles and organizational attributes necessary for a fatalityprevention effort, including but not limited to training, root-cause analysis, and employee engagement. We should go to work with the knowledge that we all should go home afterwards as healthy and whole as when we came to work. All safety depends on each and one.
