McClelland and Hine of Houston, LLP USER S GUIDE FOR QUOTING JANITORIAL SERVICE RISKS THROUGH USLI The USLI Janitorial Service Risks program is designed to quote risks that perform janitorial services at premises they do not own, rent, lease, or borrow. This guide is designed to walk you step-by-step through the online quote process. It is our hope that herein we will provide answers to the most common questions that arise from ordinary use of the program, however if at any time you have questions or concerns that we have not addressed, please feel free to contact your MHI marketing representative or any MHI commercial lines underwriter. NOTE: If your applicant has operations besides janitorial services, e.g. painting, carpentry, handyman work, etc., you will not be able to obtain a quote through this program. In that case, please refer to the Artisan Contractor program selection on our E-Quote menu. Other underwriting considerations may also prevent you from obtaining a quote. Some of these are: Applicant sells products under his own name or label Applicant has operations on buses, trains, or airplanes, or in terminals or stations Applicant handles infectious waste or hazardous material Applicant has operations at Health Care or Assisted Living Facilities Applicant operations include Insurance Claim Response, Water Removal/Extraction, Mold Remediation, Hood/Duct Cleaning, or Security More than 50% of applicant s operations involve floor waxing Exterior operations exceed 4 stories Applicant has bankruptcies, tax liens or credit liens in the last 5 years Operations include street cleaning or debris removal Applicant s annual gross receipts exceed $1,000,000 Applicant performs work at mercantile locations during regular business hours and/or when location is open to the general public If any of the above situations applies to your risk, please contact your MHI underwriter or submit a completed application for quote consideration. If not, please use the instructions that follow to obtain a quotation.
2 1. Access MHI website: 2. Click on E-Quote 3. Login with your assigned user id and password 4. At E-Quote menu, choose Janitorial under USLI
3 5. At the Company Direct Rating Applications page, review the information provided and click on the Janitorial Services link provided. 6. The next screen is your initial quote screen. Do one of the following: a. For a NEW BUSINESS quote, click New Quote. b. To access an existing quote, either: (1) Enter the requested information and click Continue>>, or (2) Leave blank and click Continue>>. This will bring up a list of your existing quotes. Selection of an existing quote will prompt the following options:
4 7. Applicant Information Screen. All information needs to be completed. Commercial General Liability coverage is automatically selected. You have the option to quote a Package policy by checking the box next to Commercial Property. NOTE: At any time during the quote process, you can access a previous screen by clicking on the <<Back button. 8. Claims Screen. If the applicant has had no claims, enter 0 (zero) as number of claims for each year and click Continue>>. Otherwise, enter claims information as indicated. Make sure information you enter is accurate and complete.
5 9. Classification Screen. Question 1. It is important to answer this question correctly as it may impact the premium. Questions 2-3. USLI s system calculates premium based on number of employees (not on payroll). Count each owner and/or partner as 1 full time employee. One owner = 1 full time employee, 2 owners/partners = 2 full time employees, etc. Enter the number of full time employees (including each owner/partner as 1), and enter the number of part time employees. 10. Location and Limits Screen. Enter applicant s location address. If location address is the same as mailing address, click the box that says Use as mailing address. Select the desired General Liability limits. Note that you may select a limit for Products/Completer Operations, however the quote will show this limit as Included.
6 11. Independent Contractors / Additional Classification Screen. Answer questions as indicated. If applicant would like to purchase coverage for Independent Contractors, you will need to know the total annual cost of subcontracted work. NOTE: if applicant has operations other than janitorial services, risk will need to be submitted for a quote. 12. Liability Coverages / Inland Marine Coverages. Select the Property Damage Extension Endorsement if desired. This provides $5,000 per occurrence, $25,000 aggregate property damage coverage for PD occurring as a result of the insured s operations performed at premises at which insured is contracted to perform Janitorial Services. (Please review coverage form L-440 for complete details.) Select the blanket Additional Insured Endorsement if desired. Note that additional insured status is only applicable to persons or organizations as reported to the Company or to McClelland and Hine in writing prior to a loss. Select Inland Marine Coverage if desired. This provides inland marine coverages as follows: Janitorial Equipment - $2,500 per item / $10,000 per loss, $500 deductible, ACV Equipment Rental Reimbursement - $250 per day / $5,000 per loss, No deductible, ACV Lost Key Coverage - $25,000 limit, No deductible, ACV (Please review coverage forms IH0068, IH6802, IH6803 and P227 for complete details.)
7 13. General Liability Applicant Eligibility Screen. Answer questions appropriately. 14. Quote Summary Screen. Review information provided for correctness. Make any necessary changes by clicking on the individual Edit buttons in each section. When review is complete, click Continue>>.
8 15. Continuation of Quote Summary Screen. This screen lists all the endorsements that will be attached to the policy when issued. If you d like to attach a copy of the form to the quote, place a check next to that form. You may also click on the form name/number to view a copy of the form on your screen. Review information provided. When review is complete, click Continue>>.
9 16. Additional Applicant Information and Retailer Information Screen. Complete all relevant additional information regarding applicant and retailer. You may place any necessary explanatory notes or notes to the underwriter on the bottom of this screen in the Notes to show on quote screen. When data entry is complete, click Save & View Quote.
10 17. Final Quote. Your final quote will be displayed in.pdf format. You will be able to print, save, or the quote from this screen in your usual manner.
