2 Annual Security Report Tampa, Florida The safety of students, associates and visitors is an important concern of Concorde Career Institute. Concorde Career Institute has prepared this report to increase your awareness and provide information to protect your safety and well-being. This report includes statistics for the previous three years concerning reported crimes that occurred on campus; in certain off-campus buildings or property owned or controlled by Concorde Career Institute; and on public property within, or immediately adjacent to and accessible from, the campus. The report also includes policy statements concerning campus security, sexual assault, sexual misconduct and other matters. You can obtain a copy of this report by contacting a school administrator or by accessing the following website: REPORTING CRIMES & OTHER EMERGENCIES ANNUAL DISCLOSURE OF CRIME STATISTICS REQUIREMENT It is the policy of Concorde Career Institute to comply with the requirements of the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act, commonly known as The Clery Act. The school is responsible for gathering the applicable crime statistics from internal records, local police department(s) and campus security (where available). The campus completes the Campus Security Report and forwards all information to the Campus Support Center s Regulatory Affairs department. The Regulatory Affairs department utilizes the information to complete the Annual Security Report. REPORTING OF CRIMINAL OFFENSE All victims of crimes that occur on campus or persons observing crimes, suspicious activities or accidents may report these incidents on a voluntary and confidential basis in an accurate and prompt manner to the Campus President, local law enforcement or by dialing 911. a. Kathy Tillinghast, Academic Dean, VOLUNTARY CONFIDENTIAL REPORTING The Campus President provides to all victims of crimes that occur on campus the opportunity to report the incident to the local police department. The Campus President or campus personnel will assist the student in notifying law enforcement authorities if the student requests assistance. If a victim declines to report the matter to the local police department, the incident will be handled as an institutional matter. Concorde Career Institute reserves the right to treat an offense as a disciplinary matter whether or not it is being investigated by a police agency. In the instance of an emergency threat to health or safety, Concorde Career Institute will notify local law enforcement. SECURITY & ACCESS POLICY Academic buildings are open to students and associates during prescribed hours as long as students and associates comply with the rules and regulations of the campus. The administration offices are open to associates, students and staff during normal business hours. The campus is not open to the public. Visitors are subject to the rules and regulations of the campus. Visitors must check in at the receptionist s desk and be accompanied by an associate. The facilities maintenance of Concorde Career Institute is coordinated by the Campus President. Maintenance personnel are not available on an emergency basis and generally do not deal with safety concerns of the campus. A school official must be notified.

3 CAMPUS LAW ENFORCEMENT POLICY STATEMENTS OVERVIEW Concorde Career Institute cooperates with outside law enforcement agencies and exchanges criminal information. The local police department; local county sheriff s department; and other local police agencies, state agencies and federal law enforcement agencies have criminal jurisdiction over Concorde Career Institute. Concorde Career Institute encourages all victims of crimes that occur on the campus or persons observing crimes, suspicious activities or accidents to report these incidents on a voluntary and confidential basis in an accurate and prompt manner to the Campus President and/or local law enforcement agencies. Concorde Career Institute does not have a campus police/security department. Concorde Career Institute does not provide pastoral or professional counseling services. Referrals to outside agencies that can provide these services are available upon request. Concorde Career Institute does not have any officially recognized student organizations. TIMELY WARNINGS & EMERGENCY NOTIFICATION & EVACUATION POLICY In the event that a situation arises either on or off campus, that, in the judgment of the Campus President and applicable Campus Support Center officials, constitutes a serious or continuing threat to students or employees, a campus-wide Timely Warning will be issued. The Campus President, along with applicable Campus Support Center officials, is responsible for the issuance of a Timely Warning. Timely Warnings may be issued through the following means: to faculty, staff and students Pushed texted messages Posting a copy of the warning in classrooms, labs, break rooms, and all entrances and exits Posting a special announcement on the campus-specific company website at Campus President or another member of the school s management team will visit each classroom to inform all students of the situation Campus President will inform all applicable local law enforcement agencies Emergency Notification & Evacuation Policy The Campus President or designee along with Campus Support Center officials will be responsible for determining whether an emergency or dangerous situation exists. An emergency or dangerous situation is defined as an immediate threat to the health or safety of students or employees and occurs on campus. Once the determination has been made that an emergency or dangerous situation exists, the school will, without delay, and taking into account the safety of the community, determine the content of the notification and initiate the notification system, unless issuing the notification will, in the professional judgment of the responsible authorities, compromise efforts to assist a victim or contain, respond to, or otherwise mitigate the emergency. Once the determination to issue an emergency notification or evacuation has been made, all students and staff will be notified. The content of the notification will be created by the Campus President or designee and reviewed by the Campus Support Center officials. Emergency information will be disseminated via local news media, text messages, posts to the campus-specific Web page at telephone pages, visits to classrooms and any other means necessary to ensure the health and safety of all students, staff and faculty. Notification, response and evacuations will be tested on an annual basis, including announced and unannounced tests. The tests will be documented as follows:

They may also be requested from the Campus President. SEXUAL ASSAULT, DOMESTIC VIOLENCE, DATING VIOLENCE & STALKING POLICY Sexual misconduct, assault, and/or harassment are a violation of Concorde Career Institute Standards of Conduct for students and associates and the Sexual Harassment policy, as well as a violation of the penal code. Concorde Career Institute is committed to ensuring the health and safety of our students, staff and faculty. In April 2014 the wall of resources was completed for all staff and students. In July 2014, all faculty received OSHA training. Sexual Harassment training is provided to all new associates. The school continually provides information to all new students regarding the Florida Department of Law Enforcement Sexual Predator and Sexual Offender Registry website and provides community information to help students in difficult situations. The school also provided all new students with Drug-Free Schools and Alcohol policy. Whenever possible, victims should report an incident of sexual assault, domestic violence, dating violence, stalking or rape as soon as possible. It is important to preserve evidence as it may be necessary to prove that domestic violence, dating violence, sexual assault or stalking occurred. Additional information can be found in the Sexual Misconduct Policy and Procedures posted at All victims of crimes that occur on the campus or persons observing crimes, suspicious activities or incidents may report these on a voluntary and confidential basis in an accurate and prompt manner to the Campus President or designee and/or local law enforcement. If requested, campus officials can facilitate reporting a crime to campus or local law enforcement, but they will respect the victim s request not to do so, assuming the health and safety of the Concorde community is not compromised. In the event of a threat to the health or safety of the Concorde community, local law enforcement will be contacted. The investigation and records of the resolution conducted by the campus are maintained confidentially. Information is shared internally between administrators on a need-to-know basis. Where information must be shared to permit the investigation to move forward, the person bringing the accusation will be informed. Privacy of the records specific to the investigation is maintained in accordance with state law and the federal Family Educational Rights and Privacy Act (FERPA) statute. Any public release of information needed to comply with the open crime logs or timely warning provisions of The Clery Act will not include names of the victim or information that could easily lead to a victim s identification. Additionally, the school maintains privacy in relation to any accommodations and/or protective measures. Irrespective of state law or public records access provisions, information about victims is maintained privately in accordance with Title IX and FERPA. Once complete, the parties will be informed, in writing, of the outcome, including the findings, the sanctions (if any) and the rationale therefor. Counseling for victims of sexual assault or misconduct is not available on campus. Concorde Career Institute does not employ pastoral or professional counselors. After reporting a sexual assault or sexual misconduct, a student victim may request changes in an academic situation. The student may seek assistance from the Campus President or make direct requests to the Academic Dean. These changes will be made if they are reasonably available. Concorde Career Institute will take immediate steps to protect victims of sexual misconduct or assault. Concorde Career Institute will maintain a positive learning and working environment by minimizing or eliminating contact with the respondent and providing reasonable academic and administrative accommodations in accordance with

5 The Clery Act and Title IX. The accuser and the accused perpetrator are entitled to the same opportunities to have others (e.g., witnesses or advocates) present during a campus disciplinary proceeding. Both parties shall be informed of the outcomes of any campus disciplinary proceeding alleging sexual assault or misconduct. Once an informal or formal complaint of sexual assault, misconduct or harassment has been received, an investigation will be conducted. Possible sanctions against a student or an associate found guilty of sexual assault through campus proceedings include expulsion, suspension and probation for a student or disciplinary action up to and including termination of employment for associates. Please refer to the student Code of Conduct located in the student catalog or the Code of Ethics and No Discrimination or Harassment Policy for associates. Sanctions for sexual harassment, assault or misconduct may involve penalties up to and including expulsion for students and termination of employment for associates regardless if the victim reports the crime to local law enforcement. STUDENT DISCIPLINARY HEARING RESULTS Concorde Career Institute will disclose the results of a disciplinary proceeding for a violent crime or non-forcible sex offense to the victim of such crime/offense or the next of kin if the victim is deceased. MISSING STUDENT NOTIFICATION POLICY Concorde Career Institute does not have campus housing and, therefore, does not have a missing student notification policy. SECURITY AWARENESS PROGRAMS Various programs are designed to inform students and associates about campus security procedures and practices and to encourage students and associates to be responsible for their own and others security. Campus security concerns are addressed in weekly campus senior management meetings. All students, staff and faculty are provided a copy of the Annual Security Report. Special security bulletins will be distributed, if a need arises, to advise students or associates of community crime patterns and suspect information. Associates are provided with security orientation information during their initial employment evaluation period. CRIME PREVENTION PROGRAMS Concorde Career Institute supports programs that offer educational information and victim assistance in areas such as acquaintance rape and sexual assault. Brochures related to various prevention programs are available to students and associates. A new associate orientation checklist includes a security section and is used to document the training that supervisors give their associates. Crime prevention is discussed at new student orientation and as needed. A parking lot escort is available for associates and students through the campus by contacting either the receptionist or Campus President. DRUG & ALCOHOL POLICY The information in this section is in accordance with the Drug-Free Workplace Act of 1988 and the Drug-Free Schools and Communities Act Amendments of The consumption and/or possession of any alcoholic beverage by any person younger than 21 years of age is forbidden as provided by state law and campus regulations. Concorde Career Institute prohibits the use, possession, manufacture, sale or distribution by its students or associates of any illegal drug. Students and employees are reminded that unlawful possession, distribution or use of alcohol may subject individuals to criminal prosecution. Concorde Career Institute may refer violations of prescribed conduct to appropriate authorities for prosecution. Please refer to the school catalog for additional information.

6 REGISTERED SEX OFFENDER POLICY In accordance to the Campus Sex Crimes Prevention Act of 2000, which amends the Jacob Wettering Crimes Against Children and Sexually Violent Offender Registration Act, the Jeanne Clery Act and the Family Educational Rights and Privacy Act of 1974, the school is providing a link to the State Sex Offender Registry. All sexual offenders are required to register and provide notice to each institution of higher education at which the person is employed, carries out a vocation or is a student. Negligent Manslaughter: The killing of another person through gross negligence. Sex Offenses - Forcible: Any sexual act directed against another person without the consent of the victim, including instances where the victim is incapable of giving consent. Rape: The penetration, no matter how slight, of the vagina or anus with any body part or object or oral penetration by a sex organ of another person, without the consent of the victim. Fondling: The touching of the private body parts of another person for the purpose of sexual gratification, without the consent of the victim, including instances where the victim is incapable of giving consent because of his/her age or because of his/her temporary or permanent mental incapacity. Non-Forcible Sex Offenses: Unlawful, non-forcible sexual intercourse. Incest: Non-forcible sexual intercourse between persons who are related to each other within the degrees wherein marriage is prohibited by law. Statutory Rape: Non-forcible sexual intercourse with a person who is under the statutory age of consent. Robbery: The taking or attempting to take anything of value from the care, custody or control of a person or persons by force or threat of force, violence and/or causing the victim fear.

8 Aggravated Assault: An unlawful attack by one person upon another for the purpose of inflicting severe or aggravated bodily injury. This type of assault usually is accompanied by the use of a weapon or by means likely to produce death or great bodily harm. It is not necessary that injury result from an aggravated assault when a gun, knife or other weapon is used which could or probably would result in a serious potential injury if the crime were successfully completed. Burglary: The unlawful entry of a structure to commit a felony or a theft. For reporting purposes, this definition includes: unlawful entry with intent to commit a larceny or a felony, breaking and entering with intent to commit a larceny, housebreaking, safecracking and all attempts to commit any of the aforementioned. Motor Vehicle Theft: The theft or attempted theft of a motor vehicle. Larceny - Theft: The unlawful taking, carrying, leading or riding away of property from the possession or constructive possession of another. Arson: Any willful or malicious burning or attempt to burn, with or without intent to defraud, a dwelling, house, public building, motor vehicle or aircraft, personal property, etc. Liquor Law Violations: The violation of laws or ordinances prohibiting the manufacture, sale, transporting, furnishing, possessing of intoxicating liquor; maintaining unlawful drinking places; bootlegging; operating a still; furnishing liquor to a minor or intemperate person; using a vehicle for illegal transportation of liquor; drinking on a train or public conveyance; all attempts to commit any of the aforementioned. (Drunkenness and driving under the influence are not included in this definition.) Drug Abuse Violations: Violations of state and local laws relating to the unlawful possession, sale, use, growing, manufacturing and making of narcotic drugs. The relevant substances include: opium or cocaine and their derivatives (morphine, heroin, codeine), marijuana, synthetic narcotics (Demerol, methadone), and dangerous non-narcotic drugs (barbiturates, Benzedrine). Illegal Weapons Law Possession: The violation of laws or ordinances dealing with weapon offenses, regulatory in nature, such as: manufacture, sale or possession of deadly weapons; carrying deadly weapons, concealed or openly; furnishing deadly weapons to minors; aliens possessing deadly weapons; all attempts to commit any of the aforementioned.
