1 REQUEST FOR BIDS The Cook County Commissioners are requesting sealed bids for pest control and termite services for all county buildings. Bid packets are available at the County Administrative Building, 1200 S. Hutchinson Ave, Adel, Georgia or Bids will be accepted through Wednesday, November 26, 2014 at 4:30 p.m., bids will be opened during the December 1, 2014 work session. Cook County reserves the right to refuse any and all bids. Vicki S. Parrish, County Clerk ATTENTION VENDORS: The awarded vendor will enter into an agreement with the county for services for a time period of three years, with an opt out for either party of 90 days. *** In order for your bid to be considered the following documentation must be submitted with your bid: 1. 3 A photocopy of the licenses from the Structural Pest Control Commission and Certificate of Insurance must a part of this bid packet. The County reserves the right to request additional services when pest are present. The company must re treat affected areas within 3 business days of the request being made. Daily re services may be required in the case of rat or mouse infestations. If the company fails to comply with the specifications or fails to render satisfactory service, it shall be given 30 days notice to correct the problem or comply with the specifications of the agreement, or the agreement will be canceled by the county immediately. Either party to the agreement may cancel participation with 90 days written notice to the other party. MATERIALS: All materials used indoors in the program will be of a low odor nature or odorless. All materials will be used in accordance with the Federal, State, and Local regulation and in exact accordance with the label instructions. PEST COVERED UNDER THE SERVICE: The pest covered under the proposed service will include the following: Ants, roaches, spiders, crickets, centipedes, silverfish, earwigs, rats, mice These pests are to be controlled or prevented by indoor applications and perimeter applications when necessary to provide indoor control of pest. Termite Inspections will be performed under this proposal twice (semi) annual each year for no additional costs to the customer other than those set out in the bid packet. Quotations of treatment prices on an as necessary basis will be provided for no additional charge to the county. DOCUMENTATION: FOR MONTHLY PEST SERVICES/OR RESERVICE ISSUES: When services are performed a work ticket will be filled out by the technician. This will be signed by the customer s representative at the application site. In cases where there is nobody at the application site, this will be indicated on the work ticket. FOR SEMI ANNUAL TERMITE INSPECTIONS: When termite inspection services are performed at each building site, a work ticket will be filled out by the technician depicting date/time of inspection and the technician will indicate on the ticket as to the findings of the inspection. This will be signed by the customer s representative at the application site. In cases where there is nobody at the application site, this will be indicated on the work ticket. Monthly or semi annual work tickets will be maintained by the company and copies will be submitted to the Commissioner s Office along with applicable invoices.
