2 JOB SUMMARY The provides and coordinates individualized care and resources to elders, disabled and others so that they may continue to live in their homes and in the community. duties may include transportation, house cleaning, personal hygiene, providing meals, and other health and wellness related activities. The will liaise with other community services and resources to ensure that all other resources are utilized when required. The provision of these services reduces the risk and/or assists the elders. These services assist extend the elders or individuals ability to remain at home rather than live in a facility. REPORTING STRUCTURE Reports to the Home and Community Care Supervisor JOB DUTIES AND RESPONSIBILITIES Provide individual home care to elders, disabled persons, and others in the community with identified needs Provide home care including general housekeeping, personal hygiene and provide simple meals or snacks Transport home care clients to appointments and activities Assist with exercises, physiotherapy and other medical plans Monitor blood pressure and other physical conditions, as required Follow medical instructions carefully and consistently Assist with Bath Program Organize time and resources based on the individual needs of clients Manage any unanticipated events or unstable situations Delivers the Home Care Program in an appropriate, caring and respectful manner Identify persons requiring home care and client needs Ensure that all First Nations members needing care are identified Coordinate appropriate care and equipment including other community resources, as required Consult with family members and other supports to ensure that care is on-going and that all client needs are identified and met Encourage clients and families to be involved in care, if appropriate Encourage clients and families to take responsibility for care, if appropriate Liaise with all family, medical and other resources, as required Advocate on behalf of clients for additional service and resources, as required pg. 2
3 Report elder abuse and/or neglect All persons employed by the Okanagan Indian Band may be required to assist the Band in providing emergency services. Duties assigned during an emergency may differ from regular duties Participate in community events as requested ADMINISTRATIVE DUTIES Ensure that care is provided according to all relevant policies, procedures and regulations Coordinate Case Management Make recommendations for changes and improvements to the program, as required Report any concerns with the supervisor Provide information to other health care professionals, as required Monitor supplies and resources Educate clients, families and support systems about the Home Care Program what can be provided and when to access other resources Develop and maintain a current, accurate, confidential client reporting and filing system Attend meetings Able to attend and/or participate in training and education sessions (overnight stays may occasionally be required) Performs all duties and responsibilities in accordance with the OKIB policies and procedures Performs other duties as required QUALIFICATIONS Education Residential Care Aid Certificate (Minimum requirement) Palliative Care Certificate Current First Aid/CPR certificate with defibrillator or willing to obtain WHMIS and Food Safe certificates Valid BC Driver s licence, minimum Class 5 and reliable vehicle An equivalent combination of education and experience may be considered Experience Prefer minimum 2 years experience working as a care aid Drivers abstract clear of penalties Experience in a health related field pg. 3
4 Experience in janitorial or cleaning Experience working with First Nations communities Competencies (KSAs & Desired Attributes) Client assessment Appreciates others and values their diversity Effectively organizes, plans and implements care Charting and reporting abilities Ability to perform needs and risk assessments Knowledge and understanding of Okanagan culture and community life as a foundation to working with home care clients and their families Experience and understanding of Alzheimer s and dementia Desire to increase capacity through professional development Ability to work both independently and as part of a team Exceptional verbal and written and interpersonal skills Excellent problem solving abilities Knowledge of community services Ability to listen and demonstrate empathy Experience working with confidential information Possess cultural awareness and sensitivity Ability to exercise tact and diplomacy and maintain confidentiality Ethical, honest and respectful Able to be flexible in job duties Is self-motivated and assumes the responsibilities of the job Delegation of tasks where appropriate Intermediate level ability with Microsoft Suite WORKING CONDITIONS Physical Demands The is required to safely lift and move clients, and may be required to bathe and /or clean clients. The may be required to administer interventions and treatments that cause discomfort to the client or to themselves. The will often work in the client s home and must practice clean, safe care to eliminate the possibility of infection or cross contamination between clients, family members and/or themselves. The may be required with lift assist of clients for care plans and/or transport. pg. 4
5 Environmental Conditions The spends some time in their office, and some time in client homes. The may be dealing with patients with potentially infectious conditions. They must be extremely careful to ensure that all medications, treatments and activities are completed safely ensuring the safety of all clients, families, the community and themselves. The may be required to drive in winter conditions and make visits during periods of cold temperatures. Client s homes may include smokers, pets or other sources of allergens. There is a potential to encounter dogs during home visits. Sensory Demands The Home Care Aid will be exposed to sights and sounds associated with clients who are ill and injured. They may also find a variety of conditions and circumstances as they visit clients in their homes. The incumbent will be exposed to unpleasant odors in providing health care. The incumbent must make acute use of the senses in assessing health care needs such as hearing (listening for heart, lung and abdominal sounds), visual to look for symptoms of illness, touch in palpitation of clients and manual dexterity and smell to detect unusual odors from wounds and physical surroundings Mental Demands The Home Care Aid faces stress from dealing with clients and family members who may be stressed and difficult to work with. A factor that can induce stress is the Home Care Aid must work independently in unfamiliar and uncontrolled situations. There is significant emotional stress in providing care for clients who are dying and providing support for their families. Another cause of stress is the need to manage and evaluate staff and program issues while managing the diverse needs of the clients.
