11 The Future of Support for Living at Home Services formerly known as Domiciliary Care At present there is a combination of block and spot contracts. Analysis of this position has identified that things need to change! We will work in partnership to develop a new approach Walsall Support for Living at Home Services framework Person centred commissioning

12 Walsall Support for Living at Home Framework Unified approach to Domiciliary care, Community Support, Continuing Health Care Across all user groups Providers determine the groups they can support No block contract or guarantees of income Designed to maximise choice and control IB Re-ablement Based upon best practice from DOH Open ended so new providers can be added Based around PPQ and CQC registration No limit to the number of providers The level of support someone receives is based on their IB not hourly rates determined by the Council

13 What is Re-ablement? Re-ablement is an approach that can be offered to a wide range of people. Its main objective is to work with peoples strengths and offer them a range of opportunities with appropriate support to assist people to maximise their independence, well being and achieve a better quality of life. The aim of Walsall s Re-ablement Programme within Adult Social Care is to utilise a range of integrated services and equipment to: Promote faster recovery from illness Prevent unnecessary hospital admission Delay the need for admissions to long-term residential care

14 Person centered Commissioning When the service users support plan is achievable within the value of their Individual budget they are able to choose to be supported by any provider who has quoted against their support plan. This is the norm and our expectation for the vast majority of the people we support. If the service users support plan cannot be met by any provider within the cost of their individual budget then value and outcomes will be considered against provider quotations in relation to the potential award of a duration of transitional funding.

15 What does this mean for Providers in Walsall? There is an expectation that Providers will deliver their services in an enabling way Service will need to be delivered to achieve outcomes rather than focus on tasks. Providers will be asked to measure progress and outcomes and report this back during reviews
