2 Cornerstone LifeCare Purpose Cornerstone LifeCare LLC is a network of professional Partners who provide Home Building, Remodeling, Repair, Maintenance Services and Products for "Aging in Place" as well as comprehensive Personal Services for "Aging Well." Our primary focus is on the senior population of Sarasota, Manatee, and surrounding counties along with their families, care givers, and circle of supporters.
3 Cornerstone LifeCare Vision To provide living environments that deliver optimum conditions for those who choose to Age in Place as well as those who desire to Age Well in any type of surroundings. Our purpose is to help create and facilitate a lifestyle where individuals will Age with Dignity through comfort, accessibility, and safety. In addition to a focus on the physical surroundings, we offer comprehensive Personal Services by connecting with professional Partners who share the same goals of expressing compassion, love, and excellence.
4 Core Product & Service Strategies Cornerstone LifeCare will build and maintain a network of Partners Product & Service Providers -- that will serve Seniors and their supporters in area markets. For purposes of describing Aging in Place, the Center for Disease Control s definition seems appropriate: The ability to live in one's own home and community safely, independently, and comfortably, regardless of age, income, or ability level. While Aging Well relates to Cornerstone LifeCare s goal of providing programs, products, and services to enable overall improved Dignity and Quality of Life.

21 Cornerstone LifeCare Consultants (CLC) A CLC is a representative of Cornerstone LifeCare who is wellversed in the products and services of the organization and our key Partners. The CLC will be prepared to have discussions and, when scheduled, meet with clients to cover their needs and develop appropriate action plans. Potential client outcome might be in the form of a one-time experience or through ongoing consultative relationships. The CLC will typically receive contact from interested parties and either process the initial request from there or arrange for a personal appointment.
22 Cornerstone LifeCare Partners & Resources 1. Partners - Fully-vetted and committed organizations with ongoing marketing agreement with Cornerstone LifeCare resulting in exposure to the highest level of participation in terms of product/service offerings and marketing/lead referral support from Cornerstone LifeCare. Based on selected criteria, there will be a limited number of Partners in each Product/Service Category. Partners will pay a Monthly Marketing Fee to be a Member in the Network. 2. Resources - Organizations that are a part of the network with no financial commitment to Cornerstone LifeCare. They will primarily be represented on the Info Links of our website, but some may be featured through dedicated web pages.
23 Cornerstone LifeCare Marketing & Sales Process 1. Client makes contact directly with Cornerstone LifeCare through calls, s, our website (Contact Us Page), or personal contact. 2. Should client access any page on the Cornerstone LifeCare website that features a Partner, they will have three navigation choices in terms of follow-up communication. (a) Contact Cornerstone LifeCare Form (b) Contact Cornerstone LifeCare Partner Form (c) Go to Cornerstone LifeCare Partner Website
24 Cornerstone LifeCare Marketing & Sales Process (Cont d) 3. Cornerstone LifeCare support personnel will maintain records of all inquiries as well as distribution of referral leads to designated Partners. There will be a monthly reconciliation between Cornerstone LifeCare and Partners regarding referral leads. 4. In all cases, we will emphasize the benefits of having a discussion or scheduling a personal appointment with a Cornerstone LifeCare Consultant.
25 Benefits of Becoming a Cornerstone LifeCare Partner 1. Partner becomes a member in a unique One-Stop Resource network of individuals and businesses serving the Aging in Place and Aging Well markets in the most opportunistic area of the United States. 2. Cornerstone LifeCare will actively promote the Network and create lead generation for Network Members through Cornerstone LifeCare Consultants and a variety of means such as but not limited to Internet marketing, social media, direct mail, multimedia advertising, special events, and more.

Partner Profile - In order to ensure the highest integrity and reputation of the Network, Cornerstone will select, recruit, and include in its Network only service providers that will maintain or enhance the reputation of the Network. A service provider s eligibility for inclusion in the Network shall be based on the following factors: a. Common Values and Commitment to Network Initiatives b. Reputation and Client Feedback c. Types and Qualities of Services Offered d. Expertise and Experience in the Product/Service Category e. Geographical Representation f. Overall Capabilities and Capacity

Lead Referral System - Network-wide lead referral system where Network Members refer leads to one another - Tracking and Reporting System where Cornerstone LifeCare will document, track, and monitor all referrals for Network Members generated by Cornerstone LifeCare and/or the various Network marketing and promotion tools - On a monthly basis, Cornerstone LifeCare will provide Network Members with a lead report, which Cornerstone LifeCare will then discuss and reconcile with Partners.

29 Events/Speakers Bureau To support the Sales/Marketing Initiatives of Cornerstone LifeCare in general as well as the Partners and Resources, we will plan, coordinate, and implement participation in expos, seminars, showcases, symposiums, workshops, and other special events. We also will develop a Speakers Bureau as a way of introducing key Partners and related organizations to the marketplace.
30 Cornerstone LifeCare Client Fee Schedule 1. No charge for phone, , or simple response to Internet inquiry. 2. No charge for Aging in Place site visit that included CLC and Cornerstone LifeCare Home Building/Remodeling Partner. 3. Fee structure for a CLC Personal Consultation. 4. Fees for ongoing CLC contract (to be determined depending on level of services).
