2 Explosion FC Parent Questionnaire This questionnaire is completely voluntary and is kept confidential between parent and club directors. EFC is always looking for participants that can help EFC in various ways. This will help us, as a nonprofit organization maximize its goals, whether financially, logistically, or just to help us grow and remain Southeastern Massachusetts Premier Soccer club. Your help in these matters are greatly appreciated and go a long way in maintaining our Mission Statement. 4 EXPLOSION FC CODE OF ETHICS All Participants, coaches, and spectators will be bound by the following Code of Ethics while participating in Sanctioned events Players I will encourage good sportsmanship from fellow players, coaches, officials and parents at all times. I will remember that soccer is an opportunity to learn and have fun. I deserve to play in an environment that is free of drugs, tobacco, and alcohol; and expect everyone to refrain from their use at all soccer games. I will do the best I can each day, remembering that all players have talents and weaknesses the same as I do. I will treat my coaches, other players and coaches, game officials, other administrators, and fans with respect at all times; regardless of race, sex, creed, or abilities, and I will expect to be treated accordingly. I will concentrate on playing soccer. Always giving my best effort. I will play by the rules at all times. I will at all times control my temper, resisting the temptation of retaliate. I will always exercise self control. While traveling, shall conduct themselves so as to being credit to themselves and their team. Coaches/Managers I will never place the value of winning before the safety and welfare of all players I will always show respect for players, other coaches, and game officials. I will lead by example, demonstrating fair play and sportsmanship at all times. I will be demonstrate knowledgeable of the rules of the game, and teach these rules to my players. I will never use abusive or insulting language. I will treat everyone with dignity. I will not tolerate inappropriate behavior, regardless of the situation. I will not allow the use of anabolic agents or stimulants, drugs, tobacco, or alcohol by any of my players. I will never knowingly jeopardize the eligibility and participation of a student athlete. Youth have a greater need for example than criticism. I will be the primary soccer role model. I will at all times conduct myself in a positive manner. Coaching is motivating players to produce their best effort, inspiring players to learn, and encouraging players to be winners. Coach s actions on sidelines during games shall be in the spirit of "good sportsmanship" at all times. Profanity, profane gestures, arguing, inciting disruptive behavior by spectators and/or players, or any conduct not in the spirit of good sportsmanship, shall require disciplinary action from the affiliate. Alcohol, illegal drugs and unauthorized prescription drugs shall not be possessed, consumed or distributed before, during or after any game or at any other time at the field and/or game complex.
5 Parents/Spectators I will encourage good sportsmanship by demonstrating positive support for all players, coaches, game officials, and administrators. at all times. I will place the emotional and physical well being of all players ahead of any personal desire to win. I will support the coaches, officials, and administrators working with my child, in order to encourage a positive and enjoyable experience for all. I will remember that the game is for the players, not for the adults. I will ask my child to treat other players, coaches, game officials, administrators, and fans with respect. I will always be positive. I will always allow the coach to be the only coach. I will not get into arguments with the opposing team s parents, players, or coaches. I will not come onto the field for any reason during the game. I will not criticize game officials. Alcohol, illegal drugs and unauthorized prescription drugs shall not be possessed, consumed or distributed before, during or after any game or at any other time at the field and/or game complex. Within the above framework, there may be disagreement and confusion on behalf of coaches, managers, players and parents. In these cases, players are asked to approach coaches in an attempt to resolve any issues. Although this may be hard for a player, we feel adopting this policy only serves to help individuals mature. The situation, of course, is different for the younger members of the club. In situations where the player is too young to deal with an issue, and it is necessary for a parent to get involved we ask that parents follow these guidelines. If a parent has questions for a coach, please: Call them or make an appointment to meet. Please remember that most coaches work for EFC on a part-time basis, and therefore, spend most of their time on their individual careers. However, they do realize that part of their job with EFC is to communicate effectively with parents if situations arise, and they are prepared to do this. If you have a question regarding something that has happened during a game, parents are not allowed to approach the coach until 24 hours after the game. If you have something to say to the coach, please make sure that your comments get to the coach. Do not voice your opinions on the sidelines. The club will not tolerate constant complaining and degrading on the sideline. Any parent who is persistent in these actions will be asked to leave the club. If, after meeting with a coach, an issue is not resolved, you may contact the EFC executive director via at d102928@aol.com Every effort will be made to reply to you in a timely manner.
6 Failure to comply in the aforementioned rules may result in the suspension of your privilege to participate in EFC sanctioned events, for the following periods: 1st offense suspension minimum thirty (30) days to a maximum of one (1) year 2nd offense suspension for a minimum of one (1) year to a maximum of Five (5) years 3rd offense suspension for a minimum of five (5)years Player Parent
7 League Affiliations: Explosion FC participates in the Maple League, Masc League, New England Premier League and Region one league. These leagues provide the highest level of competition in Massachusetts. These leagues offer several divisions, which are based on team performance from season to season. Games are played mostly on Sunday s with some Saturday s during the spring and fall season. EFC also plays indoor soccer and trains during the winter months. Responsibility of Players: Players are expected to have high discipline to create an intense and fundamental practice. Explosion FC players should demonstrate commitment to all practices as well as games. Attendance to practices will be taking into account for inclusion, by coaches to game lineups and participation. This will be at the sole discretion of the coach and will not be negotiable. Explosion FC Players are not allowed to participate with other soccer clubs. Participation in town-of-residency leagues is permitted as well as school teams. Players must be made available to play on an EFC age appropriate team for their age bracket. If they are asked to play in a non appropriate bracket one season then they may be moved to an age appropriate bracket the next. All at coaches/clubs discretion. Players and their parents are expected to follow the EFC financial policy as laid out in this contract. Failure to follow guide lines could result in suspension and expulsion from the club. Coaches discretion on excused absences. No absence excused for no show no call games or practices. Unexcused absences could result in loss of playing time, or suspension. Players should arrive at practice in clean and neat club issued training gear. Also player should always bring the alternate jersey. Players should arrive at games in clean and neat club issued game gear. Also player should always bring the alternate jersey to every game. Players and parents are expected to read and follow the EFC code of ethics. Every player is expected to purchase the current club practice and training attire. Players are expected to wear appropriate attire to all games and all practices.
8 ALL PLAYERS REGISTRATION, CONTRACT, BIRTH CERTIFICATE, 1x1 SIZE PHOTO AND DEPOSIT of FIRST PAYMENT IS DUE ON OR BEFORE REGISTRATION DATE LISTED ON EFC WEBSITE REGISTRATION TAB. PLAYERS WILL HAVE 24 HRS TO ACCEPT THIS CONTRACT OFFER TO PLAY AFTER IT HAS BEEN MADE. POSITION WILL BE OFFERED TO ANOTHER PLAYER IF YOU FAIL TO ACCEPT POSITION BY THIS TIME FRAME. ALL PLAYERS u10-u14 ARE EXPECTED TO PLAY IN WINTER MONTHS AND NO DISCOUNT WILL BE ISSUED FOR TIME OFF, VACATIONS, OR INJURY. Players from RI must have RI release in hand when turning in paperwork. See EFC web site registration tab for details. Players that were not born in the United States need to complete International clearance waivers per US Soccer policy under the direction of FIFA. These policies are stringent and time consuming. If Player is u16 and above there could be a 30 day wait period. You must notify your team manager if your player was born outside of the United States.
9 Club Services: The club is committing to signed players to train the players and make the players better soccer players. Efforts will be made by coaching staff to get players into games and see playing time. However, as with any competitive club sport there is absolutely no playing time guaranteed to any player by signing with EFC. If player is signed as a goal keeper they are not guaranteed field play time. On occasion they may have this opportunity. Quality coaching as determined by the EFC Executive board Quality competition Commitment to players comprises any of the clubs aforementioned sanctioned leagues for the fall (u14 and below) and spring season (all teams) including outdoor practices (2xper wk) at one of the clubs several fields. In addition, indoor training (1xper wk) will be provided during the winter months for all teams. Game day is on Sunday for spring and fall season. There will be occasions that ALL TEAMS will participate in programs, tournaments, and additional training sessions that the costs are not covered in the terms of this contract. These programs costs are the responsibility of the parents of players and will be collected for when team signs up for them. Sunday Futsal and/or winter league games payments are NOT covered under the terms of this contract. EFC s sole purposes is to make better soccer players.
10 Club Fees: Fee Agreement: Upon acceptance of a roster position on a team for your child, you agree to pay the yearly fee for your child s particular age group. It is imperative that all scheduled payments are to be made in a timely manner as described in the payment schedule you choose online. There is a budget plan option with auto debit or credit. See pages below for details. Failure to meet a payment deadline may jeopardize your child s position on the team. No player will start the Fall/spring season without full payment, agreed to budget plan or without the authorization of the EFC management Executive board. Explosion FC has a no refund policy on monies paid for any reason. Late fee could be charged if payments not made on agreed upon dates. ALL NEW PLAYERS, $125 uniform fee for this season ONLY! This is the clubs last season with it s current Nike uniform. Next fall All players in the club will be required to purchase a NEW full uniform package as we renew our contract with our uniform manufacturer. Therefore we are only requiring the basic of uniform packages. You may purchase the full package if you desire. All uniform purchases must be made at registration once you accept position on club. The basic package $125 Home Jersey, Away Jersey, Training Jersey, Home shorts, away shorts, 2 pair of socks. The full package. $350 Home Jersey, Away Jersey, Training Jersey, Home Shorts, Away Shorts, Practice Shorts. 3 pair socks, Warm up pants, practice ball, Warm up Jacket and back pack. Cost of uniform includes all embroidery/screening and personalization.
11 Additional fees will be required to cover for additional optional tournaments at Coaches/clubs discretion not described. Also, Players are expected to pay all coaches fees for all extra tournaments. This fee to include transportation, lodging and meals. Players amount that they are responsible for will be reviewed with players and parents prior to tournament by team manager/coach. This fee will vary considerably by team level of tournament and participation and by skill set. This amount is to be paid by the players, directly to the team manager via cash or check. These fee s are NOT able to be paid via credit card thru the EFC online financial management system. All registration payments MUST be made by credit/debit card or via our online financial management program. The link for this is located at the Explosion FC website ( ALL CLUB PAYMENTS MUST BE MADE ONLINE VIA SYSTEM. EVERY FAMILY WILL HAVE THEIR OWN ACCOUNT TO MANAGE. NO CHECKS ACCEPTED FOR REGISTRATION PAYMENTS. RECURRING PAYMENTS WILL BE SET UP FOR ALL PLAYERS IF REQUESTED DURING YOUR ONLINE REGISTRATION. A ONE TIME ONLINE PAYMENT (WITH DISCOUNT) WILL ALSO BE AVAILIBILE THRU THIS SYSTEM.

Train min. twice per week with various club coaches on fundamentals of game (fall & spring - outside) team training once per week with various club coaches on fundamentals of game (winter - indoor) included additional night of indoor training focusing strictly on strength and conditioning. Development of technique, footwork, passing, defense, dribbling and game tactics. Sessions of soccer classroom theory during outside rainy day soccer practices at our Camp Finberg facility. One spring, one fall tournament at clubs discretion. Again, additional fees will be required to cover for additional optional tournaments at Coaches/clubs discretion not described below. Also, Players are expected to pay all coaches fees for all extra tournaments. This fee to include transportation, lodging and meals. Players amount that they are responsible for will be reviewed with players and parents prior to tournament by team manager/coach. This fee will vary considerably by team level of tournament and participation and by skill set. This amount is to be paid by the players, directly to the team manager via cash or check. These fee s are NOT able to be paid via credit card thru the EFC online financial management system. EFC DOES NOT OFFER REFUNDS OF FEES FOR ANY REASON.

15 -club will offer $75 discount to all players that pay 100% of their club fees upon accepting their position to play. Parents opting for this program will make FULL payment online and once payment posts to your account EFC will credit your account 10% of your registration fees (uniform fee s are not discounted) -club will offer a budget plan payment option for you to choose for your convenience via our online payment management system. Upon accepting a payment plan you must login to EFC financial management systems secure online server and set up your plan. Your plan will require you to set up your EFC online account with your credit or debit card and your account will be auto debited monthly. Online payments are the ONLY option for payments this season. No Checks!! Budget payment plan. 20% down payment and 9 equal monthly installments of remaining balance.. The plan will have a $5 convenience charge added to each payment to use the budget plan.
16 EFC will offer a family financial aid program to help defer the cost of registration for families in need. There is a separate application for financial aid. This application will be located on page below. Explosion FC Financial AID program EFC is a 501c3 organization Explosion FC offers financial aid to club members in need of assistance. This program is based on a sliding scale and will be approved or declined based on the needs of the family. This application must be completed in it s entirety, or it will NOT be considered under any circumstances. For consideration please supply all requested documents as agreed upon in full by the parent(s) or guardian(s) of the applicant player. A player previously receiving financial assistance does not automatically mean that player will be granted financial aid in this and future years. Not all families that qualify for financial aid. There is a limited amount of financial aid available for each club team. EFC will require ALL financial aid recipients to agree to reciprocate to the club and participate in club needed activities. These may include field lingning, nets, goal maintenance. May include help with camp Finberg or other facilities general upkeep or events. ALL financial aid recipients are expected to participate in ALL club related fundraising events at 200% minimum. If the club is selling tickets and players are asked to sell 10, a scholarship player will be required to sell 20. If player receiving aid is not 100% Paid on their portion of their financial responsibilities or does not
17 participate in the aforementioned items above then EFC will rescind the financial aid and the player will be responsible for 100% of their dues. Any financial aid recipient not paying their dues in this manner will dismissed from the club. 19 ******this application, if approved will be considered complete and final at end of current soccer year. Player parent/guardian must resubmit application each soccer year for future consideration. This application is considered confidential and will be viewed by EFC directors only. Documents will be destroyed after decisions are rendered. By signing this application above you are agreeing to allow the EFC management team to view your documents and render a decision of scholarship arrangements. You also are agreeing to all the rules and regulations as outlined in the Scholarship cover letter and will participate in fundraising efforts vigorously. You also agree that you have submitted all information truthfully to all questions enclosed in this application. Any members found to be untruthful that are offered scholarships will have said scholarship privileges revoked immediately and may cause immediate expulsion from club.
