3 How do the free assessments work? Health and education assessments check for any health and learning problems you may have that might be making your life hard. At the end of the assessments the assessor will make suggestions about things that could be done to help you. They will also organise for you to see a specialist if they think that is a good idea. The suggestions will help people consider the best options to help you. assessments

PAUL, from Child, Youth and Family Ben, from down the road I want to talk to you about health and education assessments. Yeah, why? Sometimes young peole have health or learning problems that haven t been picked up before. they help everyone understand stuff that might be making your life hard. Like what?

5 Well, one girl had a problem with her hearing. She d been feeling down, with lots of headaches and pains. This made her really angry and she started taking it out on people. Well, duh Yeah, but it can be simple things that make a huge difference to your life. One boy was encouraged to go to a school that suited his needs better. Now he s a Kapa Haka leader and enjoying school. OK, how do assessments work? Like what do these health guys do? Health assessors are a doctor or nurse. They check your skin, diet, eyes, hearing and make sure you re not feeling sad or down. What s the education thingy? We ask how you re going at school, look at how you learn and how learning could be better. What happens with the stuff I say? The doctor or nurse will write a report about what they find, and come up with suggestions about what help you might need. Private stuff talked about by you and the assessor won t be shared unless it is not ok to keep it secret.

6 So whaddya say, give it a go? Nah, it must cost heaps! My family couldn t afford it. No, it s all free. True? No cost and might make my life better OK, nothing to lose where do I sign? After you agree to the free assessment, we ll contact you and make a time to meet the assessor. They ll talk to you about all sorts of stuff at home and school. They may need to examine you, but they will make sure you know what they need to look at and why and check that you are ok about it. CYF170

7 assessments Health and education Assessments Informed consent for young people Thanks for agreeing to a health and education assessment. This assessment will help identify ways to keep you healthy and well. If you have any questions at any time, talk to your social worker. Consent of young person I give my permission to have a health and education assessment. I understand this means: a) A doctor or nurse will carry out a full health check-up on me. b) In preparation for the assessment, the doctor or nurse will ask other professionals who I have seen in the past information about me that may be helpful in understanding about my health. This may include people like health clinics, school, social workers, counsellors, ACC and my parents or caregivers. c) My teacher will provide information about how I m doing at school, and will share this information with the doctor or nurse and my social worker.

8 d) Information from the assessment will be shared with my social worker and may be discussed at my family group conference. Some of this information may also be shared with my health provider, teacher, guardians and legal counsel. e) I can talk to the doctor or nurse about the report before they send it to anyone. I will be able to stop any information being included that I do not want others to know (unless it impacts on my safety or that of others). 9 The assessor may also talk to your parent or guardian and other important people who may know information about you that could be helpful. Sometimes you may not be aware of some things yourself. You can talk to the assessor on your own, and so can your parent or guardian. The assessor will talk to you about what they have found and you can discuss with them what you want other people to know. You can include everything if you want to, and often this is really helpful.

11 education Education We look at how you learn, your progress in school and how school can best help. This can involve: looking at your strengths and learning skills looking at how you get on with others talking to your teachers, or other people who might know how you re going at school. > Health Health assessors look at your body, how it is working and things that might be worrying you, including how you get on with others. This can involve: health reviewing your health history talking to you about things like home, school, activities, alcohol and other drugs, and sex checking out your body including your skin, eyes, ears, teeth and other things talking to your family/whänau or other important people in your life asking a specialist to see you if they could help with any issues.

12 > Do I have to pay? The assessment is free, and if you have trouble getting to see the assessor, we can also help arrange transport. Can i bring someone? You are welcome to bring an adult with you; just let us know beforehand. Will my information be confidential? The things you talk about with your assessor are private; they don t tell anyone without your permission except if there are worries that you or others are not safe. Do I have to agree to an assessment? It s a great chance to have a check up by people who understand issues faced by young people. If you re unsure about signing the consent form, ask us to explain it to you. Other questions? If you have any other questions or concerns, please ask us to explain it to you.
