3 Willamette Family s Buckley Services has been licensed by the Addiction and Mental Health Division of the Oregon Health Authority since the early 1960 s to provide Alcohol and Drug Detoxification Services. Buckley Services is the 2nd largest Detoxification program in the State of Oregon, with a 22 bed capacity. Buckley Services is uniquely situated to become a regional provider of ASAM Level III.7 D services with our geographic location in the center of the State and along the Interstate 5 corridor. We are the southernmost Detoxification Program among the existing Detoxification programs in the State of Oregon.

4 Levels of Care American Society of Addiction Medicine (ASAM) Placement Criteria for the treatment of Substance-Related Disorders (PPC) ASAM Detox Level Level II-D Level III.2-D Level III.7-D Care Setting Ambulatory/Out patient with extended monitoring conducted in general health care facility to determine more or less intensive detox services. For mild to moderate withdrawal, individuals monitored over a period of several hours. Clinically Managed/Non-medical or social detox setting, conducted in an appropriately licensed health care or addiction treatment facility. Sub-acute: deals with severe symptoms such as cravings, Acute: stabilized as outpatient deals with severe withdrawal syndrome. Both acute and sub-acute warrant 24 hr. support from Peers and/or counselor. Medically Monitored detox conducted in a freestanding detox center. Individuals are medically managed and stabilized in a licensed health care or addiction treatment facility. Withdrawal includes severe physical and psychological symptoms that require medical management with medications and 24 hour medical care from medical professionals.

5 On May 29th 2012, Buckley Detoxification Services enhanced our level of care to fully meet requirements for ASAM III.7-D, Medically Monitored Inpatient Detoxification services: Medical Director, Dr. Nicholas Telew, M.D. Psychiatry Physician Assistant, John Brandon, PAC Nursing Manager, Donna Salata, RN Registered Nursing Staff on 24/7 Director Clinical Supervisor Tom McKee, CADC II 3- CADC Case Managers

6 Medical/Pharmacological Services At this level of care the following medical services are provided: Addiction focused medical history A physical examination Appropriate laboratory and toxicology tests Physician Assistant daily rounds Hourly Nurse monitoring of progress and medication administration and withdrawal support Narcotic withdrawal protocol buprenorphine assisted pharmacological intervention Alcohol withdrawal: CIWA (Clinical Institute Withdrawal Assessment) scale Narcotics withdrawal: COWS (Clinical Opiates Withdrawal Scale) scale

7 Psychosocial Interventions CADC staff provide all services in the addiction treatment milieu. Including: A planned regime of 24 hour evaluation care and treatment A full ASAM bio-psycho-social screening assessment An Individualized Treatment Plan Daily assessment of progress through detoxification Discharge /Transfer planning beginning at Admission Referral arrangements made as indicated Individual and group therapies provided as clinically indicated Documentation through progress notes

8 Accessing Services and Hours of Operation Buckley Services is open 7 days a week, 24 hours a day. Services can be accessed by following a simple 3 step process: 1. Patient calls or visits to complete a Pre-Screening Assessment 2. If the patient meets our criteria for admission an entry time will be scheduled (Bed held of 12 hours for travel if out of County) 3. Patient completes a Nursing Pre-Screening and with RN approval the patient will enter our program a) Monday through Fridays 8am to 4pm a Sub-Acute Detoxification Case Manager is responsible for all OHP/Private Insurance pre-approvals and communication between Buckley and the OHP/Private Insurance case managers.

9 Detox s Stabilization Role in the Addiction Treatment Continuum of Care 1. Research indicates that medical detox interventions linked directly to SUD treatment situated in a comprehensive continuum of care is the most effective method for increasing recovery success. 2. Medically monitored interventions prevent potentially life-threatening complications for those individuals who indicate risk associated with abrupt abstinence from prolonged substance use. 3. Stabilization includes medical and psychosocial processes of assisting the client through acute intoxication and withdrawal to the attainment of a medically stable, fully supported, substance-free state. 4. Referral arrangements made as indicated. All patients completing Detoxification will have an appointment scheduled with an appropriate addiction treatment provider of their choosing prior to discharge.

10 Service In an effort to better serve the patient and improve their outcome from the Detoxification Addiction Treatment episode Buckley Services is attempting to work closely with the Patient's Primary Care Physician, and Insurance Case Managers by following the below Protocol when appropriate releases of information are in place.
