3 Welcome to your Wellness Programme Your Wellness Programme Without doubt one of the most important decisions you can make, for your own benefit and that of your business, is to become a member of a quality Wellness Programme. We are delighted to introduce Nuffield Health as our health partner. With you for the journey Nuffield Health take a highly personalised approach to the complete physical and physiological health and wellbeing of each Business Leader. Your journey with them will be shaped by their team of expert clinicians, at the Tesco office in Cheshunt, in partnership with you. They will review your full medical history, lifestyle, diet and pathology results (blood screening). Health Assessment 6 Month Follow Up Healthscore TM Nuffield Health will then use these outcomes to advise you on the most appropriate course of action to improve your health and wellbeing, including a 6 month follow up appointment and where relevant a series of follow-up sessions and touch points they have agreed with you, to support and promote your optimal wellbeing. As part of the programme you will also be supported by Healthscore TM, Nuffield Health s online and mobile fitness and lifestyle tool that combines expert knowledge about fitness and wellbeing with the latest smart technology. With it, you can monitor your health and progress anytime, anywhere. Access to Additional Modules 24/7 Private GP access 4 5
4 The four key components Your Health Assessment You will attend our newly created Health and Wellbeing Centre in the Tesco office in Cheshunt, for your Health Assessment. This may take up to 3 hours and will consist of a number of components or modules. Working with the clinical team, you will discuss to what extent you will utilise and benefit from the various elements of your Business Leader Wellness Programme. Your physician will guide you through the clinical tests that are available and relevant to you. Physiology Session 1 Hour Physician Session 1 Hour Physiology Health 1 Hour 6 Month Follow Up Physiologist (30 minutes) Doctor (30 minutes) Making decisions and handling change requires responsiveness. Meeting your ongoing and changing health needs requires it too. Lifestyle measurements including resting ECG testing, discussion on lifestyle behaviours Full physical examination, review of pathology and genetic profiling Exercise ECG (if clinically indicated) and a goal setting session on lifestyle behaviours and appropriate actions To discuss progress, repeat any appropriate tests and re-evaluate action plan 6 7
5 Your Health Assessment Your Personalised Wellness Report Nuffield Health undertake a comprehensive series of tests to help gain a true understanding of your health and potential risks. Detailed lifestyle questionnaire Height and weight measurement Body mass index Body fat percentage Waist measurement Nuffield body composition index Hydration levels Urine analysis Blood biochemistry and haematology profile Blood test for glucose and cholesterol Measurement of general nutritional status Computerised Spinal Assessment Lung function test Blood pressure Physiological measurement of resilience to stressors Resting ECG Dynamic Cardiovascular Assessment (if clinically indicated) Assessment of fitness level and Cardiovascular risk score Bowel cancer test (for those aged 45 and over) Testicular examination Prostrate cancer blood test (for those aged 50 and over) Breast examination Pelvic examination and cervical smear Genetic cardiovascular screening High vaginal swab (if clinically indicated) Thyroid stimulating hormone blood test (females aged 50 and over) Mammography (where clinically indicated) Within 10 working days of completing your Health Assessment, you will receive a detailed and comprehensive report on your health and wellbeing. This will incorporate your agreed Personalised Wellness Programme, plus details of any followup sessions and key touch points based around your own needs. Following your Health Assessment an appointment will be scheduled in for 6 months time for your follow up sessions with a physiologist and doctor. These sessions will enable further discussion on your health and wellbeing, re-assessment of some of your health risk factors where appropriate and agreeing further action planning. As part of your Business Leader Wellness Programme, where clinically indicated you will be able to access a number of specially designed modules which will target specific areas of health and wellbeing including fitness, nutrition, resilience and musculoskeletal concerns. Throughout the 12 month programme you will also have 24/7 access to a dedicated GP hotline. 8 9
