16 2015/16 COLLEGE CREDIT PLUS (CC+) ACADEMIC ADVISING PLAN This form must be completed with the guidance counselor at your high school and include all signatures. New CC+ students will register by presenting this form at orientation. If you need to speak to an advisor prior to attending orientation, appointments are available at the Darke County Campus and on the Main Campus by calling Students who were PSEOP in 2014/15 Due to new legislation, ALL students must now meet with an advisor. You will be assigned an Edison advisor. This form must be completed with your high school guidance counselor each semester for CC+ classes, and be presented when you meet with your advisor. Students, who need to make schedule changes, must submit an additional form at the time of the requested change unless the change is a section change only.
