The JobCo PHaMs Employment Program servicing the Moreland area is located at level 1, 175 Sydney Road Brunswick on the corner of Michael Street Using public transport: TRAM - Catch the 19 Tram along Sydney Road to either the Number 20 stop Barkley Square or the Number 21 stop Glenlyon Road. TRAIN Upfield line, Jewell Station walk to Sydney Road, turn left and walk North a little over 200metres. Newsletter Brunswick PHaMs sends out a Monthly Newsletter that provides information about all the activities PHaMs, CRG and other JobCo. programs are doing that month. As well as activities in your local community, recipes and healthy living tips! We would love your contributions! The PHaMs Employment Program: Welcome to the Personal Helpers and Mentors (PHaMs) Employment Supports people whose lives are or have been severely impacted by mental illness. Focuses on strengths Focuses on recovery; we believe that people who experience a mental illness can lead a fulfilling life with the same opportunities as others in the community Assists personal capacity for self-reliance Assists in goals that relate to employment Is flexible and driven by you Offers counselling at no charge What does PHaMs Employment do? Page 9 Working directly with you and providing support to address issues in your life that have been identified as barriers to employment Supporting your family and support networks as needed to ensure they understand and support your transition to work. Communicate with clinical and primary care providers to ensure they are aware and supportive of your employment goals and tailor treatments accordingly Assisting you to navigate employment services and Centrelink systems, including referring you to appropriate employment services and accompanying you and advocating at appointments and assessments. Being available to both participant and the employer to assist if circumstances change and/or a participants job is in jeopardy ( example, the person has an episode of their mental illness) Providing on the job support as necessary, to assist you to maintain a job

4 Working with PHaMs Complaints and Grievances: Once you have been accepted into the Program, your Support Worker will meet you regularly in a mutually acceptable location. If you are not happy with the service you are receiving you can make a complaint in a number of ways: Your Support Worker will spend time with you to understand your needs from which a full assessment will be completed with your participation. Talk to someone you think might be able to assist you with making a complaint Your Support Worker will assist in developing an Individual Recovery Plan that will outline areas on what you wish to work. This plan can be changed at any time and is designed by you, for you. Your Support Worker understands that recovery is an individual process and that the needs of participants may vary over time. Therefore, your Support Worker will negotiate with you the level of support that will be provided to you, and this level may be reviewed at any time. While you will work closely with your key Support Worker you will also benefit from the assistance of other Support Workers on a regular basis. In addition to your PHaMs employment program, you may also be referred to other agencies to ensure you receive specialised service when you require it. Your needs and goals are discussed as part of your Individual Recovery Plan. The PHaMs Program will be available to you until your needs and recovery goals are met. If you are happy with our service we would like to know, however, the Manager, Support Workers and other staff are unable to accept gifts or money from program participants, their family or carers. Speak directly to your Support Worker or another staff member Complete a Grievance Report Form and hand the form to your Support Worker or address it to the PHaMs Manager at the JobCo office you attend. Grievance Report Forms are available at the reception desk of your JobCo office Ask someone you trust to advocate on your behalf Call the formal complaints service of the Government department that funds the Program, (DSS), on Feedback: To ensure that you are receiving the service that you want, we will ask you regularly to provide us with your feedback. You may also provide feedback at other times, whenever you feel the need. We would like to know how you feel about the service you are receiving. Feedback forms are available at the reception desk of your JobCo Office or by asking your support worker. Page 3 Page 8

5 Confidentiality and Privacy: Your privacy is protected by law; in order to deliver the highest quality service we will ask you to share information about yourself; however we will protect your privacy in the following ways: While collecting such personal information, PHaMs staff will comply with the requirements of the Privacy Act (The Privacy Act of 1988, establishes regulations for the collection, storage, use, sharing and disposal of, your private information). The information we collect will be used only for the purpose of delivering our service to you The PHaMs Program will only release or share your information with your signed and/or verbal consent Your information is kept in your file and stored in a locked cabinet on secure premises You may request access to your file under the terms of the Information Privacy Act To gain access to your file you will need to speak to your worker and make a formal application. What does Strengths Based mean? Strengths-based means that by meeting and talking with you we will assist you in the identification of your strengths, aspirations and goals that you may not be aware of. An individual s strengths may be described in physical, personal and environmental terms. Examples of these include but are not limited to: Intellectual, physical and interpersonal skills Personal interests and motivations Family, friends, colleagues, neighbours Dreams, aspirations and desires During your meetings with the PHaMs Support Worker and following the identification of your current and emerging strengths and taking into account your social, cultural and personal difficulties you will be able to have a firm action plan that includes achievable goals. In addition to the above, you and your Worker will discuss your values around change and how these can assist you to obtain the goals. Page 7 Page 4

6 Your Responsibilities Your Rights: Treat all staff respectfully and courteously Respectful and courteous treatment To provide the information we require to deliver the highest quality service to you Ask questions if there is something you don t understand or if you require further information Privacy and confidentiality To be provided with information that you understand Service that is safe and supportive Actively engage and commit to the process of recovery To be involved in planning, decisions and evaluation Keep appointments with workers or let us know if you need to change or cancel an appointment Refrain from using non prescription drugs or alcohol prior to meetings with your Support Worker To be self directed Respect for cultural, religious and political difference To make a complaint or air grievances. Cooperate with requests relating to the comfort and safety of others Refrain from verbal and physical abuse as staff will not work in an offensive environment If we visit your home requests to remove animal etc should be adhered to JobCo. is a non smoking environment, please do not smoke during support worker visits, unless in an appropriate environment Page 5 Page 6
