2 ADVENTIST HEALTH HOME CARE SERVICES Adventist Health has provided safe and effective, in-home medical and personal services since Our qualified team of registered nurses, physical therapists, occupational therapists, speech therapists, medical social workers and certified home health aides work with physicians to help patients recover in the comfort and privacy of their own homes. Adventist Health is our hospital s agency, which means we have access to resources for patients that are not available at other home care agencies. Accredited by The Joint Commission with Medicare certification, Adventist Health Home Care Services meets or exceeds all requirements of the State of California Department of Health Services. We work with physicians to customize treatment programs to meet individual patient needs. 4 TELEHEALTH SERVICES The Adventist Health Home Care program is pleased to offer Telehealth as a resource for physicians to closely monitor patients in the comfort of their own homes. It is a convenient program to help patient health care teams keep track of daily clinical information. 5 PERSONAL CARE SERVICES Adventist Health Personal Care provides customized care for your family s convalescence, chronic illness, maternity or child health care needs. You will have peace of mind knowing that your loved one is receiving the personal assistance they need. Family members can continue in their careers and focus on their responsibilities without the loss of productivity or income and the sense of feeling overwhelmed. All care givers are supervised by a registered nurse and extensive background checks are performed to ensure safe and compassionate care for all patients. Services include: Personal daily living care such as grooming (dressing/bathing), meal preparations, light housekeeping, grocery shopping and prescribed exercises or short walks Blood pressure and vital sign monitoring Homemaking and home safety services 24 hour live-in service by the day, week or month Over-night attendants Hourly attendants (4-hour minimum) Adventist Health Personal Care accepts private pay, private insurance and worker s compensation.
