5 Title: President Smithville Telephone Company, Inc. Tariff I.U.R.C. No. 1 Original Sheet 2.1 NONRECURRING CHARGES- ALL EXCHANGES Application of Nonrecurring (Installation) Charges 1. SERVICE ORDER CHARGES The Service Order Charge applies for the operations required to generate and process the service order. The charge applies on a per request basis rather than per item. The charge is not applicable to the removal of services from the customer account. There are three types of Service Order Charges: A. New Service - The New Service Order Charge applies for the operations required to generate and process a service order to establish a new or separate service account with the telephone company. This charge also applies to outside moves and restoration of disconnected service due to non-payment status over 60 days. B. Subsequent Additions and Changes - The Subsequent Additions and Changes Charge applies for the operations required to generate and process a service order to add or change the services provided on an existing customer account. The charge applies to, but is not limited to, orders for change of name, change of address, and addition of call features. This charge also replaces the service order charges previously noted for change of number and Vacation/Suspension service. C. Reconnect (Non-Pay within 60 days) - The Reconnect Charge applies for the operations required to generate and process a service order to restore service to a customer's line when it has been suspended for non-payment. If the service has been suspended for over 60 days, a New Service Order Charge will apply rather than the Reconnect Charge to restore the service. 2. CENTRAL OFFICE CHARGE - The Central Office Charge applies for the operations in the central office required to perform the work necessary to complete a service order. The charge is applicable per service order per line. The charge is not applicable to name and address changes or to remove services from the customer's account. Effective: Officer: William H. VISIT TO PREMISE CHARGE - The Visit To Premise Charge applies for the operations required to perform work at the customer premise. The charge is applicable for new installations when a telephone company technician completes work at the customer premise, and for repair visits when the service problem is found not to be in the telephone company-owned facilities. The charge applies per visit. 4. RETURN OF BAD CHECK - The Return of Bad Check Charge applies to each check returned to the telephone company office as not paid by the customer's financial institution. It applies per returned check. 21 11 th Revised Sheet 17 DUAL PARTY RELAY SERVICES SURCHARGE 1. DESCRIPTION Pursuant to the requirements of House Enrolled Act 1608, all customers billings on and after June 1, 1991 shall reflect an additional charge to fund the Indiana Telephone Relay Access Corporation for the Hearing and Speech Impaired (InTRAC). The purpose of InTRAC is to deliver a statewide dual party relay service for Indiana s deaf and speech impaired citizens that will provide access to telephone services that are functionally equivalent to those provided to individuals not having hearing or speech impairments. The relay will be the link between a hearing or speech impaired person using a telecommunications device for the deaf and a non-impaired person using a telephone. 2. CONDITIONS This surcharge will apply to all access lines excluding WATS, Public Coin, and Company Official Lines. This charge is subject to change pursuant to the procedure set forth by House Enrolled Act CHARGES Monthly Rate Business and Residence InTRAC surcharge, per line $0.03 (R) Effective: April 1, 2005 Title: President
22 Original Sheet 18 END USER ACCESS SERVICE The Telephone Company will provide End User Access Service (End User Access) to end users who obtain local exchange service from the Telephone Company under its general and/or local exchange tariffs. 1. General Description End User Access provides the ability for use of the End user Common Line (T) (EUCL), ie; the loop, and the portion of the switch required to allow end users access to the public switched network. This allows end users access to services such as emergency services, operator services, directory assistance and interexchange service. Access to interexchange service is the ability of the end user to interconnect with interexchange and toll services and is not subject to carrier access charges. (T) 2. Limitations 2.1. Exclusions Telephone number, detail billing, directory listings and intercept arrangements are not included with End User Access. 3. Undertaking of the Telephone Company The Telephone Company will provide End User Access at rates and charges as set forth in 7 following, as follows: (T) (T) (D) (D) (T) (T) 3.1. Use of an EUCL for intrastate Access Services provided under this tariff. (T) Such use will be provided when the end user obtains local exchange service. (T) 3.2. 23 Original Sheet 19 END USER ACCESS SERVICE (Cont.) (S) 4. Obligations of the End User 4.1. When the end user is a Radio Common Carrier (RCC), or provider (C) of paging services, such end users shall designate whether the local exchange service they are provided by the Telephone Company is used as access line for RCC or paging services, or used as an administrative line. (C) (D) (D) 5. Payment Arrangements and Credit Allowances 5.1. Minimum Period The minimum period for which EUCL End User Access is provided to an end user and for which charges are applicable is the same as that in the general and/or local exchange tariffs for the associated local telephone exchange service Cancellation of Orders (T) End User Access is canceled when the order for the associated local telephone exchange service is canceled. No cancellation charges apply Changes to Orders When changes are made to orders for the local exchange service associated with End User Access, any necessary changes will be made for End User Access. Allowance for Interruptions When there is an interruption to an EUCL, requested End User Access credit allowances for interruptions will be provided as set forth for credit allowance for interruptions in Section 2. C. 2. preceding Temporary Suspension of Service When an end user temporarily suspends its local exchange service which is Associated with EUCL, one-half of the EUCL per month charge will be Temporarily suspended for the time period the local exchange service is suspended. 6. Rate Regulations 6.1 Who is Billed EUCL per month charges will be billed to the end user of the associated local exchange service 6.2 Residence Service When an end user is provided local residence exchange service(s) in a state, (N) and when the local residence exchange service is provided under the general and/or local exchange service tariffs, the EUCL Residence Individual line or or trunk rate as set forth in 7.1. following, applies to each such local residence exchange trunk. Business Service - Single Line When an end user is provided a single local business exchange service in a state, and when the local business exchange service is provided under the general and/or local exchange service tariffs, the End User Common Line (EUCL) Single Line Business Individual line or trunk rate as set forth in 7.2 following applies to each such business individual line or trunk Business Service - Multiline When an end user is provided more than one local business exchange service in a state by the same Telephone Company, and when the local business exchange service is provided under the general and/or local exchange service tariffs, the End User Common Line (EUCL) Multiline Business - Individual line or trunk rate as set forth in 7.3 following applies to each such local business exchange service Pay Telephone Service The EUCL - Multiline Business rate will be assessed when a Payphone Service Provider obtains an exchange service line for the purpose of offering public telephone service Radio Common Carriers (T) For each local exchange service used only as a path for the transmission of Radio Common Carrier (RCC) traffic between the Telephone Company serving wire center and the RCC's radio equipment, End User Access Charges do not apply. End User Access Charges will apply to the Radio Common Carrier's local exchange service used for administrative purposes.
