Box Lawrenceville, GA Application for College Scholarship Delta Profile Delta Sigma Theta Sorority, Incorporated is a non-profit public service organization with membership of over 250,000 college educated women in over 940 chapters located in the United States, England, Japan (Tokyo and Okinawa), Germany, The Virgin Islands, Bermuda, Jamaica, The Bahamas and The Republic of Korea. Founded over one hundred years ago, in 1913, the organization was established to provide programs promoting human welfare and currently has a five point focus in the areas of Educational Development, International Awareness and Involvement, Physical and Mental Health and Political Awareness and Involvement. Chartered in March 2003, the Gwinnett County Alumnae Chapter (GCAC) of Delta Sigma Theta Sorority, Inc. has over 200 members. GCAC is committed to making a positive difference in the Gwinnett community through exemplary service programs that include: Δ Delta Academy/Delta GEMS/EMBODI- mentoring and personal development programs for young women and men; Δ Taboo Tea and Women s Health Summit- a program focusing on the health and wellness needs of adult women; Δ Wealth Building Seminar- program that provides financial, budgeting, and debt-reduction information and strategies; Δ Cotillion~Beautillion Program- an enrichment program for high school junior and seniors who have demonstrated exemplary moral standards and plan to attend a postsecondary school. The program culminates with a formal Cotillion~Beautillion ball in the spring of 2016; Δ Bring The Noise- Psalm 98: A Youth and Young Adult Gospel Choir Competition- fundraiser designed to raise money for scholarships for Gwinnett County High School Seniors. Δ Scholarship Program- a partnership with Fortitude Foundation, Inc. that has awarded over $90,000 in scholarship monies to high school seniors.

2 GCAC has also formed an alliance with Partnership Against Domestic Violence (PADV) and participates in the semi-annual AIDS Walk Atlanta &5K Run, Gwinnett Relay for Life and Gwinnett Great Days of Service. Additionally, the chapter supports international relief efforts and holds local political forums, voter registration drives and health fairs. Fortitude Foundation, Inc. Profile Fortitude Foundation, Incorporated is a nonprofit, tax exempt 501(c)(3) organization which exists to make possible gifts and support for the Gwinnett County Alumnae Chapter of Delta Sigma Theta Sorority, Incorporated s (hereinafter GCAC ) scholarship, philanthropic, leadership, and educational programs. It exists through the financial support of members of Delta Sigma Theta Sorority, Incorporated and corporate and civic donors. The mission of Fortitude Foundation, Inc. is to enhance the quality of life of Gwinnett County residents by engaging in community and economic development initiatives and promoting higher education through scholarships and programs. The Foundation may also partner with the Gwinnett County Alumnae Chapter of Delta Sigma Theta Sorority, Inc. and other collaborative organizations in the implementation of serviceoriented community projects and programs that align with the Foundation s mission of serving the Gwinnett County community. Fortitude Foundation provides support in the following program areas: 1. Educational Development: The emphasis is on creating educational programs that include activities related to youth services, scholarship, training, and cultural enrichment. 2. Economic Development: Empowerment for urban change by mobilizing the community. Some of the areas embraced are a) economic self-help; b) job opportunities; and c) consumer education and protection. 3. International Involvement: The emphasis is on collaborating with local, national, and international organizations to render meaningful services for the improvement of human life and conditions. 4. Housing and Urban Development: The emphasis is on combating community deterioration and addressing public health and safety issues. 5. Physical and Mental Health: This area encompasses programs and projects devoted to the physical and mental health of an individual and the nation. It includes programs geared to de-stigmatize mental illness and promote physical well-being. 2

3 Scholarship Eligibility Requirements: All African-American senior young women and men who attend a high school in Gwinnett County, except Centerville, Snellville and Loganville are eligible to apply. o Application Requirements: o A completed, signed and dated application. The application must be typed or legibly printed in blue or black ink. o Type-written essays on the topics listed on the Essay Instruction sheet. o Three (3) letters of recommendation from the following: o 2 Core Academic Teachers (e.g. Math, Science, Language Arts, Foreign Language and Social Studies) o 1 Community Service Leader o A copy of an official high school transcript o Completed application, essay response, letters of recommendation and official transcript must be mailed together in the same envelope and postmarked by February 15, Mail to: Delta Sigma Theta Sorority, Inc. Gwinnett County Alumnae Chapter Attention: Scholarship Committee Chair P.O. Box Lawrenceville, GA o All completed applications will be reviewed and scored by the Scholarship Committee. The top scoring applicants will be granted an interview and required to submit a sixty second video to determine scholarship recipients. All scholarship recipients will be notified no later than March 15, To avoid disqualification, please complete all of the application requirements and meet the submission deadline. 3

7 PART V RECOMMENDATIONS Please submit recommendation letters from the following: (a) 2 Core Academic Teachers (Academic & Personal Recommendation must be on school letterhead) (b) 1 Community Service Leader (Community Service Recommendation) Acceptable Core Academic recommendations are Reading, Language Arts, Math, Science, Social Studies and Foreign Language. The community service must be unpaid. Examples of community service include Hands on Atlanta, Gwinnett Great Days of Service, Hosea Feed the Hungry, Partnership Against Domestic Violence (PADV), AIDS Walk Atlanta & 5K Run, Gwinnett Relay for Life or Mentoring, Tutoring and Outreach through your school, youth or faith group. A. Each reference letter must include the following information: 1) Name, occupation, telephone number and address of each reference; 2) How long they have known you; 3) The type of community activities in which you have been involved and; 4) A description of your character and work ethic. B. Each letter of recommendation must be submitted in a sealed envelope with the reference s signature across the sealed portion. C. A total of three letters of recommendation must be included in your application packet. 7

8 PART VI ESSAY You will be assessed on your ability to communicate your thoughts. Your essays must be typed, and double spaced in 12pt font. Attach your essays to this sheet and include your name at the top of each page. There are three essay topics. One is mandatory and you have a choice between the other two. Type two pages or less on the following: MANDATORY - Delta Sigma Theta Sorority Inc. is a public service organization that confronts the challenges of African Americans, which impacts all Americans. In accordance with the organizations overarching goal of improving the community, they have developed a Five Point Thrust, which includes: Economic Development Educational Development International Awareness and Involvement Physical and Mental Health Political Awareness and Involvement Please select one of the Five Point Thrust areas listed above and discuss how you would implement in order to have a positive impact on your community. OPTIONAL: On a separate sheet of paper, please type a one page essay on one of the topics listed below: TOPIC A: Should the death penalty ever be used as a consequence once a person has been convicted as a crime?
