3 Home Site 1.0 LEGISLATIVE BACKGROUND AND PROCESS 1.1 This Report and Survey has been commissioned in response to The Control of Regulations Under Regulation 4 of these regulations duty holders have a responsibility to manage the risks arising from asbestos in buildings under their control. Duty holders must take reasonable steps to find and identify Containing s (ACMs) in their premises and to check its condition. Duty holders must provide information on the location and condition of the asbestos to those people who are likely to disturb it e.g. contractors who undertake maintenance and refurbishment work which disturbs the fabric of the building or its fixed installations. The Surveys along with previously collated asbestos information will form the basis for a borough wide strategy for compliance with the Control of Regulations 2006 and the councils operational plan for the management of asbestos containing materials in the Council s non-domestic premises. 1.2 The Survey, to which this report refers, was carried out in compliance with Health & Safety Executive guidelines by experienced P402 qualified surveyors trained in the inspection of structures for the presence of asbestos containing materials. The type of survey is described on a block by block basis in Section 11 of this report. 1.3 Prior to the survey, a preliminary desk top study was carried out whereby all previous asbestos information relating to the building was collated. 1.4 A preliminary walk through the site was made to determine possible hazards and / or problem areas and a Risk was carried out before commencement of the formal asbestos survey of the premises named in this report. 1.5 The survey was carried out in a safe, organised, structured and non-intrusive manner with observance of the clients / building users safety protocol. Due regard was given to all fire alarm and emergency evacuation procedures. 1.6 Photographs were taken of suspected asbestos materials but, where similar material types were located, duplicate photographs were not taken. 1.7 Location plan drawings were also marked with locations of confirmed and suspected asbestos materials identified during the course of the survey. These drawings are included with this report and issued to site 1.8 The content of all survey reports has been quality assured by the Council s Building Surveying Manager. 1.9 The full asbestos survey details and the full report are held electronically by the Council s AMCP Division including any associated asbestos analysis certificates 1.10 A synopsis of the full report is issued to each site and signed for as a recorded document. The synopsis report contains the following information: The Alert sheet detailing the location of suspected and confirmed asbestos containing materials (including a laminated copy of the same) A checklist of areas inspected/not inspected Floor plans showing the location of hazards 3

4 Home Site 2.0 ADDRESS & LOCATION 2.1 The address of the building to which this report refers is: - Home Site,, Prescot Road, WA10 3UU 3.0 SITE ACCESS / LAYOUT 3.1 Home Access by foot is gained to through main front entrance located on Prescot Road. Deliveries to the building are accessed via the Staff and Visitors Car Park Entrance which leads from an entrance lobby into a wide Staff and Visitor reception area, giving access to all the facilities within the building. Access is gained to the Boiler Room externally. External Areas see Main Building 4.0 BUILDING CONSTRUCTION 4.1 Home Home is a predominately single-storey building with a pitched tiled roof, there is a small 1st floor Staff area. The Construction of the main building is of Brown Brickwork/Timber/AIB Painted Board and timber window frames. 5.0 ASBESTOS SITE SURVEY & RISK ASSESSMENT 5.1 The survey to which this report refers is described on a block by block basis in Section 11 of this report and conforms to Health & Safety Executive MDHS 100 [Method for Determination of Hazardous Substances] guidelines. 5.2 Home Site risk assessment and asbestos survey was carried out as follows: Home - 05/11/2014 External Areas - 05/11/ The following issues were noted by the inspector during the survey Home No Access to roof areas Due To Requiring Scaffold External Areas 6.0 LOCATION OF SUSPECT ASBESTOS CONTAINING MATERIALS 6.1 Containing s were PRESUMED to 0, STRONGLY PRESUMED to 1, CONFIRMED to 1, REMOVED to 0 and NOT DETECTED to 114 items within this building and are identified in the Location & Survey Appendage section. 4

5 Home Site 6.2 This report contains photographic evidence as to the location of suspect asbestos containing material products. See rear photographic appendage. 6.3 This report contains location plan drawings of the site, indicating locations of both previously confirmed asbestos containing material [determined by positive sampling / laboratory analysis certificate sheet] and suspected asbestos containing materials determined by the survey. See location drawing appendage at the rear of this document. 7.0 LOCATION OF PREVIOUSLY PROVEN ASBESTOS CONTAINING MATERIALS 7.1 Where a previous asbestos survey has been carried out on the building and materials have been found and proven to contain asbestos via testing by an accredited testing laboratory, the asbestos Certificate of Analysis will be held by the Council s Contract and Asset management Division. Ditto, where testing has proved that asbestos containing materials were not present 8.0 ITEMS / AREAS NOT INSPECTED 8.1 In accordance with Health & Safety Executive HSG 264 & MDHS 100 guidelines ALL items / areas not inspected are STRONGLY PRESUMED TO CONTAIN ASBESTOS. 8.2 The Appendices to this report contain the location & assessment of suspected asbestos containing materials ( Alert Sheet) and also a checklist of areas inspected/not inspected. 9.0 INSPECTION OF NON ASBESTOS ITEMS NOTED 9.1 Home Refurbishment Summer 2013 / BUILDING WORKERS ALERT SHEET 10.1 containing materials are only a danger to health when in a poor condition or they are disturbed or damaged thereby causing fibre release. In most cases asbestos-containing materials in good condition should be left in place since the removal of asbestos in good condition may possibly give risk to unnecessary health risks The persons most likely to damage asbestos containing materials are building maintenance / service installation workers carrying out their day-to-day activities. By the course of their actions they may disturb and release asbestos fibres into the air exposing both themselves and other building users to hazardous asbestos fibre inhalation To assist in the safe management of the above a synopsis of the full report is issued to each site. The synopsis contains the following information: An Alert Sheet detailing the location of suspect ACMs (including laminated copies) Checklist of areas inspected/not inspected Floor plans showing the location of hazards 10.4 The duplicate laminated copy of the alert sheet must be displayed in the office / reception signing in point where persons visiting the building to carry out building works can be made aware of the suspected location/s of asbestos containing materials as part of the signing in process 10.5 The full asbestos survey details and the full report are held electronically by the Council s AMCP Division including any associated asbestos analysis certificates 5

6 Home Site 11.0 ASBESTOS PRODUCTS FOUND TO BE IN A HAZARDOUS CONDITION 11.1 Should any materials (confirmed or strongly presumed to contain asbestos) identified in this report, deteriorate or become damaged giving rise to concerns about the potential risks to the health of the building user, the following officers must be notified. 7 Home Site Management Action Plan The Duty Holder must have a clear plan for ensuring suspected asbestos materials in the premises are managed safely. This should include the following: All site based staff must be made aware of the location of the report and understand its contents The survey report must be referred to before any maintenance work that might potentially disturb CONFIRMED / STRONGLY PRESUMED materials is undertaken. The laminated Alert sheet should be displayed at the appropriate location Contractors and Maintenance staff etc. must complete the Confirmation of Access Report Form. Staff must report incidents of damage involving CONFIRMED / STRONGLY PRESUMED materials on receipt of a damage report in respect of any CONFIRMED / STRONGLY PRESUMED asbestos containing materials or as part of a responsive repair or planned works scheme the Duty Holder shall notify the Council s Management Team (D Boyes or John Smith) who will advise on the procedures to remove or encapsulate any ACMs The Survey Report shall be amended in accordance with the works identified/carried out and be re-issued to the Duty Holder The building shall be subject to a re-survey on a 5 year cycle as determined by the Council s Management Team KEY TO ASBESTOS ALERT SHEETS HAZARD CONFIRMED or STRONGLY PRESUMED TO CONTAIN ASBESTOS HAZARD REMOVED or MATERIAL DOES NOT CONTAIN ASBESTOS = RED COLOUR = GREEN COLOUR 7
