Medical Necessity Criteria Subject: Origination Date: 05/15 Revision Date(s): 05/2015 Developed By: Medical Criteria Committee 06/24/2015 External Infusion Insulin Pumps Page 1 of 10 Approved: Mary Engrav, MD Date: 06/24/2015 Description: An insulin pump is an external battery operated device that delivers subcutaneous insulin into the body in a programmed and controlled manner. It is indicated for insulin-dependent diabetics whose blood glucose levels cannot be controlled by intermittent insulin dosing even with maximal patient compliance. The goal of an insulin pump is to achieve near-normal blood glucose levels, to prevent acute metabolic complications and to delay the onset and progression of late-stage secondary macrovascular and microvascular complications of diabetes. Criteria: CWQI HCS-123 (This criteria is consistent with CMS guidelines for External Infusion Insulin Pumps) I. Continuous Subcutaneous Insulin Infusion (CSII) Pumps and related drugs/supplies are covered to plan limitations for adults or children when All of the following criteria is met: a. External insulin pumps are considered medically necessary when One or more of the following are met: i. The patient is considered insulinopenic per the updated fasting C-peptide testing requirement when ALL of the following criteria is met: 1. Fasting C-peptide level is less than or equal to 110 percent of the lower limit of normal of the laboratory's measurement method. 2. Fasting C-peptide levels will only be considered valid with a concurrently obtained fasting glucose 225 mg/dl 3. Levels documented in the medical records (Levels only need to be documented once) ii. For patients with renal insufficiency and a creatinine clearance (actual or calculated from age, weight, and serum creatinine) < to 50 ml/minute, a fasting C-peptide level is less than or equal to 200 per cent of the lower limit of normal of the laboratory s measurement method. iii. Beta cell autoantibody test is positive
2 b. The patient must also meet One or more of the following criteria for an insulin pumpto be approved: i. The patient has completed a comprehensive diabetes education program and ALL of the following: 1. Patient has been on a program of multiple daily injections of insulin (i.e., at least 3 injections per day) with frequent self-adjustments of insulin dose for at least 6 months prior to initiation of the insulin pump 2. The Patient has documented frequency of glucose self-testing an average of at least 4 times per day during the 2 months prior to initiation of the insulin pump, and meets One or more of the following criteria (1-5) while on the multiple injection regimen: a. Glycosylated hemoglobin level (HbA1C) greater than 7 percent b. History of recurring hypoglycemia c. Wide fluctuations in blood glucose before mealtime d. Dawn phenomenon with fasting blood sugars frequently exceeding 200 mg/dl e. History of severe glycemic excursions ii. The patient has been on an external insulin infusion pump prior to enrollment in Medicare and has documented frequency of glucose self-testing an average of at least 4 times per day during the month prior to Medicare enrollment. iii. The patient has an insulin pump and a replacement pump is required. A replacement pump will be authorized if the pump is past warranty and is not functioning properly. iv. Pregnancy related conditions are considered medically necessary when One or more of the following criteria are met: 1. The patient has the onset of gestational diabetes and experiences erratic blood sugars despite maximal patient compliance; or 2. Pregnancy occurs in a previously diagnosed diabetic and the patient has erratic blood sugars despite maximal compliance; or 3. When pregnancy is anticipated within 3 months in the diagnosed diabetic and One or more of the following: a. The patient has erratic blood sugar b. Patient has ketoacidosis c. Patent has symptomatic hypoglycemia despite maximal patient compliance II. Renewal and continuation of an external infusion pump and supplies is considered medically necessary when ALL of the following are met: a. The patient has been seen and evaluated by the treating physician at least every 3 months b. The external insulin infusion pump must be ordered and follow-up care rendered by a physician and ALL of the following: i. Physician manages multiple patients on continuous subcutaneous insulin infusion therapy
3 ii. Physician works closely with a team including nurses, diabetic educators, and dieticians who are knowledgeable in the use of continuous subcutaneous insulin infusion therapy. III. Limitations and Exclusions: a. If criterion I. a. or b. is not met, the pump and related accessories, supplies, and insulin will be denied as not reasonable and necessary and ALL of the following: i. The request is Not for an Insulin pump upgrade as they are not covered. (Per manufacturer guidelines, pump upgrades are provided when the pump is still under warranty) ii. The request is Not for additional software or hardware required for downloading data to a personal computer to aid in diabetes self-management as it is considered a convenience item and not medically necessary. iii. Requests are Not for usage of infusion pumps for other than E0784 as they will be as not reasonable and necessary. iv. Chronic intermittent intravenous insulin therapy (CIIIT) or pulsatile IV insulin therapy (PIVIT) v. The request is Not for a fully implantable infusion pump for the infusion of insulin to treat diabetes. The data does not demonstrate that the pump provides effective administration of insulin and is therefore not covered vi.
