Tailor Made Packages 1:1 & 2:1 Support For Case Managers, PCT s, social services and solicitors VP Community Care can provide - Detailed care needs assessments Stringent selection & recruitment process Staff retention Staff rota management Staff team trained to the individual clients needs Full care management service Clinical management team Development of long term care plan with client Support can be reduced as independence grows We are able to assist case managers and healthcare professionals with 1:1 and 2:1 care packages, nationwide. We are specialists in providing complex care packages for clients who require 1:1 and 2:1 support in the community. We have often been able to manage the most difficult of clients where others have failed. Very often our clients approach us after hearing our success stories with clients who we have worked with. Our emphasis is focused on a strong and capable staff team, which starts with our stringent selection and recruitment process, ongoing staff training and supervision. We take into consideration the personalities of our clients and match staff to work with clients where they will have common interests.
Specialist Brain Injury Staff Our specialist brain injury team are carefully selected, you can have confidence that each and every staff member has been chosen for the skills, experience, knowledge, reliability, honesty and integrity that they have. We have an in depth recruitment and selection process which ensures we only ever offer a position to someone who we personally feel is suitable to work within the specialist area of brain injury. As part of our recruitment process all of our staff are thoroughly interviewed and references are received and an enhanced CRB is undertaken. All staff attend our mandatory training and our specific brain injury training. We hold regular supervisions and appraisals for our staff in line with our quality assurance process, we also hold periodic service reviews to ensure that our high standards are maintained. We can specifically recruit for your client, providing permanent staff to join your team or we can provide a team in the locality of your client. We are committed to delivering care which is of an excellent standard whilst continuing to provide specialist care for clients who have a brain injury.
Testimonials Many thanks for stepping into the breach so quickly to help my client Brain injury case manager We are very happy with the staff secured through you in the first wave and you were very supportive and professional in assisting us in doing so and therefore would be happy to continue the relationship going forward at such time that it is required Brain injury rehabilitation unit manager VP have provided an experienced staff team to support my client who has a brain injury, I approached VP 18 months ago when the existing care package had broken down. My client now lives a fulfilling life in the community and has engaged in activities we never thought would be possible - Brain injury case manager Bristol Experienced, trained high quality staff who have actual experience working with clients with a brain injury and it shows when VP s staff interact with my client - Care coordinator NHS Over the course of the last 15 years, my relative has had a number of care regimes...none of them have been able to cope with his behaviour. We are faced with a sad prospect of...a secure unit when I was referred to VP as a possible solution. My relative is very positive, very happy and speaks really highly of all the carers. All care had been provided using communication and motivation and they have dealt with violent outbursts in this positive way. So far VP have done exactly what they said they would and have enabled my relative to live in the community...they have given him the chance to lead as normal a life as possible. - Family member of a service user
Case Study - Alice my story Alice was highly ambitious, had a successful career and had her whole life ahead of her. At the age of 27 Alice was involved in a road traffic accident where she suffered a brain injury which left her needing hospital treatment for eight months. Alice spent one year in a rehabilitation unit recovering from her injuries, Alice s personality and cognitive function had changed and her behaviour had become a risk to her self and to others. Alice wanted to move back into her home where she could start to re build her life. VP became involved at the beginning of the transition, from the rehabilitation unit to the community. We provided a graded approach introducing a specialist brain injury staff team into Alice s current environment, before the move into the community. Alice met our staff through a series of meet and greets where Alice had a choice of who she would like to work with her. We provided Alice with 2:1 support and our ABI manager implemented a rehabilitation plan and identified a series of short term goals. These included helping Alice with day to day activities and daily living skills including meal preparation, how to use public transport and learning to use the local library. Six months later Alice is living in the community and attends the local Headway where she meets other people who have had a brain injury. Alice now receives support through VP on a 1:1 basis as she has developed her skills to live more independently. Alice has started volunteering at the local RSPCA shelter with the support of her staff team and Alice s long term goal is to increase her days volunteering and to eventually take paid employment. After my accident I couldn t imagine where my life would end up, since having the support from VP I have found the confidence and self worth, to be able to do things that I once could Alice
Case Study - Jon my story Jon sustained a brain injury after he was assaulted in 2005, Jon was walking home from the pub one night when he was viciously attacked with a metal bar. Jon was in a coma for one week and spent four months in hospital. The physical injuries healed but since the brain injury Jon has cognitive difficulties, including poor attention and concentration, short term memory loss and problems processing information. Jon was very anxious and lacked in confidence, he could be quite impulsive at times and didn t always think through the consequences of his actions, Jon has lived in the community since VP began supporting Jon in 2008 where we were appointed to take over the existing care package. Our ABI manager carried out a full assessment to see whether we could meet the needs of Jon. We implemented a new staff team keeping two of his existing support workers and Jon helped choose the remainder of the team. Jon now receives 1:1 support, 24 hours a day where the staff team help to support him with day to day activities. Jon lives a full and varied life including attending music lessons and playing his guitar, often to those who visit him at his house. Jon visits his family on a regular basis with the support from the staff team to support him with the journey. Jon has started a college course in gardening and hopes to take a voluntary placement once he has completed the course. Jon s long term aim is to work as a landscape gardener. Since VP have helped to support me I remember to attend my music lessons, I can now play the guitar a lot better. I like my staff, they help me to do things, we often do the gardening together Jon
