1 School Year Name School WICHITA FALLS INDEPENDENT SCHOOL DISTRICT Formative evaluation will occur throughout the school year in both formal and informal observations by the Director of Special Education in consultation with the appropriate Support Specialist and Building Principal. The Director of Special Education will annually complete the evaluation form using a with appropriate documentation. The completed form will be reviewed with the Educational Diagnostician/School Psychologist WF (03)
2 I. ASSESSMENT PROCESS SUMMATIVE EVALUATION No later than October 1st, the primary evaluator and the evaluatee shall cooperatively complete Column A. The Primary Evaluator will, at the end of the year, complete Column B awarding 1 point if the evaluatee "Meets Expectations" and no points (0) if they do not. a. selects appropriate test instruments for use with specific students. b. administers and scores test instruments in accordance with publisher's guidelines and recommendations. c. presents test materials to students in a confident manner. d. interprets assessment results in terms of eligibility guidelines and educational programming. e. demonstrates ability to communicate assessment results at a level that facilitates understanding by parents and professionals. f. makes recommendations to the ARD Committee which are clear and consistent with assessment data. g. assists teachers in the development of IEP's, teaching strategies, and classroom management. 1.a 1.a 1.b 1.b 1.c 1.c 1.d 1.d 1.e 1.e 1.f 1.f 1.g 1.g II. 3 f. demonstrates ability to work with building personnel for effective program implementation. g. demonstrates ability to prioritize and/or organize assigned duties. h. follows established procedures for the completion of paperwork. (SEMS routing of folders, etc.) i. establishes system to facilitate communication with campus personnel. j. arrives at assigned school meetings at appropriate time. k. participates in ARD meetings in an efficient and organized manner. III.
