2 Roles, Responsibilities and Review The Head of People Development and Safety is responsible for ensuring this Order is implemented across the Brigade. The Health and Safety Officer will be responsible for the day to day operation of the Order. The Health and Safety Officer will review this Order 3 yearly, when new legislation arises or as and when organisational needs require. 2 of 7
3 Brigade Order Health and Safety 9 Part 1 - Reporting and Investigation of Personal Injury, Near Miss or Dangerous Occurrence at Work or on Duty Purpose This Order provides guidance for the reporting and investigation of accidents, near misses and dangerous occurrences. Strategic Aims Strategic Aim 3 To reduce the number of fire related deaths and serious injuries Introduction It is vitally important that all accidents, however minor, are properly investigated, with particular reference to any failure of equipment or unsafe systems of work. It is only then that trends can be identified and appropriate action taken to minimise or eliminate risks. To ensure that this process takes place the following procedures are to be adopted when an employee, visitor or contractor experiences an accident, near miss, or a dangerous occurrence on Service premises. Definitions An accident is an unplanned event which causes injury to persons, damage to property, loss of operational efficiency or damage to the environment. For example: a fall resulting in a fracture, operation of equipment leading to damage, allowing fuel to escape into a water course. A Near Miss is an unplanned event which does not cause injury or damage, but could do so. For examples, items falling near to personnel. Minor Injuries: these include cuts, abrasions, bruises, minor strains, slips, trips, falls on level surfaces, minor burns, minor manual handling injuries, where the individual will normally be able to return to work immediately following minor first aid. Brigade Order Health & Safety No 9 Part 4 Reporting and Investigating of Minor Injury and Accidents at Work should be used. A Dangerous Occurrence is an incident at work that has a high potential to cause death or serious injury, for example, an explosion or major fire on Service property. Legal obligations Employer Service To investigate all accidents, near misses and dangerous occurrences Record all discrepancies between the investigation and the circumstances reported 3 of 7
4 Employees all staff To inform their employer of all accidents, near misses and dangerous occurrences immediately or if unable to do so, then as soon as possible Accident investigation The prime objectives of any accident investigation are: To determine the sequence of events leading to the accident To establish the unsafe practices and/or conditions that caused the accident To initiate short term action and recommend a longer term programme to prevent re-occurrence Recording of accidents All accidents must be recorded in the Station/Place of Work Accident Book BI510. The entry should be made by the injured person or, if this is not possible, by the Officer in Charge/Line Manager. Details should be recorded as soon as is reasonably practicable and in any case prior to the end of shift or the working day. Reporting of accidents Action by Officers in Charge and Line Managers The Accident Reporting Aide Memoire at Appendix C provides guidance on the accident reporting process. Initiate immediate action to prevent recurrence Pass details of the injury to Fire Control immediately Ensure details of the injury are entered in the Accident Book BI510 Complete the Personal Injury Report form FB238 Ensure that a copy of Section 1 of the Personal Injury Report is dispatched to the Health and Safety Officer along with the BI510 accident record within 24 hours. Pass the Personal Injury Report form to the Investigating Officer (This is not required for minor injury) Action by Fire Control Inform the Principal Officer Follow instructions as directed in the Fire Control Standard Operating Procedures manual 4 of 7
5 Action by Principal Officer Select an Investigation Officer, dependent on the nature/or severity of the accident or injury, to carry out the investigation of the accident Consider the early involvement of Occupational Health Monitor the investigation of the accident Ensure that remedial action is taken to prevent a recurrence of the incident by appointing an Action Implementation Officer Vet and submit the Personal Injury Report completed by the Investigation Officer to the Health and Safety Officer Action by the Investigating Officer Ensure that relevant equipment or records are impounded Interview all witnesses and, where possible, injured persons Inform the Principal Officer of any immediate Health and Safety implications which require urgent remedial action and any action taken Submit the Personal Injury Report and other relevant information to the Principal Officer within 3 weeks of the accident. In the absence of the Health and Safety Officer, ensure that all Notable Accidents and Dangerous Occurrences are reported direct to the Head of People Development and Safety. Confirm and action, if necessary within 15 days of the accident, if the accident is reportable under RIDDOR (Reporting of Injuries, Diseases and Dangerous Occurrence) events For RIDDOR reporting procedures see Brigade Order Health and Safety 9 Part 2 Reporting of Injuries, Diseases and Dangerous Occurrence at Work or on Duty. Action by Action Implementation Officer On being appointed by the Principal Officer, the Action Implementation Officer will ensure that any recommendations/actions are completed and will return the completed FB238 Personal Injury Report form to Health and Safety Officer 5 of 7
6 Action by Health and Safety Officer Ensure that Personal Injury Reports are processed and monitor any action that is recommended Liaise with Administration Support for report processing and forwarding of Personal Injury Reports when completed (see Action by Administrative Support ) to the Human Resources Manager for filing in the individual s EPR file Liaise with Human Resources ensuring that any occasion where personnel have been injured and subsequently book off duty sick as a result of the accident is identified; especially if the individual has initially remained on duty after the accident Arrange for any impounded items to be labelled and placed in Technical Services via Head of People Development and Safety/Principal Officer Provide advice and guidance for Investigating Officers on any: procedures legislation and/or any remedial action that may be required Action by Administration Support Receive completed Personal Injury Investigation Reports Enter accident on database and spreadsheet - Accident number generated Photocopy front sheet of the Personal Injury form and forward copy to Occupational Health Return completed form to Health and Safety Officer Action by the Human Resources Inspect and file the Personal Injury Report in the EPR of the injured person Personal Injury reports The Personal Injury Report form will be used to report the investigation of all accidents. Notes for guidance are contained in Brigade Order Health and Safety No 9 Part 3 Personal Injury Report Form Notes on Completion. 6 of 7
7 Near Miss reporting A Near Miss incident should be reported in the initial stages to the employee s Line Manager, who will inform the Principal Officer. A flow chart at Appendix A is provided to assist in the reporting procedure. The Line Manager will be responsible for conducting an investigation and completing the Service s Near Miss form FB237 (Appendix B) and forwarding it to the Health and Safety Officer. The Line Manager, in consultation with the Principal Officer, will initiate any remedial action required. Delayed reporting Where personnel have failed to adhere to Brigade procedures for the reporting of accidents and injuries, the Investigating Officer, in the completion of their investigation report, will need to clearly state the reason for the delay in reporting of the accident or injury and where it is not possible to identify a cause or verify injuries, state the reason for the lack of evidence and other relevant details. 7 of 7
