PURPOSE AND SCOPE The purpose of this procedure is to describe the standards required for traffic management, on designated construction general and gazetted roads with or adjoining the construction project and facilities. This procedure shall apply to all construction work places in regards to all project and contractor personnel associated with the Broken Hill Operations (BHOP) Rasp Mine project. Identify and provide information to allow safe access and egress of mobile plant, equipment and vehicles on the Rasp Mine construction project. Provide guidelines to protect personnel and equipment from traffic hazards that may arise during construction activity. Minimise disruption, congestion and delays as a result of construction activity. Identify, develop and implement additional traffic control measures as required. 2. ROLES AND RESPONSIBILITIES It is the responsibility of all management, supervisory staff, contractors and work group personnel to ensure that hazards and safety issues are resolved as quickly as possible and the workplace environment is free from hazards. GR Engineering Services Limited (GRES) supervisory staff shall ensure that the traffic management plan is implemented and adhered to during all aspects of the project construction. The traffic management plan shall be monitored to ensure compliance by all site personnel, GRES supervision shall be responsible for identification, reporting and correction of areas found to be in non compliance to the plan, and adapt the plan where required, to encompass operational change during the phases of construction. GRES shall ensure that all mobile plant, equipment and vehicles shall comply with the required statutory and state legislation. 4. INFORMATION Traffic management plan shall be communicated to all GRES and contracting personnel, initially by site induction, additional information shall be conveyed by toolbox meetings and safety meetings. Hard copies of the traffic management plan shall be available at the GRES site office and safety office. 5. PROCEDURE 5.1 Occupational Health and Safety GRES and contracting personnel employed on the Rasp Mine project shall comply with OH&S Statutory Legislation, Codes of practice, Guidelines, and Standards. BHOP company policies and procedures, GRES policies and procedures and Rasp mine project site requirements. Accidents/incidents involving traffic, or are traffic related shall be subject to GRES accident/incident reporting and investigation procedures. BHOP shall be informed of any accidents/incidents that occur and investigation results. Traffic management plan shall incorporate the use of GRES procedures, job hazard analysis (JSA), and risk assessments to assist in providing a safe place of work. 5.2 Operational Personnel GRES shall ensure that all personnel operating mobile plant/vehicles on the Rasp Mine project are currently qualified and competent to perform the duties required in the operation of mobile plant, equipment and vehicles. Personnel shall be informed of the traffic management plan and site requirements. Broken Hill Operations Pty Ltd Page 4

5 Personnel shall be informed of changes to the traffic management plan that may be required as the project develops. Personnel shall be informed of heavy transportation movement, heavy lifts and crane activity prior to commencement of activity. GRES and contractor personnel who fail to comply with the requirements of this procedure shall be subject to disciplinary proceedings. 5.3 Emergency Situation Emergency vehicles shall have right of way on the construction site at all times. Personnel operating mobile plant, equipment and vehicles shall be informed either by radio communications or upon hearing the warning siren, relocate the vehicle to the nearest off road position and turn of the ignition. Personnel operating plant whilst carrying a load shall lower load and deactivate the equipment. All personnel shall cease work activities and proceed to nominated muster point. 5.4 Mobile Plant and Vehicles All mobile plant, equipment and vehicles utilised on the Rasp Mine project shall be compliant with the statutory and state legislative requirements and be in a roadworthy condition prior to site activation. Mobile plant, equipment and vehicles shall be maintained as per the manufactures instructions and standards. Mobile plant, equipment and vehicles shall be registered, licensed and insured. Evidence of mobile plant, equipment, and vehicles registration, certification, maintenance records shall be documented and recorded. Mobile plant, equipment and vehicles shall be subject to a service and maintenance program during the lifespan of the construction program. A prestart shall be conducted prior to activation of light vehicles, mobile plant and equipment. Mobile plant, equipment and vehicles, which are identified as defective, damaged, or not of roadworthy condition shall have an out of service tag applied to main isolation point(s) and placed out of service. 6 Mobile plant, equipment and vehicles, which are identified as defective, damaged, or not of roadworthy condition shall be immediately reported and recorded. Mobile plant, equipment and vehicles, shall be equipped with a communication device. A vehicle and mobile plant channel shall be nominated and implemented. (While working in GRES site area CH13 shall be used). 5.5 Site Requirements of Mobile Plant and Vehicles All mobile plant, equipment and vehicles, shall have an operational flashing identification light attached at all times during movement on the construction site and mining lease. All mobile plant, equipment and vehicles, shall have an operational audible reversing alarm. Light vehicles shall carry a first aid kit and vehicle fire extinguisher with a current inspection tag. All mobile plant, equipment and vehicles, shall comply with construction site speed restrictions of 10 kph. BHOP mine site speed restrictions of 40kph will be complied with on site roads. All mobile plant, equipment and vehicles, shall comply with traffic signage located in the construction area and mine site area. All mobile plant, equipment and vehicles, outside the perimeter of the construction area, on the BHOP mining lease shall be subject to the speed restrictions and vehicle operations move off procedure imposed by the BHOP management. All vehicles and plant shall use the following vehicle operation horn signals outside of the GRES construction area: Engine start single blast. Move off forward two blasts. Move off reverse three blasts. Operating (stationary) plant, equipment, shall have high-visibility cones separating the operational area from areas of moving traffic. All Mobile plant, equipment and vehicles, shall not be left unattended whilst in operation. All Mobile plant, equipment and vehicles not in operation shall be deactivated. Additional personnel shall be made available, should operations require the use of a spotter/guide, (restricted areas of movement, restricted vision, and congested areas). The wearing of seat belts is compulsory in all GRES services and contractor mobile plant, equipment and vehicles. Broken Hill Operations Pty Ltd Page 6

7 Overtaking of a moving vehicle on the construction site is prohibited at all times. 5.6 Site Entry Entry to the mining lease and construction site is strictly limited to the access point on Eyre Street. GRES visitors, delivery vehicles and plant entering the site are required to report to the GRES office located at Radford House. 5.7 Traffic Control Traffic control shall be regulated and controlled on the Rasp Mine project with the implementation of a delineated road framework to service the plant construction, which shall encompass the road system used by the existing plant and facilities (reference site map). The road network shall be identified and defined by using a series of windrows as road boundaries and appropriate signage. Traffic movement shall be controlled in all construction areas by the installation of signage at predetermined locations, indicating: travel direction, speed limitations, and right of way. Signage shall be installed and displayed to satisfy: Durability. Sound construction. Reflection (Night). Direction. Safe Movement. Speed Limitation. Location requirements: Signage is within driver s vision. Signage cannot be obscured / blocked. Signage cannot obscure other signage from the driver s vision. Signage cannot become a hazard. Signage cannot direct traffic into an undesirable path. Signage stands shall be located a maximum 300 mm from road boundaries and intersection points. Signage: shall comply with AS 1742, AS1743, AS1744, signage shall include but not limited to: speed restrictions, stop and give way, parking/no parking, one way. Signage shall be displayed in all construction areas where applicable. Broken Hill Operations Pty Ltd Page 7

8 5.8 Designated Parking Construction Site A predetermined area shall be delineated for office and personnel parking at the construction site. The nominated parking area shall be marked with the use of capped star pickets with removable high visibility barricading fencing securely attached. Access/Egress points shall be installed. Signage shall be displayed at the entrance to the car parking facility, indicating location of office and visitors vehicle parking area. Signage shall be displayed indicating reverse parking only. 5.9 Designated Parking Radford House The nominated parking area for visitors and personnel to the GRES site office is located at the front of Radford House. Additional parking is available located at the BHOP administration building next door to Radford House. Visitors, personnel and delivery vehicles are restricted from parking on Eyre Street as imposed by BHOP management in agreement with Broken Hill City Council (BHCC) Designated Lay Down Areas Equipment and material lay down areas shall be pre-determined and shall be marked with the use of capped star pickets with removable high visibility barricading fencing securely attached or a system of high visibility cones (witches hats) shall be used to determine boundary zones in areas where barricading is not a feasible option. Access/egress points shall be installed and identified. Signage shall be displayed to indicate lay down area and access/egress points. Access/egress points shall be of sufficient width to allow oversize plant and vehicles unrestricted movement. Access/egress points shall be free of visual restrictions and impairments. Equipment and materials shall be stored / located as to ensure clear and unrestricted access / egress to personnel / plant and vehicles. Broken Hill Operations Pty Ltd Page 8

9 5.11 Escort Vehicles Oversize equipment, plant arriving on the mine and construction site. To ensure the safe passage of oversize transport and minimal disruption to community, mine and construction activity, the intended arrival of an oversized load will be coordinated by the appointed transport company. Communication of such a load will be by verbal or written form. Client shall be informed of: Oversize load required. Number of transport vehicles. Load weight. Load height. Load width. Intended travel route. Estimated arrival time. Designated drop point. GRES shall consult with the client prior to any oversize load arriving to site, to determine the requirement of escort vehicles. In the event that an oversized load is required to travel on Holten Drive to enter and exit the construction site GRES shall inform BHOP of the consignment, travel route and date of arrival. All permits, legislative and governing requirements will be met by the transport company. Additional requirements: Escort point. Radio channel. Radio procedure. Travel route Journey Management GRES as part of their duty of care have available a system to record travel movement of personnel by road from the Rasp Mine construction site to a nominated destination to ensure safe passage, the system provides verification of safe arrival. If the status of safe arrival is not achieved within a 1 hour time frame, the travel party is communicated with to ascertain status, and to determine if emergency proceedings are required to be initiated. 10 The intent of the system is to capture: Mode of transport. Number of personnel. Destination(s). Travel route. Time of departure. Time of arrival (destination). Return to site/town time (if applicable). It is a requirement of GRES that all personnel who are engaged on the Rasp Mine project complete the journey management travel form; this includes access to company vehicles during RDO s. The journey management system applies to all contractors, unless the contracting company has a system that provides minimum standard or improved capabilities with the transporting of personnel from site. 6. FATIGUE MANAGEMENT Personnel employed on the Rasp mine project shall not exposed to potential fatigue as a result of transit, personnel shall travel within the nominated shift time. A designated driver shall be appointed for each vehicle, and BAC tested prior to transit of personnel from the town to the construction site. Personnel travelling by private vehicle, in terms of R & R periods shall be expected to comply with this document. On site personnel who may be required to repair a breakdown/mechanical/electrical emergency shall be returned to town by a member of supervisory or office staff. 7. NOTIFICATION OF CHANGE Prior to any change of information contained in this document or information that is required to added/deleted. BHOP and other interested parties shall be notified, and consulted prior to change of information. BHOP and other interested parties to which any amendment to the document affects or requires consent, shall be notified by electronic mail where possible, of a minimum period of 24 hours. On site GRES personnel shall be notified of change to the traffic management plan through prestart/toolbox meetings and written confirmation on office and crib room notice boards. 12 APPENDIX A TRUCKING FROM MAWSONS QUARRY Broken Hill Operations Pty Ltd Page 1 Appendix A

13 A1. GENERAL Basin Sands Logistics (BSL) have been contracted through Mawsons Quarries to deliver structural fill to the site using single trailer side tipper trucks. Three trucks will be utilised resulting in approximately forty five loads being delivered to the Rasp Mine each day. It is proposed to have BSL deliver approximately 31,000 tonnes of structural material into the Rasp Mine Site from mid July 2011 onwards, estimated to take approximately 30 days. This trucking will take place between 7.00a.m. and 5.00p.m. daily. A1.1 Traffic Route It is proposed to truck the structural fill from Mawsons Quarries works at 1 Holten Drive, Broken Hill to the Rasp Mine Site using the route shown on the attached site map. The trucking route is generally located on private roads except for a road crossing on Holten drive where trucks are required to cross the road and travel onto the Rasp Mine Site. Trucks will travel loaded with structural fill from Mason Quarries onto the Rasp Mine Site, deposit their loads within the Rasp Mine Site and then travel back unloaded along the same route. A1.2 Traffic Signage Stop signs will be positioned at the exit to Mawsons Quarry works prior to Holten Drive crossing and on the Rasp Mine Site exit road prior to Holten Drive as shown on the site map. This will ensure that all vehicles using Holten Drive will have right of away over the BSL trucks. Signs indicating truck movements will be placed on Holten drive at both of the T junctions again as shown on the site map. A1.3 Inclement Weather No trucking will take place during wet weather. A1.4 Material and Slurry Control Material on all unsealed roads will be controlled by means of wetting utilising a water truck with rear spray bars. All trucks exiting the Rasp Mine Site will be washed down using a water truck with hose attachment. On the mine lease at the point of exit a road sweeper licensed for public roads will be employed on an as required basis to keep the Holten Drive crossing clean. The road sweeping will be available at all times of trucking if required. If the road sweeper is not available and material is being deposited on the road, operations will cease until such times as the sweeper is available A1.5 Implementation of Trucking Measures Consultation will take place with BHOP prior to trucking starting. Pre start meetings will be held daily with BSL operators to ensure they are aware of the road signs, section of the public road (Holten Broken Hill Operations Pty Ltd Page 2 Appendix A
