Platium Cocierge Services is a service beefit offered 24 hours a day, seve days a week by callig or ad covers the followig assistace services: Travel Emergecy Assistace (icludig Emergecy Medical Assistace ad Legal Assistace). Lifestyle Services. 1.2 Assistace Providers. Platium Cocierge Services is a service beefit of Sigapore Airlies Westpac Cards. Westpac Bakig Corporatio ( Westpac ), the issuer of the Sigapore Airlies Westpac Cards, shall utilise the services of reputable third parties i the provisio of services fallig uder Platium Cocierge Services. 1.3 Cardholders. For Platium Cocierge Services, Cardholders are defied as: 2.0 Eligibility Sigapore Airlies Westpac Platium Cardholders. Sigapore Airlies Westpac Platium Additioal Cardholders. Local + Overseas Travel Emergecy Assistace. Travel Emergecy Assistace is valid for trips for a maximum duratio of 90 cosecutive days, i the case of accidet ad emergecy oly. Local + Overseas Lifestyle Services. Lifestyle Services are valid locally ad worldwide. 4 5

4 3.0 Travel Emergecy Assistace 3.1 Travel Assistace. The followig services are available uder Travel Emergecy Assistace worldwide Emergecy Card Replacemet Iformatio o preparig for a jourey Iformatio o customs ad duty regulatios. Platium Cocierge Services shall provide iformatio relatig to customs ad duties o goods carried ito coutries/cities icludig the quatity of duty free goods travellers are permitted to take ito such coutries/cities. Platium Cocierge Services reserve the right to provide iformatio oly o goods carried for persoal use Iformatio o foreig exchages rates. Platium Cocierge Services shall provide official foreig exchage rates ad value added taxes of foreig coutries Referrals to Embassies or Cosulates. Platium Cocierge Services shall provide the address, telephoe umber ad hours of opeig of the earest appropriate cosulate ad embassy worldwide wheever ecessary Referrals to America Express Travel Service Offices ( TSOs ) worldwide. Platium Cocierge Services shall provide address, telephoe umber ad hours of opeig of the earest America Express TSO Replacemet of broke, lost or stole glasses or cotact les. Platium Cocierge Services will arrage ad orgaise trasportatio of replacemet spectacles or cotact leses left behid or lost, provided that the Cardholder is able to provide sufficiet iformatio to Platium Cocierge Services to locate the optical provider regularly used by him/her. Platium Cocierge Services will pay costs of research, trasport, ad packig. The cost of replacemet leses, spectacles ad ay costs icurred due to import duties or taxes will be charged to the Sigapore Airlies Westpac Platium card accout (the Cardholder s accout ) withi 30 days from the date of shipmet Emergecy Cash Advaces. I the evet of lost or stole cash, Travellers Cheques, credit ad charge cards or i the evet that there are o braches of Westpac, TSOs or ATMs available at the Cardholder s locatio, Platium Cocierge Services shall advace cash to the Cardholder of up to US$1,000 for each ad every case required by the Cardholder (free telephoe assistace, related hadlig fee to be bore by Cardholder, ad the cash advace amout to be charged to Cardholder s accout, ad subject to authorisatio by Westpac). This will be billed to the Cardholder s accout withi 30 days from the date of the service Lost Luggage Assistace. Platium Cocierge Services shall provide assistace i locatig lost luggage ad shall provide to the Cardholder regular updates o the locatio status. 3.2 Travel Medical Assistace Referrals to medical services. Platium Cocierge Services will provide the Cardholder with the ame, address, telephoe umber ad if requested by the Cardholder ad if available, the office hours ad the level of Eglish laguage proficiecy of Physicias (icludig both Geeral Practitioers ad Specialists), Hospitals, Cliics, Detists ad Detal Cliics (collectively referred to as 6 7

5 Medical Service Providers ), ambulaces, private duty urses, services for disabled, opticias, ophthalmologists, pharmacies, suppliers of cotact leses ad medical aid equipmet. Wherever possible, the Cardholder will be provided with the details of two or more providers. The fial selectio of the provider shall be the resposibility of the Cardholder Dispatch of a doctor o the spot. If the Cardholder s coditio or the circumstaces require it, the cost of health treatmet ad ay doctor s fees shall be bore by the Cardholder Hospital Admissio. Orgaisatio of hospital admissio ad, if requested, the guaratee of medical expeses (to be charged to Cardholder s accout ad subject to authorisatio by Westpac) Dispatch of ecessary medicies which caot be foud locally. If medical supplies are ecessary for treatmet of a uforeseeable disease or accidet ad caot be obtaied locally, Platium Cocierge Services will do its utmost to obtai ad dispatch them to the Cardholder at the earliest possible time. The cost of such medicies, ay duties ad taxes, if applicable, will be bore by the Cardholder. These will be billed to the Cardholder s accout withi 30 days from the date of the service Trasfer of lost or forgotte prescriptios. If allowed by applicable law, Platium Cocierge Services will use its best edeavours to facilitate the trasfer of a prescriptio from a Cardholder s home pharmacy to a local pharmacy. The cost of trasfer, ay duties ad taxes, if applicable will be bore by the Cardholder. These will be billed to the Cardholder s accout withi 30 days from the date of service Medical Assessmet of treatmet received locally/medical moitorig. Platium Cocierge Services shall take all reasoable steps to arrage ad obtai a medical assessmet of the treatmet of a Cardholder ad co-ordiate ad, where it is medically advisable i the opiio of a physicia desigated by Platium Cocierge Services (Desigated Physicia), moitor the local treatmet util the Cardholder is released from treatmet or is set home. Platium Cocierge Services warrats that the Desigated Physicia of the relevat jurisdictio shall be of good repute ad ot have bee the subject of ay reprimad or malpractice proceedigs. These steps iclude but are ot limited to: a) The Desigated Physicia shall follow up with the local physicia, discuss the prelimiary diagosis ad evaluate the recommeded treatmet based o the iformatio made available to the Desigated Physicia; ad b) The Desigated Physicia will cotact the local treatig physicia throughout the treatmet period, whe deemed medically advisable, to moitor the progress of the Cardholder ad to determie the adequacy ad ecessity of treatmet beig provided to the Cardholder. I cases where the Desigated Physicia does ot deem it medically advisable to cotact the local attedig physicia every 48 hours, or more frequetly, while the Cardholder is i a hospital, a represetative from Platium Cocierge Services will cotact the Cardholder at least every 48 hours, if possible, ad if ot, will cotact the local physicia i order to moitor the progress of the Cardholder. 8 9

6 3.2.7 Medical Evacuatio ad Repatriatio. If the Desigated Physicia determies that the Cardholder is ot receivig adequate treatmet locally, Platium Cocierge Services will orgaise his/her trasfer to a more appropriate hospital, at o cost to the Cardholder. If ecessary, depedig o the ijury or illess ad if medically advisable, the Cardholder will be repatriated to the medical cetre closest to his/ her usual place of residece. If trasportatio is required, it will be arraged by oe of the followig optios: First Class trai, couchette or sleepig car, taxi, ambulace, air (o a scheduled flight, busiess class where possible), or air ambulace. Repatriatio will ot be provided for illesses or ijuries of a mild ature, which ca be treated locally ad do ot prevet the Cardholder from cotiuig their jourey Repatriatio of depedet childre. If depedet childre aged 23 years old or youger, travellig with the Cardholder at the time of ijury or sickess of the Cardholder, are left uatteded, Platium Cocierge Services shall arrage ad pay for oe-way ecoomy class trasportatio to retur the childre to their usual place of residece. Should it be ecessary for the childre to be accompaied by a attedat, Platium Cocierge Services shall pay for a qualified escort to accompay the childre. If the child of a Cardholder or a member of the Cardholder s family desigates a retur destiatio other tha the Cardholder s pricipal residece, Platium Cocierge Services shall make the travel arragemets, purchase the tickets ad pay for such travel, up to the cost of travel to the usual place of residece of the Cardholder. Ay additioal expeses icurred for ad o behalf of the Cardholder will be charged to the Cardholder s accout Trasportatio of a Travellig compaio. If a decisio is made to evacuate or repatriate a Cardholder to aother hospital or treatmet facility, Platium Cocierge Services will provide for oe-way air trasportatio for oe travellig compaio who is covered uder this agreemet (the Covered Travellig Compaio) to retur to the usual place of residece of the Cardholder uder the followig circumstaces: a) If the origial ticket of the Covered Travellig Compaio has become ivalid as a result of delay caused by the illess or ijury of the Cardholder the class of air trasportatio will be the same, if available, as the Covered Travellig Compaio s origial ticket; ad b) If the Travellig Compaio holds o ticket to retur to the usual place of residece, Platium Cocierge Services will provide for oe-way ecoomy class trasportatio. If the Covered Travellig Compaio desigates a retur other tha the usual place of residece of the Cardholder, Platium Cocierge Services shall make the travel arragemets, purchase the tickets ad pay for such travel up to the cost of the travel to the usual residece of the Cardholder, as provided above. Ay additioal expeses icurred for ad o behalf of the Cardholder will be charged to the Cardholder s accout Trasportatio of the travellig compaio to the ew place of hospitalisatio. Whe Platium Cocierge Services moves a Cardholder from oe hospital to aother, other tha a medical facility ear his/her usual place of residece, Platium Cocierge Services shall arrage ad pay for oe travellig compaio of the Cardholder or a family member who is with a Cardholder, but ot both, to travel to the ew 10 11

7 hospital locatio. The travellig compaio will use the same mode ad class of trasport as the Cardholder, wheever practicable Visit of a Immediate Family Member to the Cardholder s Hospital Bedside. Platium Cocierge Services shall arrage ad pay for emergecy roud trip ecoomy class trasportatio for oe member of the family of the Cardholder to travel to the place of treatmet if: a) The Cardholder is travellig aloe or with a child of 23 years or youger, or with a travellig compaio who is ot a family member ad who has bee required to leave the Cardholder; ad b) A Desigated Physicia, i cosultatio with the local physicia, reasoably aticipates that the Cardholder will be hospitalised for 10 cosecutive days or more. Platium Cocierge Services shall arrage accommodatio for the family member but shall have o resposibility for the cost of the accommodatio, meals or other expeses of the family member Emergecy hotel expeses. Platium Cocierge Services will arrage ad pay up to US$200 per ight for a hotel room, up to a maximum of 5 ights, i case the Cardholder has to remai i the coutry where travellig for emergecy medical treatmet or further medical check ups, or for the purpose of covalescece moitored ad agreed by a Desigated Physicia, if this treatmet will exted the trip duratio of the Cardholder beyod his/her origial plas Retur home o the death of a relative. Platium Cocierge Services shall make travel arragemets ad pay for the cost of the Cardholder, icludig but ot limited to purchasig a ecoomy class airlie ticket, to retur as soo as possible to his/her usual place of residece i the evet of death of a relative, i order to atted the deceased s fueral, of ay oe of the followig residig i Australia: father, mother, paret-i-law, legal spouse, child, brother or sister, provided the Cardholder s travel ticket is ot valid for a early retur to his/her usual place of residece Repatriatio of Mortal Remais. I case of death of the Cardholder, Platium Cocierge Services shall arrage the prompt trasportatio of the mortal remais back to his/her pricipal place of residece. Platium Cocierge Services shall pay all associated expeses, icludig up to US$700 for a coffi or other ecasemet of remais suitable for travellig purposes, excludig the cost of ay ceremoies or burial. The choice of coffi or ecasemet is at the discretio of Platium Cocierge Services. 3.3 Restrictios to Restrictios o emergecy medical assistace services. There may be certai coutries where emergecy medical assistace services are ot available or capabilities may be limited. The followig coditios restrict the ability to reder these stadard assistace services: War/civil strife/ivasio, atural disasters, riots/terrorist activities/hostilities; strikes or idustrial actios; circumstaces where there is dager of explosio/uclear radiatio/ecoomic sactios etc Evacuatio/repatriatio. I the evet of a medical emergecy, whe the Desigated Physicia, i cosultatio with a local attedig physicia, determies it is medically ecessary for the Cardholder to be trasported to a differet hospital or repatriated to his/her usual coutry of residece 12 13

8 for proper medical treatmet, Platium Cocierge Services will arrage ad pay for the trasport uder proper medical supervisio. All decisios as to the medical eed for evacuatio/repatriatio, the meas ad/ or timig of ay evacuatio/repatriatio, the medical equipmet ad medical crew to be used, ad fial destiatio, are medical decisios which will be made by the Desigated Physicia i cosultatio with a local attedig physicia based o medical factors, ad their decisios shall be coclusive i determiig the eed for such services Liability. Except for Desigated Physicias, the health care professioals, ad/or ay other professioal who provide the services eumerated herei, are idepedet cotractors ad Platium Cocierge Services shall ot be liable for the egligece or other wrogful acts or other omissios of such health care professioals. 3.4 Medical Assistace Exclusios Pre-existig medical coditios. Platium Cocierge Services shall have o obligatio to provide medical assistace where the illess givig rise to the ecessity of medical attetio falls withi ay of the followig categories: illess arisig from a medical coditio which existed prior to the period of a Covered Trip (uless the treatig practitioer or treatig specialist has give specific writte cofirmatio of fitess to travel prior to departure); sickess ad bodily ijury resultig from the use of drugs; attempted suicide or metal illess; premature birth or miscarriage withi three moths of the estimated date of delivery Repatriatio of persos. Platium Cocierge Services shall have o obligatio to repatriate the Cardholder i case of the followig: Illess or ijuries of a mild ature which ca be treated o the spot or which do ot prevet the Cardholder from cotiuig his jourey, fractures ad sprais of a mild ature, chroic diseases, pregacies uless a uforeseeable complicatio arises (limited to the first six moths of pregacy), abortios, recovery periods of a illess cotracted before the begiig of the jourey, (uless the treatig practitioer or treatig specialist has give specific writte cofirmatio of fitess to travel prior to departure), attempted suicide, accidets occurrig while the Cardholder is participatig i ay official sports cotest, ijuries or accidets occurrig withi 180 days from whe bodily ijury or illess covered uder this Agreemet was sustaied or cotracted Restrictios Related to Repatriatio/Evacuatio. Platium Cocierge Services shall ot be liable for costs, actios or decisios by the Cardholder or represetative to egage the services of other assistace compaies or persos for the purposes of repatriatio or evacuatio. However, i the uusual circumstace that the Cardholder is i a remote place with o meas of either themselves or a close perso beig able to cotact Platium Cocierge Services, ad ay delay may result i loss of life or severe prejudice to the Cardholder, the Platium Cocierge Services shall pay for the costs of evacuatig the perso to the earest place where Platium Cocierge Services ca be cotacted, either by the Cardholder or close perso

9 3.5 Limitatio of Guaratee. Whe Platium Cocierge Services orgaises a repatriatio or other trasportatio, ad if the covered claim does ot oblige Platium Cocierge Services to chage the date o which the Cardholder has iitially plaed to go back home, or if the Cardholder s ticket ca be modified, Platium Cocierge Services shall ask the Cardholder to use his/her ticket. I istaces where Platium Cocierge Services has paid the Cardholder s trasportatio back home, the Cardholder shall sed his uused ticket to Platium Cocierge Services. 3.6 Travel Assistace Legal Services Referrals ad Advace of Lawyers Fees. Platium Cocierge Services shall provide the Cardholder with the ame, address ad telephoe umber of a local lawyer. Wherever possible the Cardholder shall be referred to two or more lawyers. Although the fial selectio of a lawyer shall be the resposibility of the Cardholder, Platium Cocierge Services guaratees that ay lawyer to which it refers the Cardholder shall be admitted to practice i accordace with the laws of the relevat jurisdictio, be of good repute ad ot have bee the subject of ay reprimad or malpractice proceedigs. Platium Cocierge Services shall advace emergecy fuds to the Cardholder to pay for fees ad costs associated with such represetatio, up to US$10,000. This amout will be billed to the Cardholder s accout withi 30 days from the date of advace of the legal fee Advace of Bail Bod. Platium Cocierge Services shall attempt to secure ad properly post, where possible, bail bods for the Cardholder, up to US$10,000, for each ad every icarceratio occasioed as a result of a traffic accidet or a admiistrative complicatio. The bod cost shall be bore i full by the Cardholder ad will be billed to the Cardholder s accout withi 30 days from the date of postig of the bod, for repaymet Coditios. The total advace for legal services ad bail bods is limited to US$10,000 for each ad every icidet. If the Cardholder does ot atted the court hearig at the arraged date, Platium Cocierge Services may lauch immediate actio to recover the loss of the bail bod ad pursue the Cardholder for immediate repaymet Referrals to Traslatio Services. Platium Cocierge Services will provide the Cardholder with the ame, address, telephoe umber ad if requested by the Cardholder ad if available, the office hours Dispatch of Iterpreter. Platium Cocierge Services will provide the service if requested by the Cardholder. The cost of traslatio ad ay related costs shall be bore by the Cardholder Assistace i locatig lost articles. Platium Cocierge Services will provide the service if requested by the Cardholder. All related costs shall be bore by the Cardholder. 4.0 Lifestyle Services Lifestyle Services cover all types of persoal assistace ad lifestyle services icludig: a. Flight Iformatio. b. Sourcig, purchasig, ad delivery of tickets for theatre shows, cocerts, ad sportig evets. c. Sourcig ad arragig hospitality packages

10 d. Providig details of evets ad shows i cities worldwide. e. Fie diig, restaurat referrals ad reservatios. f. Hotel iformatio, referrals & reservatios. g. Car retal, limousie & car service iformatio, referrals & reservatios. h. Mail/Fax travel destiatio package (major cities oly). i. Coutry & major cities Iformatio: Festivals/museums/music etertaimet iformatio. Time/house/holiday iformatio. j. Health Club iformatio, referrals, reservatios. k. Golf tee time iformatio & reservatios. l. Shoppig locatio iformatio. m. Floral arragemet ad delivery.. Party plaig for special occasios. o. Help with relocatig to aother coutry. 4.1 Lifestyle Services Coditios. The Cardholder will be iformed of the cost ad optios, if available before ay bookig or purchase is made o behalf of the Cardholder. Platium Cocierge Services will ot icur costs o behalf of the Cardholder uless his/her prior coset has bee received. Ay ticket purchases oce authorised ad cofirmed by the Cardholder will be deemed o-refudable o o-exchageable items. Platium Cocierge Services will always strive to secure the best seats available accordig to the Cardholder s specific request ad price rage. 4.2 Charges. Platium Cocierge Services will edeavour to provide the Cardholder with a clear ad detailed breakdow of ay costs associated with the provisio of a service, prior to arragig the service. The Cardholder will ot be charged for research ad/or co-ordiatio services performed by Platium Cocierge Services (icludig for ay telecommuicatio charges associated with fulfillig a request ad costs icurred i utilisig the Platium Cocierge Services iteratioal etwork of agets/correspodet compaies). Platium Cocierge Services will use its best efforts to source reasoably priced shippig ad delivery charges for goods or services purchased o behalf of Cardholder. The followig are the resposibility of the Cardholder: Costs of goods/services purchased. Ay deposit paid. Costs of cacellatio. Delivery/shippig costs icludig isurace costs. Custom duties ad import taxes. Costs icurred i trasferrig fuds to fulfil a request. Fuds advaced to fulfil a request.

11 To the extet possible, goods ad services acquired o behalf of the Cardholder will be charged directly by the service establishmet to the Cardholder. If Platium Cocierge Services advaces fuds for goods or services, Platium Cocierge Services shall bill that amout to the Cardholder s accout. If the trasactio is i a currecy other tha Australia Dollars the amout shall be coverted to Australia Dollars at the prevailig foreig exchage rate detailed i the Wall Street Joural o the day the paymet is effected to the establishmet ad a surcharge of 1% of the trasactio amout will be levied. 4.3 Exclusios i locatig goods. Platium Cocierge Services will ot locate goods & services metioed i clause 4 requested for large-scale commercial use, or locate goods ad services metioed i clause 4 abroad whe customs regulatios prohibit the shippig of the items to the Cardholder. Platium Cocierge Services will ot locate items, which are prohibited uder applicable law or which cotravee popular moral or ethical stadards. 4.4 Purchase ad Shippig Restrictios. Platium Cocierge Services will purchase ad ship gifts o behalf of the Cardholder, provided such goods are for persoal use ad a shippig agecy ca be located to ship the requested quatity of items ad provide isurace to the total value of the items. Platium Cocierge Services will ot arrage the purchase or delivery of ay commercial cosigmet. Platium Cocierge Services will purchase ad ship items i accordace with iteratioal shippig regulatios ad will observe the customs ad excise restrictios i force. The Cardholder will be iformed if customs, excise ad value added taxes are applicable. The Cardholder is resposible for ay such levies. Platium Cocierge Services requires that all items shipped must be isured for the full purchase value ad such isurace costs shall be bore by the Cardholder. If the Cardholder refuses to do so, he/she will be referred to the shippig aget to make arragemets direct. 4.5 Restaurat Reservatios. Platium Cocierge Services will recommed restaurats to Cardholders ad secure reservatios, subject to availability. 4.6 etré ad America Express Selects programs. Participatig merchats ad clubs provide special offers for the exclusive use of Cardholders. Cardholder reservatios must be made though the Platium Cocierge Services. Details of merchats ad clubs participatig i the etré ad America Express Selects programs will be provided by America Express ad Westpac ad are subject to chage. 4.7 Cacellatio Charges. I the evet that a Cardholder cacels a reservatio i a establishmet participatig i the etré or America Express Selects programs or ay other promotios at short otice, the Cardholder may be liable for a cacellatio charge which will be billed to the Cardholder s Platium accout. Ay cacellatio charges that may apply will be advised at the time of bookig. 4.8 Lifestyle Services Exclusios. However, Platium Cocierge Services shall ot be held resposible for o-executio or delays resultig from ay cause or evet ot reasoably withi its cotrol, icludig but ot limited to: atural disasters; civil war; armed rebellio or coflict; war; isurrectio; military acts of foreig atios; revolutio; riots by collective actio ivolvig threats ad actual disturbace authorities to permit such services; all acts of sabotage of terrorism; radioactivity; uclear war; toxic fallout; volcaic eruptio ad all Acts of God. c) As a geeral rule, Platium Cocierge Services shall ot reimburse ay costs for which it was ot cotacted by the Cardholder i the first istace i accordace with these Terms ad Coditios
