The service has provided quality service and care for residents of the Bass Coast Shire for over 100 years. It is an integrated health service providing emergency services as well as acute (medical and surgical), sub-acute and residential inpatient beds along with an extensive array of community and primary care services. Services are also provided from a number of other sites including Wonthaggi, San Remo, Inverloch and Cowes. Position Summary This senior management position provides effective clinical leadership and overall management of the Bass Coast Health Community Nursing Program. It also oversees the management and coordination of the Hospital in the Home, District Nursing, Palliative Care Nursing, Community Health Nursing and Clinical Nurse Consultant services to ensure that relevant Department of Health & Human Services guidelines and other funding body requirements are met. Key Responsibilities 1. Provide effective leadership and clinical and administrative management of the Community Nursing Program. This includes human resource management, orientation, credentialling, staff supervision and training/guidance, professional development, competency compliance, performance management and annual performance review. 2. Complete mandatory competencies 3. Ensure the delivery of safe, high quality person centred care services 4. Ensure staff work within the scope and skills consistent with their classification structure and program guidelines maintaining ethical and professional standards 5. Work collaboratively with Executive Director of Primary & Community Care, and other health service managers and staff to ensure that the Community Nursing Program meets the current and emerging needs of the local community 6. Work collaboratively with the inpatient setting staff and relevant GP to ensure effective HITH implementation 7. Provide clinical leadership, coordination and management of the following service areas: o Community Palliative Care Nursing o Hospital in the Home o Community Health Nursing o Clinical Nurse Consultant o District Nursing 8. Maintain and coordinate the Bass Coast Health Vulnerable Persons Register 9. Ensure compliance with the Department of Health & Human Services and other relevant funding body guidelines in relation to program implementation, development and reporting 10. Apply service coordination principles in the delivery of person-centred care 11. Ensure programs in area of responsibility embrace and foster the principles of the Active Service Model 12. Develop and implement an annual budget and ensure it is effectively managed 13. Develop and implement an operational plan, aligned with the BCH 2016-2020 strategic plan, for all service areas to ensure program activity throughput targets are met 14. Set specific and measureable KPIs, goals and objectives for staff in line with the operational plan 15. Ensure staff understand the data management and documentation systems, ensuring staff compliance within the relevant Community Nursing Service guidelines and organisational policy, data capture and reporting Page 2 of 7
16. Support the Team Leader, Data Integrity/Rosters to: o Ensure adequate human resources are available to deliver service o Ensure that accurate data is available on a monthly basis for the data administration team to submit to Commonwealth and State departments 17. Actively nurture integration and multidisciplinary team functioning across departments and settings within Bass Coast Health 18. Support the Team Leader, Intake/allocation to: o Enable timely access for service users o Ensure appropriate allocation of human resources to implement care 19. Promote multidisciplinary quality improvement initiatives aimed at enhancing evidencebased clinical best practise, and improving person-centred care delivery and outcomes 20. Engage and actively seek input from consumers regarding service and care planning 21. Support the Team Leader, Education/Training to build the capacity of Community Nursing Program staff through promotion of ongoing professional development aimed at enhancing clinical practise, career development, teaching and research capabilities 22. Ensure appropriate succession planning is in place 23. Keep abreast of contemporary management, healthcare and professional policy and practises and facilitate staff to do the same 24. Develop regular written reports for the Executive Director of Primary & Community Care regarding current issues within area of responsibility 25. Attend meetings as scheduled: o Primary and Community Care Quality Committee o Primary and Community Care Managers Meeting o Commonwealth, State Regional, Industry Advisor and Local level meetings as required o Internal Committee / Working group meetings as scheduled 26. Hold monthly minuted staff meetings for the Community Nursing Program 27. Actively seek funding opportunities and write funding submissions to secure new funding for service enhancement 28. Actively network and develop collaborative relationships within the health service and with external service providers 29. Deliver community and group presentations on Community Nursing related topics where appropriate 30. Ongoing review, modification and evaluation of policies, procedures and activities to ensure ongoing progression and advancement of contemporary practices 31. Implement effective change management and communication at all times Qualifications Essential: Bachelor or Masters of Nursing Current Registered Nurse registration with Australian Health Practitioner Regulation Agency (AHPRA) Minimum five (5) years post-graduation experience Management and human resource management experience Page 3 of 7
