Position Registrar/Fellow in Intensive Care Medicine Assignment Department of Anaesthesia, St. James s Hospital. Commencement Date Monday, 10 th July, 2017. SACC Directorate. The Surgery, Anaesthesia and Critical Care Directorate (SACC) focuses on the surgical patient process from outpatients through to discharge. Core principles are based around delivering a safe, personal, clinically effective and high quality service to all patients, with clear accountability for all staff members. St. James s Hospital delivers a wide range of surgical services at a local, regional and national level. There are 13 Specialities within SACC. These are; Urology, Gynaecology, General Surgery, ENT, Breast Care, Maxillofacial,, Vascular, Plastics, Orthopaedics, Cardiothoracic, Pain Management, Anaesthesia and Critical Care. The directorate is managed by the Directorate Management Team led by Clinical Director and Operations Manager. Regular engagement with the Clinical Teams through Leads for each Speciality has been a big focus in 2015, with the establishment of Monthly Speciality Meetings. The main purpose of these was to improve communication, address operational issues and at a strategic level, set out a development plan for each service. Department of Anaesthesia The department of Anaesthesia provides the anaesthesia, intensive care and pain medicine services at St. James s Hospital, which is the largest acute academic hospital in Ireland affiliated with TCD. There are twelve theatres, including a stand alone Burns theatre, a stand alone endovascular suite and a dedicated stand alone Day Case suite, with two major and one minor theatres and a pre-operative assessment unit. The Intensive Care service covers the General Intensive Care Unit, High Dependency, cardio-thoracic ICU, Burns Unit, in addition to ventilated patients in the Coronary Care Unit and an emergency consult service to the wards and Emergency Department. The Department provides anaesthesia for adult patients in the following disciplines: general surgery, cardio-thoracic, vascular, otorhinolaryngology, plastic and reconstructive surgery including burns, maxillo-facial, orthopaedics and gynaecology. There is a large oncological surgery component in all of these disciplines. In addition the ICU admits trauma and acute medical patients. The ICU takes nation-wide referrals. The Department is recognised for training by the following bodies: - College of Anaesthetists for both BST and SpR training, - by the Royal College of Surgeons in Ireland for two months' BST in Intensive 1

Care Medicine - Royal College of Physicians in Ireland for SpR training in Intensive Care Medicine - The Irish Board of Intensive Care Medicine - The Australian New Zealand Joint Faculty of Intensive Care Medicine There are trainee posts in anaesthesia intensive care, pain medicine and general internal medicine. There is a Fellow in Intensive Care medicine and a Fellow in Pain Medicine. Training in Intensive Care Medicine is recognised by the Australian New Zealand Joint Faculty of Intensive Care Medicine for one year s accreditation towards their Fellowship. The Pain medicine team, led by Dr McCrory and Dr Fitzgerald run the acute and chronic pain service, with a Pain Medicine Fellow and a rotating registrar. There are three Clinical Nurse Specialists in the Pain Medicine service. There is a ward-based epidural service. There is an active teaching programme in which the successful applicant will participate. The Department is involved in teaching under-graduate medical students, post-graduate anaesthesia and intensive care trainees and nursing and paramedical staff. The Department also participate in the Primary and Final Fellowship preexamination courses and in the Intensive Care Diploma Course. There are three college tutors who supervise training. The research activities of the department are steadily increasing and there are e research Fellows, undertaking doctorates, attached to the department. Principal Duties and Responsibilities The Registrar post contributes to a twenty two person team which includes thirteen Specialist Registrars. Learning opportunities are based around situational teaching, daily Consultant-led patient handover and ward rounds, weekly formal registrar teaching, regular inter-specialty seminars and a clinical audit programme. Research in Emergency Medicine is actively supported within the department. Candidates will:- Professional / Clinical Be responsible for assessment, diagnosis, planning, implementation and evaluation of treatment for patients according to professional standards and under the supervision of the Consultant Document all assessments, diagnoses, treatments, clinical notes, relevant contacts and summaries in accordance with department and professional standards Communicate results of assessments and recommendations to the patient and relevant others as appropriate Foster close working relationships with colleagues and other relevant professionals in maximising the patients potential Participate in teams, communicating and working in collaboration with the patient and other team members as part of an integrated package of care Attend clinics and participate in relevant meetings, case conferences and ward 2

rounds; followed by documentation of findings on each patient s chart; follow through with actions arising from the round In conjunction with the Supervising Consultant, contribute to the development and implementation of procedures, policies and guidelines while adhering to existing standards and protocols Maintain professional standards in relation to confidentiality, ethics and legislation Seek advice and assistance from the Consultant with any assigned cases or issues that prove to be beyond the scope of his / her professional competence in line with principles of best practice and clinical governance Participate in audits and research Engage in technological developments as they apply to the patient and service administration Education and Training Participate in mandatory and recommended training programmes in accordance with organisational / professional requirements Maintain and develop professional expertise and knowledge by actively engaging in continuing professional education and development Engage in planning and performance reviews as required with the Supervising Consultant Health & Safety Comply with the policies, procedures and safe professional practice of the Irish Healthcare System by adhering to relevant legislation, regulations and standards Document appropriately and report any near misses, hazards and accidents and bring them to the attention of relevant / designated individual(s) in line with best practice Work in a safe manner with due care and attention to the safety of self and others Be aware of risk management issues, identify risks and take appropriate action Promote a culture that values diversity and respect Administrative (In consultation with the supervising Consultant) Ensure good working practice and adherence to standards of best practice Promote quality by reviewing and evaluating the service, identifying changing needs and opportunities to improve services Assist the Consultant in service development, including policy development and implementation Ensure the maintenance of accurate records in line with best clinical governance, the organisation s requirements and the Freedom of Information Act, and provide reports and other information / statistics as required Engage in service audit and demonstrate the achievement of the service objectives Represent the department / profession / team at meetings and conferences as appropriate Keep up to date with change and developments within the Irish Health Service The above Job Description is not intended to be a comprehensive list of all duties 3

Eligibility Criteria Qualifications and/ or experience Applicants are expected to have completed Basic Specialist Training in Anaesthetic Medicine. Possession of a post-graduate qualification will be advantageous to the candidate, but is not a requirement for the post. Candidates are expected to be ATLS or ACLS providers. Core requirements for this post include: a demonstrable commitment to Haematology Medicine, good decision making, a broad clinical knowledge base and appropriate clinical skills, awareness of patient safety and risk management issues and the ability to work efficiently under pressure and as part of a team. Before taking up the post: Each successful candidate must hold General Registration with the Irish Medical Council or Trainee Specialist Division Registration with the Irish Medical Council.
