PURPOSE STATEMENT Brief statement of the key responsibilities/prime function of this job: Nunkuwarrin Yunti aims to promote and deliver improvement in the health and wellbeing of all Aboriginal and Torres Strait Islander people in the greater metropolitan area of Adelaide and to advance their social, cultural and economic status. The Organisation places a strong focus on a client centred approach to the delivery of services and a collaborative working culture to achieve the best possible outcomes for our clients. The Clinical Aboriginal Health Worker is a member of a multi-disciplinary team delivering high quality, culturally safe primary health care service to Aboriginal and Torres Strait Islander people in the Adelaide metropolitan area. This also includes clients in need who do not identify themselves as Aboriginal and Torres Strait Islander people. Working under supervision, direct or indirect: Provide clinical services directly pertaining to a clinic setting. These include comprehensive client health screening, assisting with project/program implementation for addressing specific health issues, and community based service delivery Act as cultural link/liaison between Aboriginal and Torres Strait Islander clients and non- Aboriginal/Torres Strait Islander staff Facilitate accurate and concise clinical documentation directly associated with client medical records and treatment including record and data entry into computers Provide administrative support across all Nunkuwarrin Yunti sites ensuring quality working relationships and client service Support Quality Improvement (QI) processes pertaining to the clinical setting Carry out extra duties as required such as providing services for the collection and transport of clients to and from appointments arranged by Nunkuwarrin Yunti. 2. KEY RESPONSIBILITIES/DUTIES Identify the significant services of work, which are the key outputs of the position KEY RESPONSIBILITIES (Outputs of the job) Provide clinical services directly pertaining to a clinic setting. Assist, as part of a multi-disciplinary team, with the provision of clinical and preventative health service delivery programs using the principles of a Primary Health Care framework. Ensure that clinical care reflects best practice and is delivered in a culturally sensitive manner. Work from time to time across different locations in the metropolitan and greater metropolitan districts to address specific health projects/programs whilst still under the auspice and governance of Nunkuwarrin Yunti. PERFORMANCE MEASURES (How will the quantity, quality or timelines be measured?) Ensure culturally safe comprehensive client health screening occurs. Provide appropriate health education to clients to enhance their health outcomes. Assist in the process of enhanced primary health care assessments within a clinical setting. Effectively assist in the case management, care planning and individual planning for client health outcomes. Provide assistance to trainees. Practice in accordance with Nunkuwarrin Yunti Policies and Procedures and in accordance with own clinical competence and qualifications. Utilise specific equipment or machinery in accordance with level of expertise and training. Abide by and follow specific instructions relating to procedure at all times. Liaise and work closely with other service providers to ensure a high standard of integrated and coordinated client care. Work, as required, in partnership with other programs and organisations. Support the QI processes. Contribute to effective data collection. Participate in discussions about the interpretation of results. Participate in discussions to identify and recommend opportunities for improvement of clinic activities. Support Team and Organisational activities: 1. Maintain a positive working relationship with other Clinical Service team members within a multi-disciplinary team. 2. Encourage a team approach to the development and provision of appropriate health education activities and programs, planning and problem solving. 3. Participate in working groups and activities. Maintain effective internal and external networks in a professional manner. Contribute to communication processes that effectively deal with challenging behaviours and the resolution of conflicts. Contribute to the development and implementation of Clinic policies and procedures. Work collaboratively with and support other Nunkuwarrin Yunti colleagues and teams where required. Contribute to the strategic planning activities of the Clinical Services. Participate in internal working groups, committees and organisational activities where requested and/or willingly volunteer to contribute to organisational activities. Maintain professional competence. Facilitate accurate and concise clinical documentation. Ensure compliance with a range of administrative and business practices which support clinical and Nunkuwarrin Yunti services. Participate in learning activities appropriate for professional development and all mandatory training required by Nunkuwarrin Yunti. Attend professional meetings as required. Maintain timely and accurate documentation of client medical and treatment records, consistent with professional standards. Contribute to statistical and other reports as requested. Ensure secure management of client data and client files and compliance with privacy policies and legislation. Carry out extra duties as required. Be willing to carry out other duties. 3. SELECTION CRITERIA ESSENTIAL includes qualifications, skills, experience and knowledge. Hold or be working towards the Aboriginal Primary Health Care certificate or have relevant training directly relating to Primary Health Care provision Knowledge of statutory requirements and quality assurance processes associated with clinical practice Hold current Senior First Aid certificate Ability to use specific medical equipment to a recognised level of training, or willingness to undertake suitable training in equipment use. Understanding and knowledge of social, health, and cultural issues effecting the Aboriginal and Torres Strait Islander population in Adelaide and the greater metropolitan region Proven well-developed written and verbal communication skills Ability to competently use computing software and an understanding of databases Ability to effectively communicate with Aboriginal and Torres Strait Islander and non- Aboriginal and Torres Strait Islander clients Self-confidence and the ability to work without direct supervision, exercising tact and diplomacy as required Ability to work well as part of a multi-disciplinary team. 4. APPOINTMENT CONDITIONS Special Conditions and Status Full time permanent position, 38 hours per week. Some out of hours work may be required. Some intrastate travel may be required. Subject to 6 months satisfactory probationary period. Current South Australian Driver s Licence and willingness to drive in the course of work duties.
