PREAMBLE The implementation of the National Health Reform in 2011 saw the establishment of five Local Health Networks across SA intended to promote, maintain and restore the health of the communities they serve. The Northern Adelaide Local Health Network (LHN) provides care for around 341,000 people living in the northern metropolitan area of Adelaide as well as providing a number of state-wide services, and services to those in regional areas. More than 3,000 skilled staff provide high quality patient care, education, research and health promoting services. The Northern Adelaide Local Health Network (NALHN) provides a range of acute and sub-acute health services for people of all ages and covers 16 Statistical Local Areas and four Local Government Areas (one of which crosses the Central Adelaide Local Health Network) and includes the following: Lyell McEwin Hospital Modbury Hospital GP Plus Health Care Centres and Super Clinics Aged Care, Rehabilitation & Palliative Care Mental Health Services (under the governance of the Adelaide Metro Mental Health Directorate) Clinical Leadership of care systems is central to the current national and state wide health reforms. NALHN care delivery is configured within clinical divisions that are patient-focused, clinically led groupings of services. Clinical Divisions are responsible for managing service delivery activities and NALHN campuses and units, bringing together empowered experts to directly make relevant decisions. The Aged Care, Rehabilitation & Palliative Care Division (ACR&PCD) is responsible for managing service delivery activity and strategic issues across campuses and units of the LHN for aged care, rehabilitation and palliative care. These services are delivered predominately in the community. All services work closely and collaboratively with the acute care sector to ensure best practice standards are maintained and client care is of a high quality. 2. SUMMARY OF THE BROAD PURPOSE OF THE POSITION The social worker participates as a team member within the NALHN Aged Care, Rehabilitation & Palliative Care Division, ensuring optimal care and outcomes, by providing a comprehensive range of social work services to a variety of client, across various settings including inpatient, home based and community. The social worker applies clinical experience, knowledge and professional judgement to plan, implement and evaluate a comprehensive and integrated range of services appropriate to the needs of the Division. This is achieved by working in collaboration with other members of the interdisciplinary team, to effectively plan and provide therapeutic interventions based on patient goals. This may involve consultation and liaison with the team, other professional disciplines and non-government organisations. The social worker is accountable for their own practice standards, activities delegated to others and the guidance and development of less experienced staff.

EDUCATIONAL/VOCATIONAL QUALIFICATIONS Must hold a recognised qualification within a degree level qualification in the social work discipline to a standard accepted by the chief executive.and be eligible for practicing membership of the AASW 3. Demonstrated flexibility and ability to adapt to changing service provision needs Experience Experience working in a hospital and/or home and community based setting Experience in the effective development, implementation and evaluation of relevant departmental policies and procedures Experience in research and quality improvement activities within an Allied Health context Experience in managing a clinical caseload of people with high and complex needs. Knowledge Knowledge of contemporary clinical practice in; Aged Care Adult Rehabilitation Palliative Care Knowledge of issues arising from treatment and management of patient/clients in a home based setting Educational/Vocational Qualifications Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
