Applications will remain current for the duration of the vacancy The Clinical will provide a comprehensive clinical pharmacy service, within a multidisciplinary framework, to clients across the HHS. The position will work within prescribed professional and ethical standards & evidence based practice, under the guidance of a senior pharmacist. The key accountabilities of the role are: Deliver a base level clinical pharmacy service that may include dispensing (inpatient, outpatient and discharge), requisitions, medication reconciliation, drug information, counselling and the provision of pharmaceutical recommendations. Apply increasingly independent clinical judgement to more complex clinical decision making under clinical practice supervision, or professional guidance of a senior clinician as experience builds. Provide input into the development of clinical practices, procedures and protocols within the work area. Develop clinical practice knowledge and expertise through active learning within the team and by engaging in professional development activities and participating in education programs and competency assessments. Provide direction and support to assist staff in medication management of clients. Actively contribute to maintaining a safe workplace that values the health and safety of co-workers, clients or visitors. Actively contribute to developing and maintaining a culture where staff are vigilant to risks to themselves, their coworkers, clients or visitors (including health and safety, business and operational risks). Actively participate in the Health Service Performance Planning and Appraisal and Individual Development Planning processes. Staffing and budget responsibilities Provide clinical education, clinical practice and operational supervision to work experience students, or those involved in clinical placements.
