About this role The Speech Pathologist will provide quality, accountable, consumer-focused rehabilitation Speech Pathology practice to clients as part of the Rehabilitation Service Line within Sub & Post Acute Care Stream. This is underpinned by a patient centred, goal orientated multidisciplinary focus. The role is based within the Community Based Rehabilitation Team which provides a centre based and outreach service to eligible patients within the West Moreton Hospital and Health Service. This is part of an integrated rehabilitation service along with inpatient rehabilitation at Ipswich and Boonah Hospitals. The key accountabilities of the role are: Comprehensive routine Speech Pathology assessment, examination, diagnosis and treatment skills including management of dysphagia and communication within a Rehabilitation framework. Ability to apply increasingly independent clinical judgement to more complex presentations, adapting routine clinical practice based on valid and reliable evidence under the clinical practice supervision or professional guidance of a more senior clinician. Demonstrated commitment to and understanding of patient centred, goal orientated care. Experience in high level communication and liaison in the delivery of advice and education with clients and families, other health professionals and other service providers to ensure effective outcomes for the client. Effective participation and engagement within a multidisciplinary team context including service planning, promotion and evaluation and clinical quality improvement. Ability to self motivate and demonstrated proactive commitment to ongoing professional development to develop clinical practice knowledge and expertise. Staffing and budget responsibilities The position reports operationally to the Team Leader, Community Based Rehabilitation Team and professionally to the Director of Speech Pathology, Ipswich Hospital. Mandatory Qualifications / Professional Registration: Appointment to this position requires proof of qualification and registration or membership with the appropriate registration authority or association within Australia. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties. For Speech Pathologists: The possession of a tertiary degree in Speech Pathology providing eligibility for practising membership to Speech Pathology Australia. This position requires the incumbent to operate a C class vehicle and an appropriate licence endorsement to operate this type of vehicle is required. Proof of this endorsement must be provided before commencement of duty. The position may be required to rotate through various sites within the West Moreton Hospital and Health Service. Key capabilities required for this role: Clinical knowledge: Demonstrate effective Speech Pathology rehabilitation clinical knowledge in accordance with prescribed professional and ethical standards and organisational protocols to develop client centred, goal orientated plans for clients. Communication and Team Focus: Demonstrate excellent communication skills and active team participation with clients, families, other health care workers and service providers at all levels. About Clinical Services Division Clinical Services consists of the Ambulatory Stream, Acute Stream, Sub and Post Acute Stream and Coordinated Care Stream. The aim is to ensure that the needs of the patient are coordinated through the entire care journey. About Sub and Post Acute Stream Sub and post-acute care focuses on continuation of care and optimisation of health and functionality. These patients are largely known patients and require their care needs to be organised in an efficient manner based on their individual needs and preferences. Patients access this stream as part of an ongoing care pathway. The goals of care in the sub and post-acute care steam include timely access and mobilisation of multidisciplinary teams to meet the care needs of the patient.
