The position aims to assist in the enhancement of clients functional capacity and carer needs in order to promote, maintain, and improve their health, functional ability and quality of life. The Post-Acute Care supports people being discharged from hospitals (including emergency departments) who, while not acutely ill, need access to time limited post-acute health care services to support their recuperation and to minimise their risk of being readmitted to hospital.

Key Accountabilities Assist in the provision of treatment to clients as prescribed by an allied health professional, with indirect supervision and grading interventions according to guidelines set by the professional Prepare individual or generic treatment resources as directed by the allied health professional. Maintain the treatment environment including equipment inventory, cleaning and maintenance as well as room preparation. Provide basic education to clients in collaboration with allied health professional. Assumes responsibility for assigned workload by utilising planning and coordination skills in the management of daily workload Contributes to the maintenance of a safe environment by prompt identification, reporting, and documentation of potential and actual hazards, accidents, or incidents. Contribute to client records according to organisational guidelines, within parameters prescribed by the professional. Undertake ongoing training and developmental activities. Under the supervision of an allied health professional assist in the provision of screening and assessments Safety and Quality Relevant to the position, participate in the ongoing education, implementation, monitoring and evaluation of safety and quality initiatives relevant to The National Safety and Quality Health s Standards and the Australian Council on Healthcare Standards (ACHS) Evaluation and Quality Improvement Program (EQuIP). Mandatory qualifications/professional registration/other requirements Possession of a valid Queensland Drivers Licence is mandatory and must be provided before commencement of duty. The successful applicant will be required to use their own vehicle for official purposes. The vehicle must be covered by third party property damage or comprehensive insurance and indemnify Queensland Health. Whilst not mandatory, a relevant qualification would be well regarded. Willingness to enrol and complete a related Certificate Course within an appropriate time-frame may be a condition of employment. In accordance with Queensland Health Infection Control Guidelines, Patient Support s staff must be immunised against Hepatitis B. Proof of Hepatitis B vaccination is required prior to commencement of employment.
