Position Title: Social Worker Grade 2 Permanent Position up to 40 hours pw Division: Community Services Reports To: Allied Health Manager Primary Objective: Direct Reports: Grade 1 Social Worker Students To work as an effective interdisciplinary team member providing best practice Social Work Services and interventions within the scope of s integrated services. The Social Worker provides a range services that include psycho-social assessment, discharge planning, counselling, education, referral, consultation and liaison to patients, clients, family members, staff and management of Position Dimension & Decision Making Authority: Key Communication Contacts: Without referral to manager Patient care. Undertake comprehensive psychosocial assessment in a variety of settings. Coordinate CAH s Restorative Care Program Provide care coordination, counselling, advocacy, mediation, referral and support for individuals, families and groups in the inpatient, outpatient and community setting. To contribute to case conferences and clinical discussions, ensuring that psychosocial factors are considered in treatment, rehabilitation and discharge planning. In conjunction with s Palliative Care Service ensure a grief and bereavement service is provided to clients and families Provide consultation to CAH staff in relation to aged care. Coordinate with the Barwon Regional Aged Care Assessment Service in Acute services. Participate and contribute to the multidisciplinary function of the multidisciplinary teams (Acute, Palliative care and Community) Tasks are at least 75% patient related and Client treatment goals achieved. File audits reflect adherence to documented standards. Evaluation results documented. Achievement of monthly Social Work targets PJB audits reflect appropriate time utilization. Deliver best practice Social Work intervention to clients in accordance with the Australian Social Work Code of Ethics Provide resources, information and coordinate referrals for staff of the. Qualifications, Experience and Personal Attributes Bachelor of Social Work degree and eligibility for AASW membership Previous experience in a hospital/health care setting Proficient clinical skills in psychosocial assessment including risk assessment, complex care planning, discharge planning, Social Work interventions and communication Knowledge and understanding of the Social Work role in acute care, rehabilitation, palliative care, aged care, community health and outreach services Excellent IT skills in word processing and presentations Ability to work both autonomously and within multidisciplinary teams Demonstrated understanding of theoretical frameworks that inform Social Work practice. Demonstrated awareness and understanding of community resources and systems.
