Our Statement of Purpose is: ST LUKE S HOSPICE (BASILDON) JOB DESCRIPTION In partnership with the community and the multidisciplinary team our organisation is committed to providing high quality, safe and supportive specialist palliative care services that will enable people to live as independently as possible and die with dignity and choice. CONTROLS ASSURANCE STATEMENT The purpose of this job description is to outline the level of responsibility and accountability of this post. This will ensure that all work undertaken by our staff is identified and lines of accountability made clear POST TITLE: GRADE: HOURS: ACCOUNTABLE TO: RESPONSIBLE TO: POST MANAGES: LOCATION: QUALIFICATIONS: Day Hospice Lead/Clinical Nurse Practitioner. Scale 7/8 (Depending on experience & qualifications). 37.5 hours per week with some evening and weekend work required. Head of Care Services. Clinical Nursing Services Manager. All qualified (1 st and 2 nd level) Nurses, Nursing Auxiliaries, Ward Clerk/Administration, Secretaries, Volunteer staff and Volunteer Drivers. Based at St Luke s Day Hospice but required to rotate internally across the In- Patient Unit (IPU) and the community as required (all shifts). The post holder should be a registered nurse with at least 5 years experience post qualification. The post holder should have at least 3 years experience in cancer and/or palliative care or related specialities working at Band 6 or above. A post registration degree in a relevant subject is essential, or the willingness to work towards desirable. The post holder should have experience in teaching and assessing, and hold a recognised teaching qualification or willingness to work towards desirable. In addition, the Post holder should have relevant courses for professional development in palliative care (or equivalent), with the Mentorship and Workbased Learning and Assessment in Health and Social Care. Validated completion of St Luke s Hospice Qualified Nurse Competencies and Syringe Driver Competencies is required. For those employed externally on Scale 7 these Competencies will need to be achieved within the six month induction period. The post holder should have or be on a degree pathway relevant to the speciality or should have courses, training and education at Level H. INTRODUCTION This is a new and exciting role to support clinical practice and service development within Day Hospice and Out-Patients services. The Post holder will work closely with the Clinical Nursing Services Manager, Specialist Nurse Practitioner, Sisters and Day Hospice team, to extend clinical practice and patient services. The Post holder will manage the Day Hospice team while working to develop clinical services. This is an opportunity to utilise leadership and management skills alongside clinical expertise in a patient focused, clinically based role. MAIN DUTIES Working in conjunction with the Clinical Nursing Services Manager (CNSM), the post holder will have continuing responsibility for the management of care delivery, deployment and education and supervising/teaching of staff and students in the clinical environment. With the Clinical Nursing Services Manager, the post holder will be responsible for the day to day operation of the Day Hospice and Out-Patients clinical practice and staff management. They will lead and be part of a team of nurses to ensure that the highest possible standards of holistic specialist palliative care are delivered to patients and relatives. The post holder will assist in audit and research projects, particularly within their own Unit, taking a lead on specified projects as appropriate. He/she will be required to actively contribute to the development and well-being of the whole Unit team. The post holder is required to demonstrate clear team-building and leadership skills, as well as a working knowledge of current developments within palliative care (up to National level). RESPONSIBILITIES 1. CLINICAL TO COORDINATE CLINICAL SERVICE DELIVERY WITHIN DAY HOSPICE AND OUT-PATIENTS 1.1 In conjunction with the Clinical Nursing Services Manager, to manage and coordinate care provided by all staff working within the Unit, providing professional support and advice as necessary. 1.2 To lead in triaging incoming referrals, allocation of initial assessments and decisions on allocation of placements/attendance at, and discharges from the Day Hospice, working closely with Day Hospice admin to process incoming referrals and placements. This will include carrying out initial holistic assessments and home visits. 1.3 To be responsible for assessing, planning and coordinating new patients referred to and starting Day Hospice. To ensure communication and documentation in relation to new patients starting Day Hospice is of a high standard, comprehensive and promotes continuity. 1.4 To complete holistic assessments with new Day Hospice patients in order to deliver an individualised management plan covering all identified needs. 1.5 To develop, plan, implement, direct and evaluate nursing care and general care for patients in the Unit having regard to the physical, spiritual, social, emotional and psychological needs of individual patients and to have an appropriate level of involvement in direct nursing care, including leading in new clinical procedures and practices. 1.6 To encourage discussion and participation by all team members, giving and receiving patient-centred handovers. 1.7 In conjunction with other members of the team identify and provide support to relatives and friends of patients, maintaining confidentiality and working to Caldicott guidelines at all times and ensuring a good relationship with patients relatives by providing frequent contact and availability. 1.8 To ensure that effective communication is maintained between staff, patients and their relatives so that appropriate support is provided through illness and death and the initial bereavement period. 1.9 Within limits of knowledge and expertise, to work as an advanced practitioner, including independent prescribing, examining and diagnosing and undertaking appropriate management plans and interventional care. 1.10 To ensure that all medication, treatments, therapies and care interventions are administered in accordance with legal, national, local and hospice philosophy, policies and procedures to be responsible for ensuring all staff adhere to same. To regularly review and evaluate treatment and interventions, in liaison with the Consultant in Palliative Medicine and the Specialist Nurse Practitioner. To work with the Accountable Officer and Clinical Nurse Practitioner with regards to Medicines Management. 1.11 To be responsible for reporting, investigating and action planning in relation to drug errors/incidents and drug related near misses, liaising with the Accountable Officer and Head of Care Services as appropriate. 1.12 To formulate, implement, disseminate and monitor amended and new policies and procedures, ensuring agreed standards are maintained, having a thorough working knowledge of policies and procedures, and identifying where existing standards should be reviewed, in a way that ensures commitment, understanding, compliance and best practice from all staff levels. 1.13 To have overall responsibility for ensuring both verbal and written communication is provided to a high standard and that documentation is completed accurately. To be responsible for monitoring, auditing and improving these standards regularly and as necessary. 1.14 In conjunction with the Clinical Nursing Services Manager and the nursing team, to liaise directly and/or indirectly with Hospice staff and other agencies, ensuring good communications exist between all healthcare professional and promote harmonious working relationships and safe practices. 1.15 To explore and develop innovative ways of supporting patients and their carers manage their condition and address changing needs. This will include facilitating new needs/condition specific groups, developing carers support services and developing other agreed initiatives. 1.16 To participate in all relevant multi-professional team meetings when necessary to discuss clinical issues. 1.17 Lead as overall Co-ordinator of the Carers Support Group, ensuring administration of the service by the Day Hospice secretaries is efficient and responding to client group needs. 1.18 Take overall responsibility for the development and promotion of Out-Patient services provided on Procedure and Out-Patients Day, including responding to available services and innovations. 1.19 To work effectively with patients and carers from diverse cultural and social backgrounds. 1.20 To explore, implement and monitor ways of preventing unwanted and/or unnecessary admissions to secondary care. 2. MANAGERIAL/ORGANISATION 2.1 Working in conjunction with the Clinical Nursing Services Manager, Specialist Nurse Practitioner and Unit Sisters, to ensure the smooth, safe and appropriate management of the Unit in the short, medium and long term, with responsibility for ensuring adequate staffing levels and skill mix are maintained in the Unit at all times. To undertake an annual establishment review. 2.2 In conjunction with the Clinical Nursing Services Manager, to appoint, manage, direct, support and discipline where appropriate all nursing (qualified and auxiliary) and volunteer staff working in the Unit, delegating the practice of Individual Performance Review of staff and volunteer staff as appropriate. 2.3 To ensure that all legal and Hospice strategies, policies and procedures are in place and adhered to within the Unit. 2.4 To ensure that a Risk Management programme is in place and that risk assessments are undertaken as part of an on-going programme to maintain a safe working environment. To be responsible for initiating spot checks and action planning re: results of same. 2.5 To participate as required, in the Senior Nurse on-call rota and initiate appropriate action to deal with urgent or emergency situations (clinical and non-clinical) arising from on-call duties. 2.6 To contribute to the overall management and strategic direction of the Hospice, and in particular the future direction and strategy of the Unit. 2.7 To ensure that the requirements of the Care Quality Commission are in place and complied with within the Unit. 2.8 To exercise sound leadership, decision-making, supervisory and managerial skills at all times that motivate staff to develop high standards of care and ensure commitment to agreed standards. 2.9 To be an active member of the multi-professional team (clinical and non-clinical) attending working management meetings and discussions. 2.10 To promote good therapeutic and dynamic communication and working relationships throughout the hospice team. 2.11 To represent the Clinical Nursing Services Manager in appropriate areas when required. 2.12 To be responsible for the arrangement and monitoring of duty rotas, ensuring the correct skill mix and most effective and efficient use of human resources. To monitor the use of and performance of bank staff. 2.13 To report and investigate complaints/untoward incidents/near misses and be pro-active in critically analysing such occurrences. 2.14 Compile and submit monthly, quarterly and annual reports, outlining statistics and activities of Day Hospice department and related services. 2.15 Take overall responsibility for the organising and risk assessing of major events held annually e.g. Christmas celebrations, parties, themes and diversional activities and delegate where appropriate. 2.16 Take overall responsibility for the Volunteer Driving Service acting as mentor and manager to Driving Service Co-ordinator. Lead in promoting staff support services for Day Hospice department staff, in particular, group and individual clinical supervision. 2.17 To be responsible for the training, development and Individual Performance Review ((IPR) of the nursing team to ensure that adequate skill mix of appropriately trained, qualified and auxiliary staff are maintained in the Unit. 3. EDUCATION/RESEARCH/DEVELOPMENT 3.1 Attendance at In-House Study Days and staff meetings (all relevant)) is compulsory. To be responsible for planning and chairing meetings on a rotational basis where appropriate. 3.2 To take responsibility for the planning, delivery and evaluation of the Patient Contact Volunteer Course in rotation with other senior nurses, including post course interviews. 3.3 Lead on mentoring of new members of staff and support existing staff who are mentoring new members of staff. 3.4 To maintain an acute clinical awareness and keep up-to-date and informed on nursing issues and other developments in the field of palliative care. To ensure that such knowledge and information is disseminated effectively to the nursing team and to develop strategies for the implementation of appropriate therapies/policies/procedures in the Unit to respond to changes in accepted practice. 3.5 To participate in the education service in the hospice, including developing strategies and policies to meet the needs of hospice nursing staff and in conjunction with other senior managers the needs of other non-nursing staff groups. Attend Education Planning meetings. 3.6 To actively use reflection in his/her own practice, encourage it in other team members, individually and as a group, and participate in Clinical Supervision as required. To take an active role in the provision of Clinical Supervision to others and support and supervise staff who are providing supervision. 3.7 To maintain and provide accurate education and training related records, reports and statistical evidence, including induction records, as required. 3.8 In conjunction with other clinical departments to ensure the Link Nurse list is up-to-date and Day Hospice Link Nurses are undertaking study and activities relevant to their Link role. 3.9 To ensure audit and monitoring outcomes are both acted on and published appropriately, ensure full feedback to the rest of the nursing team as appropriate. To attend the Audit, Research & Quality Assurance (ARQ) group. 3.10 To participate in the Corporate & Clinical Governance Group, as requested, contributing data and information to support governance systems within Day Hospice and the Hospice as a whole. To be part of the Clinical Development Team (CDT). 3.11 To maintain an awareness of developments and quality initiatives that inform self- governance and best practice. NOTES i) You must be sympathetic to and able to project the philosophy and concept of the Hospice. ii) You will potentially have access to confidential data on staff and/or patients and/or services within the Hospice. Failure to maintain confidentiality will lead to disciplinary action which could ultimately lead to dismissal. iii) You will be required to assess all risks to your systems, processes and environment and contribute towards the clinical and/or corporate governance agenda as appropriate. iv) You will be expected to produce work to a high standard and to promote quality in all areas at all times. v) The managerial and clinical philosophy of the Hospice is based on a multi-disciplinary approach.
