Establish effective working relationships with Primary Health Care teams. Manage, support and develop Senior Hospice at Home Nurses (Rennie Grove Band 6) and Hospice at Home Nurses (Rennie Grove Band 5). Act as a resource for professionals and non professionals involved in palliative care within the community. Promote the Rennie Grove mission and values within the local community. Key tasks / duties: Professional / Clinical Demonstrate clinical excellence and maintain consistently high standards of nursing practice in palliative care. Act as a role model demonstrating high standards of care and providing clinical leadership for colleagues. Advise and support on patients with complex needs and their families at home, maintaining an approach that incorporates physical, psychological, social and spiritual aspects of care, in accordance with the Rennie Grove mission and values. Manage a caseload of complex patients promoting the highest possible standards of individualised care, symptom control and bereavement support through a process of assessment, planning, implementation and evaluation. Effectively allocate and support team members to implement the care plan, monitoring and evaluating the care provided, and delivers practical nursing care when appropriate. Ensure all written and verbal communication is constructive, effective, accurate and appropriate. As Nurse Prescriber, adhere to prescribing guidance for palliative care patients. Responsible for developing and maintaining effective working relationships with community professionals, support services and members of other hospices and palliative care teams, providing clinical expertise, specialist advice and support as requested. Demonstrate a research based approach to nursing and participate in on-going Rennie Grove research. Responsible for identifying and acting on any risk of safeguarding to patients and their families in accordance with the Rennie Grove Safeguarding of Vulnerable Adults and the Safeguarding of Children Policies and Procedures. Participate in setting standards and the audit of patient and family care. Keep accurate and legible records in accordance with legislative requirements and Rennie Grove practice and procedure. Regularly attend and contribute to team, hospice, regular locality multi-disciplinary team and cancer network meetings, recognising the importance of peer support. Follow Rennie Grove procedures and statutory regulations relating to custody and administration of drugs. Work to structured shift patterns (early, day and late shifts) on a rota basis from Monday to Sunday and minimum of one overnight shift per year. Promote equality and value diversity. Managerial Effectively allocate, support and monitor the work of the team, including Bank Nurses, in collaboration with other team Clinical Nurse Specialists. Deputise for Locality Nurse Manager on a rotational basis and undertake duties as required, for example, authorisation of expenses, service reports, staff development, off duty planning etc. Manage performance and professional development of Senior Hospice at Home Nurses (Rennie Grove Band 6) and Hospice at Home Nurses (Rennie Grove Band 5). Act as a co-mentor for new staff and oversee successful completion and timely sign off of their baseline competencies. Carry out individual performance review process (annual and mid year reviews) for direct reports and complete personal development plans. Receive and disseminate information through attendance at team and hospice meetings and through liaison with other hospices and Primary Health Care teams, locality multi disciplinary team and Cancer Network. Prepare and provide regular information to Locality Nurse Manager as required for budgetary control, capacity and service provision and development. Assist in maintaining patient statistics by keeping up to date patient notes and administrative records. Maintain record of equipment borrowed and returned. Participate in agreed methods of off duty planning to provide a 24 hour Rennie Grove service for patients, their families and the Primary Health Care teams. Participate in service reviews and the development, implementation and evaluation of Rennie Grove policy, as required, to drive service improvement. Represent Rennie Grove externally as required. Positively support the Locality Nurse Manager in managerial decisions and change management issues. Monitor and prioritise team referrals with an awareness of team working capacity. Educational Act as clinical supervisor for designated clinical staff. Participate fully in the orientation / mentorship of new staff. Supervise and teach Rennie Grove trained staff. Provide specialist palliative care training to multi disciplinary team colleagues and nursing home colleagues within the community. Provide support and teaching to students on Rennie Grove work placements. Personal and professional development Comply with all Rennie Grove policies and procedures. Adhere to the NMC Code of Professional Conduct and national and local standards and guidelines. Maintain required organisational standards for all mandatory training. Keep up to date with current legislation, professional practice issues and relevant developments in palliative care and bereavement.
