JOB DESCRIPTION Title: Day Hospice Nurse Grade/Salary: Agenda for Change Band 6 Responsible to: Accountable to: Location: Senior Day Hospice Manager Director of Nursing Blackett Avenue, Malpas, Newport Hours of Working: 37.5 hpw Key Working Relationships: Multi-professional members of day hospice team Community based palliative care teams Day hospice administrative support Day hospice volunteers Job Summary: The post holder will manage and work alongside the day hospice team to provide a high standard of palliative care within the day hospice setting. The post holder will contribute and lead day hospice developments and will work closely with members of the multi-professional day hospice team to ensure consistency of approach and intervention. The post holder will deliver and coordinate oncology and supportive care treatments. Key Areas of Responsibility: CLINICAL Facilitate the development of a friendly therapeutic environment within the day hospice To take the lead in developing a multi-professional assessment of day hospice patients in order to plan individual programmes of care Implement, monitor and evaluate programmes of care as appropriate Maintain accurate records, ensuring all relevant information is documented appropriately. Assist patients with mobility and personal hygiene as appropriate Work and liaise with clinical nurse specialists to provide effective symptom control to day hospice patients Work with the primary healthcare team as and when appropriate
Develop day hospice service by extending overall provision to include carers group, young people s groups and various clinics run by other members of the multi-professional team To work with the chemotherapy outreach team from Velindre NHS Trust Comply with all St David s Hospice Care clinical policies Develop IV transfusion clinic for oncology and palliative care patients Work alongside CNS s in the community as required MANAGEMENT Ensure the efficient use of resources to meet service objectives To oversee the preparation of the required statistics and activity data and ensure they are accurate Assist in the implementation of policies, procedures and clinical protocols, relating to day hospice Assist in the development and audit of the standards related to palliative day care Ensure all day hospice equipment is properly maintained To lead the day hospice team to develop services To participate in weekly team manager meetings and feed backfeedback information as required To act as the patients advocate at all times actively participating in the timeliness of admission and discharge procedures Participate in the development of Clinical Practice guidelines using skills and experience to promote knowledge for patients and staff To undertake further education in Palliative Care as part of the role development Act as mentor and/or coach to the development of staff and students, providing facilities and resources for clinical supervision Attend mandatory lectures and training as required, to ensure a safe working environment
EDUCATION, RESEARCH AND AUDIT Assist in the preparation and provision of learning materials relevant to day care in collaboration with the education department Participate in nursing and collaborative research in palliative day care Maintain accurate documentation to facilitate reliable clinical audits Maintain own clinical competencies and professional development commensurate with the role development Additional Duties All job descriptions are subject to review. OTHER INFORMATION Unsociable Hours: There may be an occasional need for unsociable hours, e.g. attendance at fund-raising events meetings and conferences. Time in lieu will be given in respect of this. PERSON SPECIFICATION Essential Education First Level Registered Nurse Relevant degree Experience Three years post registration experience Demonstrate a proven commitment to palliative and cancer care by evidence of professional development and clinical experience Skills / Knowledge Awareness of Clinical Governance issues Recent and up to date knowledge of chemotherapy regimens Ability to cannulate Excellent communication including: - verbal, written and presentation Willingness to work as a member of a multiprofessional team and with members of the primary healthcare team Ability to develop professional relationships with healthcare professionals Excellent interpersonal and communication skills Desirable Diploma module in palliative nursing Teaching qualification Evidence of leadership development Qualities/ Attributes Flexible Highly motivated Enthusiastic Team Player Positive role model
