8 History of the International Association for the Study of Lung Cancer The Beginning In Oct. 1972, the National Cancer Institute sponsored the First International Workshop for the Therapy of Lung Cancer at Airlie House Conference Center outside Washington D.C. The exchange of ideas among this multi-disciplinary international group was both productive and stimulating. At the concluding meeting, Dr. David T. Carr from the Mayo Clinic, USA, introduced the idea of forming an international organization to continue this fruitful exchange of ideas, and generally enthusiastic support for the concept was voiced by those present at the workshop. With the leadership of Dr. Carr and the help of Dr. Oleg S. Selawry, Drs. Lawrence Broder and George Higgins from the Washington V.A. Hospital, USA, an organizational committee began the task of development. By mid-1974, approximately 250 individuals had accepted membership in the Association. The first organizational meeting was held in conjunction with the XIth International Cancer Congress, sponsored by the UICC in Florence, Italy and a slate of officers and members of the board of directors were unanimously elected. World Conferences The First World Congress on Lung Cancer, sponsored by IASLC, was held in May of 1978, at Hilton Head, South Carolina, with large attendance by participants from throughout the world. 10 Significant Moments In addition to the World Conferences, the IASLC hosts educational events each year. Activities have included regional conferences, workshops, symposia, colloquiums and webinars. Often the meetings highlight a specific topic; they ve included therapy for non-small cell lung cancer, prevention, molecular targeted therapies, small cell lung cancer and many more topics. In 2000, IASLC published its Tokyo Declaration on Tobacco in 2000, urging governments to take action against the alarming rate at which lung cancer was occurring in men and women. In 2007, IASLC revised its Declaration on Tobacco, adding a recommendation that governments initiate legislative action for ultimate elimination of manufacture and sales of tobacco products. In 2006, IASLC published its first statement on CT screening for lung cancer, based on preliminary results from the National Lung Screening Trial (NLST) in the. On July 4, 2011, IASLC revised its Statement on CT Screening for Lung Cancer to take into account new data and results from the NLST, as well as ongoing trials around the world. An IASLC Task Force will continue to make recommendations around screening. In 2008, the IASLC began partnering with other organizations to hold joint conferences. The IASLC and ESMO held the Joint 1st European Lung Cancer Conference in Geneva, Switzerland in April of The 1st Latin American Lung Cancer Conference was held in Vina del Mar, Chile Oct And the 1st Asia Pacific Lung Cancer Conference was held in Hyderabad, India Dec In 2009, IASLC unveiled the International Staging Classification System for Lung Cancer, developed in collaboration with the American Joint Committee on Cancer (AJCC) and the Union for International Cancer Control (UICC). Although the TNM (tumor, node, metastasis) classification of malignant tumors had existed for decades, the 7th TNM revision used an international database developed with IASLC to broaden understanding of the clinical staging of lung cancer. In 2011, IASLC published the International Multidisciplinary Classification of Lung Adenocarcinoma, the first revision to the classification in six years and the first to be developed by a multidisciplinary panel of experts. IASLC developed the classification with oversight from the American Thoracic Society and the European Respiratory Society. The vision of the IASLC is to conquer lung and thoracic cancers through research, education, prevention and treatment. To ensure that the IASLC achieves that future, the Board of Directors embarked on a strategic planning process in After conducting research, asking for input and careful thought, a plan was established and approved Sept. 12, Implementation of this plan is underway. In 2013, IASLC began producing Annual Reports to detail IASLC activities, achievements and progress. IASLC 2013 and 2014 Annual Reports may be found at A 35-year history of the IALSC was published in 2009, with Heine Hansen, MD, editor. Further details on the association s history can be found in that book, which can be requested by contacting the IASLC. In 2013, the IASLC celebrated 40 years of progress in lung cancer, and produced a series of videos and a monograph titled IASLC : Celebrating 40 Years of the IASLC and Research Advancements, with support for the project by Eli Lilly and Company. The videos and monograph are located on the IASLC website. 10

11 IASLC 2007 Declaration on Tobacco Lung cancer is the leading cause of cancer death in the world. The incidence is rising at an alarming rate in both men and women, particularly in the developing world. Ninety percent of cases are caused by active or passive smoking and, therefore, could be avoided or prevented. In order to achieve its goal of eliminating lung cancer, the IASLC makes recommendations to governments world-wide. In order to help achieve its goal of eliminating lung cancer, the IASLC: 1. Requests governments to: Ratify the Framework Convention on Tobacco Control and work to speedily implement its Articles. Work within each of the members Cancer Centers or Clinics to promote smoking cessation and a com plete smoke-free workplace campus. Promote smoking and all tobacco prevention programs, particularly those that are focused on youths. Promote legislation or regulations that enforce smokefree public places, including transportation systems particularly to protect non-smokers from the effects of secondhand smoking. Promote educational programs that warn of the deadly effects of tobacco on health, including programs with the school systems. Increase taxes on tobacco in order to decrease its use. Encourage governments to enforce anti-smuggling laws with increased surveillance. Initiate legislative action forultimate elimination of manufacture and sales of tobacco products. 2. Encourage other medical societies and health care institutions to join in campaigns against smoking and for anti-smoking education. 3. Encourage all health care providers to receive training in counseling on stopping smoking. 4. Requests all industrial sectors and media organizations to eliminate tobacco advertising and marketing. 5. Promises to produce materials that will aid in education concerning smoking and lung cancer. 12 IASLC 2011 Statement on CT Screening for Lung Cancer Published July 4, 2011 Screening can be improved through ongoing research, which is essential as the resolution of spiral CT imaging continues to improve, and this Lung cancer is the most lethal cancer worldwide, partly because it is usually diagnosed too late for effective treatment. If diagnosed early enough, lung cancer often can be cured by surgery or radiotherapy. can drive further progress with safer and more effective surgical management approach. Additional research also will define risk profiles of individuals who would benefit most from screening. On June 29, 2011, the New England Journal of M were screened regularly using A crucial factor required in the implementation of future national low-dose spiral computed tomography (CT) compared with standard chest x-ray. The study followed more than 53,000 current and former smokers ages 55 to 74. It was halted a year early because the reduction in cancer deaths provided an answer to the study s main question. Thus, low-dose spiral CT is the first test of any type to demonstrate a significant reduction in lung cancer mortality through early detection. This innovation provides a great opportunity for lung cancer clinicians and researchers across the world to work responsibly to provide, study and refine this new screening programs will be the participation of multidisciplinary groups of trained specialists in the relevant aspects of early lung cancer. In each country, an assessment of lung cancer screening benefit, implementation costs and potential harms must be defined in a cultural context, so that national policies about screening implementation and issues such as quality control and professional credentialing standards can be decided. Different nations will need to undertake individual health technology assessments to inform on national screening programs. approach within future clinical trials and national screening programs. IASLC encourages people to be enrolled into screening trials so that Although the NLST is the first randomized clinical trial to show a significant decline in lung cancer deaths, there are a number of opportunities to improve further this approach. There is a need to introduce quality control measures to ensure the quality of the screening management. For example, the follow-up of patients with indeterminate nodules is critically important and should be done by a team experienced in evaluation of such nodules to ensure safe, economical screening care. There are a number of trials currently under way that could provide relevant data. One of the largest is the Dutch-Belgian NELSON, a further information can be acquired as soon as possible. In light of currently available information, some people may seek to undergo CT screening now, outside of a research trial. They should discuss this with their doctor so they have access to the best available information regarding the potential benefits and risks of this approach for their situation. The IASLC website will provide objective lung cancer screening data to assist such discussions. IASLC is committed to ensuring broad communication about the potential health benefit from CT screening, as well as providing objective evidence on potential harm. population-based trial of more than 20,000 smokers, which uses refined CT IASLC will continue to advocate for effective tobacco control imaging techniques. The results of the NELSON trial will provide critical additional information not only on mortality advantage and cost effectiveness but also on the clinical management outcomes of lung cancer screening. It is appropriate for heavy smokers ages 55 to 74 to discuss relevant lung cancer screening information with their physicians to assist them in deciding whether to undergo spiral CT screening. programs in all healthcare provider venues, so that together with lung cancer screening we can most effectively reduce deaths from lung cancer. Approximately 90% of lung cancer occurs in people who have a history of smoking. If we can persuade youth not to start and adults to quit, there will be markedly less tobacco-related death. However, since an elevated risk of lung cancer persists in former smokers, it will be important to better integrate the public health messages for both tobacco control and lung cancer early detection.
