1 Lisa Lin Foundation Scholarship Master of Business Administration Program Master of Business in Healthcare Management Program In the interests of encouraging the study of Traditional Chinese Medicine and Business, and to further the knowledge that will enable students to succeed, the Lisa Lin Foundation is pleased to announce that all new and transferring applicants for admission to the Texas Health and Science University s Master of Business Administration or MBA in Healthcare Management program may apply for a tuitionreimbursement scholarship. Program of Study Initial which Scholarship Program Amount Master of Business (MBA) Up to $4,000 Master of Business Administration in Healthcare Management (MBAH) Up to $5,000 Trimester Fall /01/14 Spring /01/14 Scholarship Application Deadline Eligibility criteria are as follows: 1) Applicants must have earned a Bachelor degree or higher at an academic institution accredited by an agency recognized by the Secretary of the United States Department of Education prior to application to the Texas Health and Science University and have earned a cumulative grade point average of not less than 3.0 on a 4.0 scale; 2) Applicants who have been accepted for admission to the Texas Health and Science University s Master of Business Administration or MBA in Healthcare Management program shall have completed registration and all other application, admissions, and enrollment requirements; 3) Not later than the scholarship application deadlines posted above, applicants shall SCHOLARSHIP DISCLOSURE AND AGREEMENT rev Page 1 of 7

2 submit the following to the Texas Health and Science University s Admissions Office: A) A letter of interest; B) A curriculum vitae; and, C) Official copies of all transcripts from all post-secondary institutions attended prior to their application for admission to Texas Health and Science University. 4) Applicants MUST complete registration by the required date for their entering trimester and MUST remain enrolled full-time for the duration of the program. A student WILL NOT receive any scholarship award increment for any trimester in which the student enrolls part-time, or for additional trimesters necessary for the student to complete the program of study beyond the regular curriculum schedule. 5) Funds are awarded on a first-come, first-served basis. Scholarship funds are payable in equal increments for each trimester of full-time enrollment, except for the student s final trimester of full-time study, upon which a larger final increment shall be payable in accordance with the schedule outlined in Section B of this agreement. Funds are payable once a full-time student has successfully completed 60% of scheduled course days and all course requirements up to that point are satisfied. 6) Scholarship funds SHALL BE TERMINATED if a recipient withdraws from, is dropped from, or fails to maintain satisfactory academic progress in the Master of Business Administration or Master of Business in Healthcare Administration program by maintaining less than a 3.0 cumulative GPA. Additionally, if a scholarship recipient withdraws from, is dropped from, or does not successfully complete the degree program within 150% of the required trimester timeframe resulting in dismissal, that individual MUST reimburse all previously received scholarship funds to the Lisa Lin Foundation within thirty (30) days of terminating study. (For example, if a full time student would normally complete a degree program in 4 trimesters, the longest he or she may take to successfully complete the program is 6 trimesters.) Any scholarship funds received and not returned within thirty (30) days shall incur an interest rate of 4 percent per annum and no transcript will be released until the funds are returned in full. 7) The Lisa Lin Foundation is an independent third-party, non-profit organization which reserves the right to rescind, modify or decline the award of a scholarship. 8) Funds provided to scholarship recipients under this scholarship program SHALL NOT EXCEED the maximum scholarship amounts indicated in the table above. 3 AGREEMENT MBA and MBAH Programs All applicants must read the following terms, conditions, and disclosures, and indicate their agreement thereto by signing this document in the indicated place(s) prior to being considered for the tuition-reimbursement scholarship. A. GENERAL TERMS AND CONDITIONS 1. This offer of funding assistance is contingent upon an applicant successfully completing all admissions and enrollment requirements of the Texas Health and Science University s Master of Business Administration (MBA) or Master of Business Administration in Healthcare Management (MBAH) program. Upon completing those requirements, the applicant must submit to the Lisa Lin Foundation a physical or electronic copy of an acceptance letter or other document indicating the student s admission to the MBA or MBAH program, issued by the Texas Health and Science University for the enrollment period the applicant first plans to attend; 2. Once an applicant signs the scholarship application and submits it to the Lisa Lin Foundation for consideration, s/he is thereby authorizing the Texas Health and Science University through its staff to provide the Foundation with documents and information regarding the applicant s admission and enrollment at the University. Such documents include, but are not limited to, copies of enrollment agreements, registration for coursework offered by the Texas Health and Science University, grade reports, and any other information or documentation regarding an applicant s admission, enrollment, attendance, or financial billing with the Texas Health and Science University; 3. Notwithstanding the provisions of (1 and 2) above, the applicant agrees to provide the Foundation, upon request, documents and information regarding his or her application, admission, enrollment, attendance, or financial billing for the first academic term of his or her enrollment in the Texas Health and Science University; 4. Any tuition-reimbursement payment(s) made under the terms of this scholarship offer shall be made to a successful applicant upon the Foundation s receipt of a certified copy of grade reports, billing statements, or other documents attesting to the successful completion of a minimum of 60% of scheduled course days and all other coursework requirements up to that point in each of the applicant s trimesters of enrollment at the Texas Health and Science University; 5. No applicant for this scholarship shall receive any reimbursement for any trimesters in the Texas Health and Science University s Master of Business Administration or MBA in Healthcare Management program in which he or she was enrolled for less than full-time (9 credits) as defined by the University for the enrolled program; Additionally, if any student awarded a scholarship under either program enrolls as a full-time student and then later drops courses and thereby becomes a part-time student, the scholarship recipient SHALL BE IMMEDIATELY TERMINATED from the scholarship program and SHALL BE subject to the same termination and reimbursement provisions outlined in paragraph 6 of the Eligibility Criteria. 6. Any additional trimesters necessary for the student to complete the program beyond a total of 4 trimesters for the MBA program or 5 trimester for the MBAH program, of full-time enrollment will not result in additional scholarship funding; 7. Any trimester in which the student enrolls part-time shall result in no scholarship award SCHOLARSHIP DISCLOSURE AND AGREEMENT rev Page 3 of 7

SPECIAL PROVISIONS (For recipients who are not citizens or permanent US residents) The Foundation reserves the right to modify its Agreement regarding Scholarship payments in the case of recipients who are not citizens or permanent residents of the United States of America. Modifications in the case of such students (hereinafter referred to as International Students ) are as follows: 1) Total award amount payable for an International Student enrolled in the Master of SCHOLARSHIP DISCLOSURE AND AGREEMENT rev Page 4 of 7

5 Business Administration or MBAH in Healthcare Management program SHALL NOT exceed a total of $ (five hundred dollars) over the length of the program in which the International Student shall be a recipient of a scholarship offered by the Lisa Lin Foundation; and, 2) Funds are payable by the Foundation to or on the behalf of the eligible International Student as follows: a. 20% ($100.00) on or about the date corresponding to the eligible International requirements up to that point in the first trimester of funding under this program; b. 20% ($100.00) on or about the date corresponding to the eligible International requirements up to that point in the second trimester of funding under this program; c. 20% ($100.00) on or about the date corresponding to the eligible International requirements up to that point in the third trimester of funding under this program; d. 40% ($200.00) on or about the date corresponding to the eligible International requirements up to that point in the third trimester of funding under this program; and, e. All payments under this section shall be contingent upon the eligible International Student maintaining good standing and satisfactory academic progress, including maintaining a cumulative GPA of 3.0, as defined by the Texas Health and Science University. D. APPLICANT/SCHOLARSHIP RECIPIENT RESPONSIBILITIES 1. Applicants for this tuition-reimbursement scholarship shall report to the Foundation within 14 days of a trimester s scheduled start date that the applicant will not maintain full-time enrollment for that trimester at the Texas Health and Science University; 2. Applicants shall provide the Foundation with a letter of interest, curriculum vitae and official copies of all transcripts from all post-secondary institutions attended prior to their application for admission to the Texas Health and Science University. Applicants authorize the Texas Health and Science University to provide the Foundation with copies of all such documents from the student s individual file maintained by the admissions and recruiting office of the Texas Health and Science University; 3. Once the Foundation awards an applicant a tuition-reimbursement scholarship under this program, the recipient shall provide the Foundation with a certified copy or other official document issued by the Texas Health and Science University indicating satisfactory attendance and academic performance in, and successful completion of, 60% of scheduled course days in each of the four payable trimesters of enrollment as a full-time student in the Master of Business Administration or MBA in Healthcare Management program at the Texas Health and Science University; 4. Scholarship recipients shall be enrolled for the regular curriculum schedule for their program at the Texas Health and Science University, and MUST successfully complete a minimum of 9 credits per trimester for each of the trimesters of their enrollment in the University s MBA or MBAH degree program; SCHOLARSHIP DISCLOSURE AND AGREEMENT rev Page 5 of 7

Applicants who receive payment under the terms of this Agreement, but fail to complete the MBA or MBAH program within 150% of the required trimester timeframe MUST repay the Foundation within (30) days of terminating their program. The recipient may also enter into a signed and notarized scheduled payment arrangement with the Lisa Lin Foundation for any unpaid balance remaining after the (30) day grace period. Any remaining reimbursement amount that is due and not returned within thirty (30) days shall incur an interest rate of 4 percent per annum. 6. Applicants approved for this scholarship consent to the use of their name, image, and likeness in any and all announcements or public presentations of information concerning the Texas Health and Science University and/or their enrollment therein. E. APPLICANT/RECIPIENT RIGHTS 1. Applicants for this scholarship have the right to privacy and security in the information provided or made available to the Foundation; the Foundation WILL NOT share, disclose, or otherwise disseminate or cause to be disseminated any information or data concerning applicants, except as specifically outlined in this agreement or with the individual applicant s express written permission; 2. If a recipient is denied payment of scholarship funds by the Foundation for unsatisfactory performance during any trimester, the recipient has the right to petition the Foundation for reconsideration. In such instances, the applicant/recipient may petition the Foundation in writing, providing specific details relating to the cause(s) of their unsatisfactory performance, and the measures the applicant intends to take to prevent a future recurrence of such circumstances. The Foundation will review the petition and notify the applicant/recipient in writing as to their final determination in the matter; and 3. Applicants for this scholarship have the right to a fair consideration of their application; the Foundation shall not evaluate applicants on the basis of any factors other than those expressly set forth herein. By my signature below, I swear or affirm that I have read and understand the foregoing, and agree to be bound by the terms, conditions, and disclosures contained therein. Furthermore, I agree to the following conditions: 1. If approved for this tuition-reimbursement scholarship, I shall remain enrolled in the Texas Health and Science University s MBA or MBAH degree program for the duration of the program, and shall register for and complete not less than full-time coursework per trimester for each trimester; 2. During all periods of Foundation-determined eligibility for this scholarship, I shall remain in good academic, disciplinary, and financial standing with the Texas Health and Science University; 3. During all periods of Foundation-determined eligibility for this scholarship, I shall provide the Foundation, upon request, with any and all documents, information, or other data regarding my admission, enrollment, financial account, attendance, and academic performance at the Texas Health and Science University; 4. I authorize the Texas Health and Science University to provide the Foundation with information and/or documentation as set forth in (3) above and elsewhere in this Agreement; 5. I understand that the Foundation WILL NOT provide funding under this Agreement SCHOLARSHIP DISCLOSURE AND AGREEMENT rev Page 6 of 7

7 during any period of part-time enrollment or attendance subsequent to the completion of the regular curriculum schedule in the Texas Health and Science University s MBA or MBAH program; 6. I understand that the Foundation may, upon good cause, discontinue payments made under the terms of this Agreement, and I agree to hold the Foundation and the Texas Health and Science University wholly harmless in the event of such action.
