PLEASE READ: Please note that there are questions in this application that may not apply to you and your situation (especially on page 6). This application covers well over 30 different scholarships and some have specific requirements. Just because you do not qualify for one scholarship does not mean you would not qualify for another. One of the reasons we use a joint application is to allow the applicant to file for all of the scholarships at once. We will determine your eligibility for each different scholarship. Therefore please complete the application answering all of the questions to the best of your ability.

3 PERSONAL STATEMENT Please use this sheet to write a personal statement (not to exceed 500 words and one page total). This application has also been created in a PDF format to allow you to type the information directly into the application. If you are using the PDF application you may wish to type and edit the Statement in another computer program and then copy and paste below. The statement should include your educational and career goals. Also explain what you have done or plan to do to help yourself achieve these goals. Please do not repeat information from elsewhere in this application.

4 ACTIVITIES Please list below all school, church and community activities during your college years or post high school activities if you are a Non-Traditional Student. Include any special projects or awards and when you were involved in each activity. Examples for College students: Marching Band 13(or First year) Alpha Beta Gamma Fraternity 15 (or Third Year). Examples for Non- Traditional Students: Middle School Soccer Coach , Run For It Library Team You may need to use more than one page. Please note that you are required to use this form. Please make multiple copies for additional pages. Other types of submissions will not be accepted.

Incomplete applications will not be forwarded to the Scholarship Review Committee. 8 Tip Sheet Tips for College Student/Non-Traditional Applicants: Please complete your activity sheet for any activities you participated in from High School graduation and beyond. Make sure the activity sheet is complete. You must submit an official college transcript. Most colleges and universities prefer to submit transcripts directly to the Foundation so make sure they have the correct mailing address for TCF. Applications by college students without an official transcript are typically not competitive. Early application is encouraged.
