6 No-Cost Term Life Insurance for New Members 888/ All new AFT members who are actively employed are entitled to $12,000 of term life insurance coverage for a period of one year at absolutely no cost. An additional benefit of $12,000 will be paid in the event of an accidental death. New members must complete, sign, and return the $12,000 nocost life activator form to obtain this insurance coverage. The activator form must be submitted within 90 days of becoming an AFT member or within 90 days of becoming aware of the benefit within the first year of AFT membership. The new member $12,000 insurance activator form may be printed from the AFT insurance Web site, (Your signature is required.) Coverage is provided by Metropolitan Life Insurance Company. This coverage will remain in effect for the 12 month period as long as the insured is a member in good standing of the AFT. Disability Income 888/ A voluntary insurance program that will provide non taxable replacement income in the event a member is unable to work due to an accident or extended sickness. This plan will coordinate with other sources of income when a member is unable to perform the duties of his or her job. Voluntary disability insurance replaces up to 60 percent of base earnings in the event of a disabling accident or sickness, with a maximum monthly benefit of $5,000. Two plans are available: 14-day elimination period with a two-year benefit, or, a 60-day elimination period with a five-year benefit. Pre-existing conditions are eligible for benefits after coverage has been in effect for 12 months. Disabilities resulting from pregnancy are covered. No medical exams or blood tests. 7 Voluntary Term Life What if you hurt your back and couldn t work for an extended period? Disability income insurance helps create a personal safety net by replacing a portion of your income in the event of a disabling illness or accident. 888/ Voluntary term life insurance is designed to provide supplemental insurance in the event of death or an accident leading to the loss of a limb, hearing, speech, sight, or paralysis of the arms or legs. Voluntary supplemental term life insurance coverage in amounts from one to six times annual base earnings, up to a maximum of $1 million. No medical exams or blood tests. Pays additional benefits in the event of accidental death or specified injuries. Child Care Benefit, Child Education Benefit and six other benefits automatically are included as part of each member s coverage. Members may convert the term coverage to a permanent insurance plan upon retirement (there is no cost for this conversion privilege). Coverage is underwritten by Metropolitan Life Insurance Company. 8 Universal Life Insurance 888/ Voluntary universal life insurance provides members with an opportunity to obtain permanent cash-value insurance protection for you and your family. If actively at work, a member can obtain up to $150,000 of protection with no medical tests or health questions. Members can apply for an additional $150,000 of coverage by answering a series of health questions. Spouses/partners and children also can obtain coverage by answering a few simple health questions. Universal life provides cash-value life insurance protection to age 95. If actively at work, members can obtain up to $150,000 of coverage regardless of past health histories or pre-existing conditions. The long-term care living-benefits rider provides up to two times the face amount in benefits if a member receives long-term healthcare assistance at a certified facility or at home. Additional riders provide for an accelerated death benefit in the event of a terminal illness, waiver of premium and guaranteed options to increase coverage in subsequent years. Coverage is available for spouses, domestic partners and children. The plan is portable, with no changes, upon retirement. Underwritten by Transamerica Occidental Life Insurance Company. 9 Limited Supplemental Medical Plan 888/ The limited supplemental medical plan (LSMP) offers costeffective, limited health insurance to AFT members and eligible dependents who might otherwise go uninsured due to lack of employer coverage or the cost of plan participation. It can also help members with high deductibles and/or co-payments. The LSMP coverage includes limited reimbursement benefits for doctor visits, X-rays and laboratory testing and emergency room visits (accident-related only), as well as a prescription drug benefit for generic drugs. An expanded plan is also available that provides a benefit payment for hospital confinement, surgery and anesthesia. Both plans offer optional coverage for dental and/or vision services. Medical benefits are paid directly to the subscriber. The prescription program offers direct payments to over 25,000 participating pharmacies. Two levels of benefits allow members to choose a plan that best fits their needs and pocketbook. Base plan includes benefits for doctor visits, X-rays and lab tests, and use of emergency room services for accidental injuries. Enhanced plan provides benefits for hospital confinement, surgery and anesthesia. Both plans include a prescription drug benefit with $10 co-payment on all generic drugs and discounts on brand-name drugs. Optional dental and vision plans are available. Survivor benefit included. Continues coverage for dependent family members in the event of the death of the primary insured. Plans contain no pre-existing condition limitations, no required networks and no medical underwriting requirements. Claims are paid regardless of any other coverage a member may have. 10 Catastrophic Major Medical Insurance 888/ The Catastrophic Major Medical Plan is designed for AFT members and their families to help avoid the financial hardships that accompany major illness and injuries. The plan acts as an umbrella which supplements your basic health insurance by paying expenses not covered by your primary coverage. The chosen deductible is satisfied from all medical bills incurred within a 36 consecutive month period. Choice of two deductible amounts: $25,000 or $50,000. $2,000,000 all cause plan maximum. Coverage for members, spouses and dependent children. Covers expenses in or out of the hospital. Covers home health care, convalescent home care and hospice care. No medical exam is required. Coverage does not terminate because of age. Participants must be covered by a basic health insurance plan that meets minimum coverage requirements. 34 AFT Endorsement and Reimbursement Disclosures The American Federation of Teachers Benefit Trust ( Benefit Trust ) is a tax-exempt, not-for-profit trust fund, established for the purpose of providing benefits to AFT members. Union membership dues paid to AFT or its locals are not used to pay the operating expenses of the Benefit Trust. The Benefit Trust receives funds to cover operating expenses from negotiated expense reimbursement and/or endorsement arrangements from some of the companies who provide these benefit programs to members. The reimbursement and/or endorsement arrangements may consist of a dollar amount per AFT member using that benefit or may be percentage of premiums paid by AFT members who are insured by an insurance provider. Specific information pertaining to these expense reimbursement/endorsement arrangements is included in the communications materials and application or enrollment forms for each program that has such an arrangement in place. Expense reimbursements are used solely to defray the costs of administering the programs of the Benefit Trust including compensation, rent, office expenses and the cost of retaining outside consultants, auditors, and attorneys to advise the Trustees and staff. Where appropriate, revenues from expense reimbursements and endorsements are used to enhance the benefit programs for participants. Just as no membership dues are used to support the Benefit Trust in any way, no expense reimbursement /endorsement funds received by the Benefit Trust are paid to the American Federation of Teachers, except for the reimbursement costs of office space and personnel provided by AFT as determined by independent auditors.
