3 Ageing In Place. (A Service Guide for Healthy Ageing) Ageing is a privilege and a societal achievement. It is also a challenge, which will impact on all aspects of 21st century society. It is a challenge that cannot be addressed by the public or private sectors in isolation; it requires joint approaches and strategies. - World Health Organisation* * World Health Organisation (n.d.) The world is fast ageing have we noticed? Retrieved 16 Apr 2007 from ii

4 Ageing In Place. (A Service Guide for Healthy Ageing) A Note from SingHealth An ageing population will pose increased health, economic and social challenges to Singapore. There has been a rapid expansion of health and community services for the older persons, both by the government and voluntary welfare organisations, to meet their health and social needs. Many older persons are very healthy and lead independent lives. However, there are also others who require health and support services. Various home-based community services are available to assist the older persons and their caregivers to help ensure appropriate care at home. In SingHealth, we believe that everyone should be empowered to manage his or her own care. True empowerment stems from ready access to information. With this ready access, one is supported to make critical and informed decisions about their health and lifestyle. This is paramount for the older persons and their caregivers. This guide aims to create awareness among healthcare providers in the community on the types of services available for the older persons, so that they can better advise those under their care. A joint effort with the National Council of Social Service and Integrated Care Services, this is a SingHealth initiative to bring together key stakeholders in aged care, harnessing the strengths of each partner in creating an effective community healthcare model for the people in Singapore.
