1 Bathurst The Bathurst Cardiac Rehab program has had a busy year. During the month of February they hosted a staff blood pressure clinic. By doing this they were able to screen over 150 people for Hypertension. The clinic was also a great way to provide education and information on the importance of blood pressure awareness and the importance of knowing your numbers. The Bathurst site was also involved with the Big Bike Ride in support of Heart and Stroke. They were able to raise $ Nancy was also very excited to share that there was an addition to the CR team! Brenda Pitre RN started full time with the team! The Bathurst site also hosted the Walk of Life on September 7th raising $ to help advance the program. ACRN CAMPBELLTON The Campbellton area Cardiac Rehab program has added another element of wellness to their program. Nancy and the staff have added a Mindfulness session which was a hit with the participants proving them with another avenue to relax. Nancy also stated that she draws much of her enthusiasm and motivation for the clients as they bring great joy! Nancy identified the HTN web site, Med scape and the Canadian Obesity Network as great resources for education material Nancy and the CR team hope to be able to host a walk of life next year. Currently Dr.Whalen and Nancy Duguay are the only active members 1

2 Caraquet Nicole Martin of the Caraquet Cardiac Rehabilitation site continues to host 3 month sessions to the Francophone population. Her goal is to host a walk of life next year. Nicole's great passion for nutrition and well being keeps her engaged and delivering a high energy program to this population. Grand Falls From right to left : Nicole Martin (RN) Chantal LeChance (Physiotherapist) and Louise Theriault (Dietitian) The Grand Falls Cardiac Rehabilitation team continues to host the heart to heart program providing education and support to the participants. The team also provide 8 week programs that consist of education and support year round. In the gym they have started posting different posters that provide the participants with heart health facts and information. This way the participants are able to read the facts as they are exercising. Fredericton The Fredericton CR team has been busy! Due to the wait times between programs they have started a program called Your Heart Your Health. This program consists of four two hour sessions that run every six weeks. The participants meet all the team members who educate and empower them until they are able to attend a program. The Fredericton team has also been involved with the Ottawa model smoking cessation program geared towards the heart patients. Some helpful web sites identified by the Fredericton site are this site provides basic information and specific behavioral changes designed to improve sleep quality and reduce insomnia. Another great site Time Management: pages/main/newmn_hte.htm Use this with someone who is experiencing workplace stress due to issues with workload procrastination. It is also helpful for people who have difficulty prioritizing important activities in their daily routine. 3 ACRN Moncton The Moncton site has relocated to The Moncton Hospital, Clinic A in the new ambulatory care center. They are in a bright modern space within the healthy living clinic (diabetes education, chest clinic, obesity clinic and ambulatory care RD's), and will be a very nice fit. hoping to have one in the Spring. The Moncton site uses the Heart and Stroke website to help educate patients along with YouTube as a teaching aid regarding CVD. Kelly stated that the CR team is involved with the ABI clinic delivering 1:1 education, smoking cessation program, and they also host a post program walking program. Kelly also stated that the team is passionate about CR. They love to motivate and empower the patients to make lasting changes. For Heart Month they raised money by selling hearts for the Heart and Stroke Foundation The Moncton site had to cancel their Walk of Life event due to the relocation but are Sharon Pitre RD, Lisa Steeves RN, CCN (C), Kelly Gould RN, CCN (C) (program coordinator), Denise MacCormack RN, CCN (C), Sheila Nicolle BSc PT Moncton (CEPS) Monique Dufour Doiron heads up the Moncton site. This Cardiac Rehabilitation program provides a 6 week program to the participants. After completing the CR program the participants are able to start in a maintenance program at the CEPS that is guided by the RN. 4 ACRN Miramichi The Miramichi site continues to offer 12 week programs as well as 8 week programs throughout the year. To accommodate long wait times the Miramichi site started hosting Primary and Secondary prevention education sessions; which consisted of coaching form the RN and RD over a 3 hour period. The team also continues to case manage patients who are unable to attend a program; providing education and support to the individual/family. In partnership with a local gym Chantal hosted 2 Spin classes with the donated proceeds going towards the Walk of Life. In preparation for the Walk of Life past participants were invited to walk with the staff every Tuesday morning along the walking path at the hospital. We had 8 to 10 regular attendants. This year they kicked off their second annual Walk of Life on September 7, The total amount raised this year was almost $13,000! Dr.Nowak, Jacqueline Noseworthy (dietitian) Stacy MacKinnon (Physio) Kristen Copp (RN) Danielle McIntyre (admin support) and Chantal Ricard (Coordinator) With the money raised from the 2012 Walk of Life the Cardiac Wellness team was able to purchase a NuStep St.Quentin The St.Quentin site has started a 12 week program that has been a great success. They continue to offer Heart to Heart sessions that are held in the Spring and Fall. For Heart month they held a HTN clinic at the local Tim Hortons, Irving and shopping mall. They had also participated in the annual Big Bike Ride this year. When asked what keeps you excited about CR? The St.Quentin CR team stated that it is helping and seeing a change in the participants and the sense of security that they develop! 4

5 Saint John Saint John C A R D I AC R E H A B I L L AT I O N Saint John Hampton St.Stephen These sites have been busy! Karen Crane reported that the team continues to provide 12 week outreach community programs continuing in the locations of Hampton, NB and YMCA Saint John. The Hampton program continues with yoga and breathing techniques as part of the program. 12 week outreach via video conferencing in the community of St. Stephen for education only (no exercise) occurs alongside 6 other 12 week programs at the Regional Hospital (education and exercise). Steve had also reported they continue to host the Pulmonary rehab program as well. The SJ team continue to case manage clients that are unable to attend a program. Addressing weight management issues and overall fear of exercise and reconditioning post cardiac event. The staff continues to be invited to speak on heart disease within local corporations such as; Irving, Seniors Wellness Centers; Teachers. Karen also stated that they have a wonderful team and are fortunate to have Steve Mundle's expertise and knowledge. Cleo allows each of us to search out areas in our growth of knowledge both educational and experimental. Steve reported that they had hosted the annual Walk of Life on September 7,2013 with $ raised to date. 5
