1 INFORMATION FOR THE COLLEGE OF HOSPITALITY, RETAIL AND SPORT MANAGEMENT (HRSM) SCHOLARSHIP APPLICATION (Please keep this page for your information) The College of Hospitality, Retail and Sport Management awards College-wide scholarships once each year in the Spring semester. These scholarships have been established through generous donations from alumni, friends, and industry supporters of the College, and promote outstanding student academic success and service. To apply for a scholarship, please complete and submit the following application. This application, which requires supporting materials as noted below, must be typed and is due in hardcopy before 5:00 PM on March 16, Incomplete applications will not be considered. A letter of recommendation from a former professor or an industry professional (or teacher or guidance counselor if you are an incoming freshman applicant). Letters of recommendation should be in a sealed envelope with the recommender s signature across the envelope flap. When requesting a letter of recommendation, please remember that this is a busy time of year, and you should allow the recommending professor/professional ample time for completion. 2. A copy of your unofficial undergraduate transcripts. If you are an incoming freshman applicant, please skip this step. 3. A personal statement that explains why you believe you should receive a scholarship and how you will benefit from the scholarship. The statement should be a minimum of 150 words but no longer than 300 words. Please be sure to include your name on the personal statement. 4. An updated copy of your resume/cv. If you are an incoming freshman applicant, please skip this step. PART FOUR: REQUIREMENTS AGREEMENT Please read and initial/sign the follow statements to confirm that you meet and agree to the requirements and qualifications for the College of Hospitality, Retail and Sport Management scholarships. I am a student currently enrolled and majoring in a program in the College of HRSM or an incoming freshman planning to pursue a degree in the College of HRSM. If I am awarded a scholarship, I understand the funds can be applied to tuition and/or registration costs ONLY. If the money is applied to my University account and is used for any other purpose, I agree to reimburse the scholarship fund. If I am awarded a scholarship and I change my major to one outside of the College of HRSM, I understand that my scholarship funds will be returned to the College. If I am awarded a scholarship, I agree to thank my benefactor(s) through a written thank you note and/or attend an event to receive my scholarship/meet my benefactor(s). I do not have outstanding fees on my University account. By signing below, I am verifying that all of the information on this application and in supporting materials is truthful and accurate. I understand that if any of the provided information is found to be untrue or inaccurate, I will forfeit my opportunity for a College of HRSM scholarship.
