2 INTERNATIONAL STUDENTS ADMISSION REQUIREMENTS While Sterling Health Center believes in equal opportunity to all candidates regardless of political affiliation, religious belief, race, creed, age, gender, color, national origin, sexual preference or disability, it does reserve the right to deny admission to anyone deemed unfit or inappropriate for the classes, the program, or the profession. To be considered for admission to the school, and issuing form I-20 (Certificate of Eligibility for Non-immigrant student) the following criteria must be met: Applicants must be physically, mentally, emotionally and financially capable of completing the course. All applicants who are accepted for admission are required to provide a current photo ID, such as a copy of his/her driver's license. Applicants must submit a completed registration form and non-refundable application fee of $ Completed and signed Financial Statement / Affidavit of Support that should be verified by a bank or employer. A letter, from sponsor, proclaiming financial responsibility and guarantying financial support for the student must accompany the financial support (The U.S. Department of Immigration and Naturalization requires the school to determine how an international student will meet expenses while enrolled at the school before being permitted to issue an I-20 form.). Proof of a score of at least 450 on the TOFEL exam or a proof of English language speaking and reading ability of the applicant acceptable to the school Director. Applicants should be free of contagious disease. Any question regarding this requirement should be discussed with the Director at the time the application is made. State law prohibits the registration of any student who has: a. been convicted of, entered a plea of nolo contender or guilty to, or received deferred adjudication to crimes or offenses involving prostitution or sexual offenses. b. been convicted of a felony or misdemeanor wherein the crime directly relates to the duties and responsibilities of a Licensed massage therapist as set out in e of the Texas Massage Law (relating to persons with criminal backgrounds). c. practiced or administered massage therapy at or for a sexually oriented business. Students from foreign countries should contact the Director to facilitate housing and travel accommodations. 2
3 MASSAGE THERAPY LEVEL I (BASIC) PROGRAM Tuition The total tuition for the Basic Massage Therapy Program (Level l) is $ Tuition may be paid by cash, check, money order, Master Card, Visa, Discover or American Express. 4 Financial Statement U.S.A Government regulations require international students to verify that they will have funds available to pay for all educational, living and other expenses. The total tuition for the educational program at Sterling Health Center Massage School is due at the time of registration. Part 1. (student complete self sponsored) I have a minimum of $ in U.S. currency available to me for the Sterling Health Center Massage Therapy program's tuition and fees. I will also have sufficient funds to cover living and travel expenses of myself and for any dependents who accompany me during my stay in United States. These funds can and will be provided during my entire period of studies by the individual or agency noted in part 2 and Part 3. I can make the arrangements to have necessary funds transferred to the United States and available to me prior to registration at Sterling Health Center Massage School. (Signature of Student) 4
