3 MESSAGE FROM CAMPAIGN LEADERSHIP The phone is ringing and in the simple act of answering it, your world is forever altered and sent down an unimaginable path. Ten years ago I answered our phone and was told to bring our 10 year old daughter to the hospital for immediate testing. Hours later the diagnosis was confirmed chronic myelogenous leukemia, a form of blood cancer that is only cured by a stem cell transplant. In the months that followed, Canadian Blood Services David Lehberg found an anonymous stem cell donor, a match was confirmed and after nine months of fear, our lives began to slowly return to a new normal. Our daughter is now 20 and in her third year at university. Stem cells found in the umbilical cord blood of newborn babies can save lives. Around the world, cord blood banks collect this life-saving blood to use in medical treatments. But in Canada where approximately 1000 patients are waiting to find matching stem cell donors there is no national collection system for umbilical cords. Currently in Canada, stem cells are extracted from adult bone marrow or Dale Sheard peripheral blood donations, but the shortage of donors makes finding a match difficult and many patients never get the life-saving treatment they require. CANADIAN BLOOD SERVICES CAMPAIGN FOR ALL CANADIANS My family s happy outcome is not always the case, unfortunately. Currently there are nearly 50 diseases that can be treated using stem cells. The number of patients on the list waiting for a stem cell match is growing rapidly but finding a match for many patients is becoming increasingly difficult because we don t have enough registered donors and we don t have an existing public cord blood bank in Canada. In fact, Canada is the ONLY G8 country that does not have a national public cord blood bank. Even though the Canadian health system can access international stem cell registries, Canadian DNA is unique and can be difficult to match due to our history of blended multi-ethnic genetics. We need a Made in Canada solution for this challenge and Canadian Blood Services is committed to establishing a new national public cord blood bank with your help. We are proud to bring you the Campaign For All Canadians and we urge you to participate in this important initiative. 4 Gabe at 5 years old Gabe was born into a family of firefighters and he too hopes to grow up and help others. Always a happy and energetic child, Gabe was diagnosed with acute lymphoblastic leukemia in 2010, a life-changing event not only for him, but also his family and community. Although he is in remission, Gabe s condition is complicated by a genetic deformity that can cause a relapse at any time. If he does relapse, to survive, he will need a stem cell transplant and his best chance of a match is from someone of Aboriginal ancestry. 2

5 THE COMPELLING CASE FOR A PUBLIC CORD BLOOD BANK Currently in Canada, umbilical cords at virtually every birth are discarded. And yet these cords are a rich source of lifesaving stem cells. Each cord has a potential life-saving stem cell match in Canada perhaps even somewhere in the world. For certain diseases, a stem cell transplant is the only treatment option available to patients in order to survive. Other sources of stem cells can be obtained from adult donors through bone marrow or peripheral blood collection. However, umbilical cord blood offers a range of compelling added advantages: Greater availability: Once an umbilical cord blood unit is entered into a registry, which is part of an international stem cell network, that unit can be located and shipped in just days. By contrast, collecting bone marrow or peripheral blood stem cells from an adult donor can take weeks and sometimes months. The time gained through umbilical cord stem cells can mean the difference between life and death. More matches: A stem cell transplant requires a DNA match between the donor and recipient, which is more likely to occur between members of the same ethnic group. Stem cells found in cord blood have special properties that do not require the DNA match to be as close as stem cells from bone marrow or peripheral blood, thereby allowing for more possible matches. CANADIAN BLOOD SERVICES CAMPAIGN FOR ALL CANADIANS By collecting and storing umbilical cord blood in an accredited national public bank, as most developed countries do, we could save many lives and help to reduce the suffering of many families. Since Dr. Elaine Gluckman performed the first cord blood transplant in 1989, this procedure has been successfully performed 20,000 times globally. Today 500,000 cord blood units are stored in more than 46 public cord blood banks in 27 countries. Yet we still have difficulty finding a match for every patient. Fewer possible complications posttransplant: Because umbilical cord stem cells are less mature than adult stem cells, there s less chance of rejection, which occurs when the transplanted stem cells recognize the host as foreign and attack the healthy cells. The stark reality of stem cell transplantation is that 70% of patients looking for a match will have to look outside their immediate family. For communities such as the Canada s First Nations, Inuit and Métis peoples, matches cannot be found within international registries. We need a Canadian solution. The national Public Cord Blood Bank will include many more First Nations, Inuit and Métis donors, resulting in more stem cells available to match the patients in need. As Canada s population continues to become more ethnically diverse, a national public cord blood bank is needed to address the urgent need for more stem cells to be available for all Canadians. 7 STEM CELLS SAVE LIVES Approximately 1000 Canadians today are desperately in need of a stem cell transplant because it is their last and best hope for recovery. Canadian patients currently largely rely on stem cells from bone marrow or peripheral blood donors. There are not enough donors and the registry of donors does not reflect the ethnic diversity of Canadian patients. Stem cells from umbilical cord blood could help solve this critical problem but in Canada, umbilical cords are discarded at birth. REGENERATIVE QUALITIES OF STEM CELLS Stem cells are cells that can continually regenerate more stem cells and specific other kinds of cells. Adult stem cells have predetermined types of cells that they can generate. Stem cells from umbilical cord blood are immature adult stem cells. These stem cells can generate blood cells, connective tissue cells (like bone, cartilage and ligaments) and structure cells (like blood vessels). Adult bone marrow stem cells can generate the same three types of cells however cord blood is richer in the type of stem cells that can generate blood cells. WHAT ARE BLOOD STEM CELLS? Blood stem cells are the building blocks of the cells found in our bloodstream: red blood cells for oxygen transportation, platelets for blood clotting and white blood cells for fighting infections. The cells in our blood stream are in a perpetual state of dying off and rebirth, so ongoing stem cell production is important. The three sources of blood stem cells for stem cell transplant are bone marrow, peripheral blood stem cells found in circulating blood and umbilical cord blood. WHY ARE THEY SO IMPORTANT TO MEDICAL TREATMENT TODAY? Cord blood stem cell transplants are used for treating over 50 diseases and disorders, such as leukemia, lymphoma, aplastic anemia, inherited immune system and metabolic disorders, as well as sickle cell disease. Cord blood stem cell transplantation began as a medical treatment for pediatric patients, but now is also commonly used for adults who generally require two cord blood donations. In addition, stem cell research and treatment is a growing area of medicine that has the potential to change the treatment of many diseases. Known as Regenerative Medicine researchers hope to one day treat a variety of diseases, such as: Parkinson s disease, Multiple Sclerosis, Huntington s disease, Alzheimer s disease, heart disease, joint repair, diabetes, asthma and numerous other treatments. Promising results from a wide variety of research projects show the potential for turning this research into safe and effective medical therapies in the future. SOME OF THE DISEASES TREATED BY STEM CELL TRANSPLANTS IN CANADA, 2011: Acute Lymphoblastic Leukemia Acute Myelogenous Leukemia Adrenoleukodystrophy Chronic Lymphocytic Leukemia Chronic Myelogenous Leukemia Hodgkin s Lymphoma Hurler Syndrome Myelodysplasia Myelofibrosis Non-Hodgkin s Lymphoma Refractory Anemia Severe Aplastic Anemia Waldenstrom s Macrolobulinemia Wiskott Aldrich Syndrome CANADIAN BLOOD SERVICES CAMPAIGN FOR ALL CANADIANS The Campaign For All Canadians will allow us to address a growing healthcare need amongst the Asian-Canadian population. With the demography of Canada reflecting a fusion of many cultures, often the unique genetic requirements of Canadian patients waiting for a stem cell transplant cannot be matched by international stem cell registries. A national public cord blood bank would offer a larger source of stem cells that is reflective of Canada s diversity, thereby creating more life-saving transplant opportunities for patients in need. For leukemia patients from the South Asian community, it is much harder to find a stem cell match, and I want to help change that. Giving mothers a chance to donate their child's cord blood to save lives is such a simple act of love and kindness that can have a big impact on patients searching for a second chance. 8 Nate at 1 1/2 years old Nate was born in April He was diagnosed with Wiskott Aldrich Syndrome and needed to find a stem cell match through the OneMatch Stem Cell and Marrow Network. A match was found and Nate had his cord blood transplant on December 2, when he was just shy of eight months old. 6

9 PARENTS CAN SHARE THE JOY OF THEIR NEW BABY BY DONATING THEIR CORD BLOOD Blood stem cells collected from umbilical cords which are currently discarded after birth have some unique advantages over those collected from bone marrow or peripheral blood: Cord blood is collected in advance, stored and ready for use immediately as needed, decreasing patient wait times for a stem cell donor search. Cord blood transplants can lower the risk of rejection, a common serious side effect of stem cell transplants from donors unrelated to the patient. Cord blood stem cells can be more easily matched with patients About 70 percent of patients who need a stem cell transplant must look outside their family for a match. Matching is done based on the DNA of the patient and donor and DNA is derived from a persons ethnic origins. Currently the registry of Canadian stem cell donors is relatively small, and does not reflect the ethnic diversity of our Canadian population. Therefore, Canada relies on international sources for all cord blood transplants. Umbilical cords are a rich source of stem cells that can provide renewed life for many patients waiting for a stem cell match. A national public cord blood bank will provide new mothers an opportunity to donate their child s umbilical cord for the benefit of patients in need of a stem cell donation. The collection of the cord does not interfere with or impact the delivery of a baby. Canadian mothers can help build this new public cord blood bank by donating their baby s cord blood and with the increased number of stem cells available they will be giving hope and a future to the many patients in need. Most of the cord blood donations imported to Canada come from the United States, Spain and Italy. Cord blood matches need to meet only 4 out of 6 criteria, while bone marrow matches must meet at least 9 out of 10 criteria. Cord blood stem cells are the youngest and most adaptable form of cells which the body more readily accepts. Less than 30 per cent of patients in need of a blood stem cell transplant are able to find a match within their family. Today, approximately one thousand Canadians and thousands more worldwide are searching for an unrelated stem cell donor. CANADIAN BLOOD SERVICES CAMPAIGN FOR ALL CANADIANS Our reliance on international stem cell registries underestimates the needs of Canadians of unique origins in particular Canada s Aboriginal population. These needs can only be served by an accredited and robust national public cord blood bank offering access to an increased number of stem cells. Such a bank must also enable greater outreach so as to ensure a greater diversity of potential donors. Canada is one of only two G20 countries without a national public cord blood bank. We as Canadians need to embrace the establishment of such a vital healthcare resource. At present, only about 50% of those who need an unrelated stem cell donation find a suitable match. 11 MESSAGE FROM THE CEO Canadian Blood Services is responding to an urgent health care need to meet the increasing demand for stem cells in Canada, a need that is growing at a staggering rate. The number of Canadians waiting for life-saving stem cell transplants has tripled over the past five years and continues to grow. As the population ages and medical therapies increasingly incorporate stem cell-based treatments, we expect demand to rise even higher. However, no matter what the numbers, the outcome is simple: these patients, who are suffering from diseases such as leukemia, lymphoma or aplastic anemia, require a stem cell transplant to survive. We need your help to save more lives. Canada is the only G8 country that does not have a national public cord blood bank. As a result, if a transplant doctor requests a cord blood stem cell unit for a patient; this unit must be imported from an international registry at a cost of about US$42,000. We are excited and honoured to be leading the project to build a national public cord blood bank for Canada. Our business plan for the development of a national cord blood bank is aggressive: we aim to reach a target of 20,000 cord blood units by This cord blood will contain stem cells that are representative of the unique DNA of our ethnically diverse population. Once operational, this bank will greatly improve the odds of finding a match within Canada and will become an international resource for patients around the world.
