1 The American Academy of Ophthalmology Adopts SNOMED CT as its Official Clinical Terminology H. Dunbar Hoskins, Jr., M.D., P. Lloyd Hildebrand, M.D., Flora Lum, M.D. The road towards broad adoption of electronic health record systems is built upon a foundation of health informatics standards for vocabulary. A terminology is needed not only for an electronic health record but also for clinical decision support, access to clinical information, research and survey purposes. Dr. Chalmers notes, Within any field of collective endeavor, the ability of people to communicate accurately and unambiguously with each other is vital to the advancement of knowledge. One of the most fundamental components of scientific communication is a comprehensive vocabulary that encompasses all aspects of the discipline under study. 1 On May 6, 2004, the Departments of Health and Human Services (HHS), Defense, and Veterans Affairs announced the adoption of standards to allow for the digital exchange of clinical information across the federal government. 2 Specifically, one of the standards endorsed was the Systematized Nomenclature of Medicine Clinical Terms (SNOMED CT ) for laboratory result contents, non-laboratory interventions and procedures, anatomy, diagnosis and problems. President Bush issued an Executive Order on August 22, 2006 that HHS and all federal agencies shall ensure that internal programs and external contracts implement relevant interoperability standards. 3 HHS is one of nine charter members of the new International Health Terminology Standards Development Organization (IHTSDO) for SNOMED CT. Among its goals are to make this terminology accessible to developing countries, align it with international public health standards, and use it to collaborate in research and public health surveillance. 4 The National Health Service (NHS) in the United Kingdom has adopted SNOMED CT for its national NHS Care Record, defining it as A single unified terminology to underpin the development of the integrated electronic patient record by providing an essential building block for a common computerised language for use across the world. 5 In 2004, the European Commission set forth a comprehensive definition and goals for e- health: 6 e -health is today s tool for substantial productivity gains, while providing tomorrow s instrument for restructured, citizen-centered health care systems and, at the same time, respecting the diversity of Europe s multi-cultural, multi-lingual health care traditions. There are many examples of successful e-health developments including health information networks, electronic health records, telemedicine services, wearable and portable monitoring systems, and health portals. We need a common language in order to communicate exact meaning of information needed for e-health developments. As described by a Health Level 7 workgroup, To maximize the usefulness of shared information and to apply applications like intelligent decision support, a higher level of interoperability is required. This is called semantic interoperability which is the ability of information shared by systems to be understood

2 so that non-numeric data can be processed by the receiving system It goes beyond structuring the data and gets into communicating the intent or understanding of the meaning of the data to the sender to the information user. 7 For successful communication, a base terminology must meet the following criteria. It must be: 1) precise, 2) reproducible, 3) standardized, 4) detailed enough to capture complex clinical descriptions and findings, 5) structured to not only uniquely identify a concept but also describe the relationships between concepts, 6) able to identify synonymous terms while allowing flexibility in different settings and locales to use differing terminology and 7) capable of translating clinical concepts into machinereadable codes (making the information useful for computer systems and digital exchange. SNOMED CT describes concepts in relationships to other concepts and in various hierarchies, classifying them in common disease categories used by physicians. These classifications help organize information, permitting the grouping or aggregation of related entities. Another feature is the association of multiple synonyms to one unique concept. Independent studies have demonstrated SNOMED CT to be the most complete clinical reference terminology in existence today. 8,9 Chiang et al parsed 20 complete ophthalmology case presentations from an ophthalmology journal into 1603 concepts and evaluated the adequacy of five medical terminologies for representing concepts in ophthalmology. They found SNOMED-CT had the highest mean overall coverage had higher coverage scores than any of the other terminologies for concepts in the diagnosis, finding, and procedure categories. 10 Researchers at Mayo Clinic evaluated how well it could identify clinical problem lists and found that it exactly represented 92% of terms commonly used. 11 They recommended that health care organizations be given incentives to adopt SNOMED CT. In contrast, International Classification of Diseases (ICD) 9-CM does not rely on hierarchical relationships, does not acknowledge synonymous terms and has less granularity. One study found that ICD9-CM had a 37% coverage rate for clinical problems. 12 In 1996, when the Academy began working on eyecare terminology, SNOMED CT included about 2,000 eye related terms. At that time, the Academy recognized the need for more comprehensive coverage of ophthalmic terminology and that new concepts would continue to evolve. On behalf of the Academy, a few ophthalmologists have spent time modeling" ophthalmic terms, which includes adding new concepts, defining accurate relationships among the different terms, identifying synonyms and preferred names, etc. As a result of these efforts, the ophthalmology subset now numbers 14,733 terms, including 9,510 unique or preferred terms, and 5,223 synonyms and will be available in the future as an ophthalmology SNOMED subset. The SNOMED CT system is being used to identify the digital images and content in the Academy s Basic and Clinical Science Course and will be used to tag all digital content in the Ophthalmic Network for Education, the Academy s website for clinical education,

3 so that information can be readily retrieved and collated at varying levels of detail and organization. The vision of the Ophthalmic Network for Education is as follows: Point of care information has been determined as the best method of altering physician behavior, and improving patient outcome. creating the Ophthalmic Network for Education based on all Academy ophthalmic assets, the Academy believes that every member will have access to the latest evidence based, or consensus based information to enable them to practice better and improve their patient care. Without a standardized and comprehensive terminology, it would not be feasible to organize and retrieve information in an efficient and clinically relevant manner. For the user, the terminology backbone is largely invisible; it is primarily used in the background to encode concepts and to aggregate related terms when queried. There are a myriad of opportunities for use of SNOMED CT. As described above, it can be used to search and aggregate information on clinical conditions. It can serve as the backbone of electronic health record systems to allow reporting and exchange of patient data that can be compared and collated across different settings. It can be used to embed diagnosis-driven clinical decision support, based on best practices and guidelines. Data collection for research and survey purposes could be facilitated by forms encoded with SNOMED CT. Its more granular, detailed nature will aid researchers much more than the use of ICD9-CM. Information on outcomes, complications and post-marketing surveillance could be more readily compared and collected together, given a standardized terminology. For example, if a medication is withdrawn from the market, use of a standardized terminology could expediently identify all the patients receiving such a drug. Standardized terminology could be used by office systems (electronic health record systems, practice management systems, imaging equipment) to communicate information about procedures and diagnoses to enable efficient workflow and billing. The Academy s Board of Trustees approved a resolution to adopt SNOMED CT as its official clinical terminology in September It is the most comprehensive, detailed and robust terminology for use in eye care. For the first time, we have a set of eye care concepts that have been reviewed by practicing ophthalmologists and made relevant to clinical practice, and integrate with a terminology set encompassing the rest of medicine. This common language enables health information systems to communicate and share information in a form that will be understood in exactly the same way by both sender and recipient. We are deluged with information. Physicians suffer from information hunger in the midst of plenty, noted Dr. Hayward. 13 The problem is not the amount of data at our fingertips, its how to harness it and mine it for our benefit. We can leverage SNOMED CT to organize and make sense of all our information. The power of aggregating information and comparing data across settings will be unleashed as more and more groups adopt it. References:
