2 CREDIT OPTIONS FOR LICENSED VOCATIONAL NURSES ADN Program Curriculum, LVN-ADN 30 Unit Option Prerequisites must be completed with a GPA of at least 2.5 and a grade of C or better for each course. Students should consult with a counselor or the college catalog for current information. The following nursing courses must be completed for the 30-unit option candidate 2nd Sem. Fall 2009 Nurs 4 Beginning Medical-Surgical Nursing 5.5 Nurs 5 Mental Health Nursing Units Progress through the program will be contingent upon space availability for each of the following nursing courses. 3rd Sem/4 th Sem. Spring 2010 Nurs 7 Intermediate Medical-Surgical Nursing 5.5 Nurs 9 Advanced Medical-Surgical Nursing 3.5 Nurs 10 Community Based Nursing 2 11 Units I. ADMISSION FOR LICENSED VOCATIONAL NURSE - OPTION I, 30-UNIT OPTION A. General Information 1. Licensed vocational Nurses (LVNs) may receive credit for prior education in vocational nursing programs. 2. The 30-unit option will allow the candidate to meet requirements to take the Registered Nurse licensing exam (NCLEX-RN). 3. The candidate is exempt from all college assessment testing. 4. The candidate WILL NOT be a graduate of San Joaquin Delta College (SJDC) or the SJDC Nursing Program. 5. Some states may not recognize non-graduate registered nurses. In addition, some health care facilities will not employ non-degree nurses. 6. Admission into the nursing program is on a space availability basis, second priority. 30-unit option candidates will be offered space after all qualified LVN-ADN and PT-ADN degree-option candidates have been offered space. 30-Unit option students will be taking courses out of regular semester sequencing. Progression through the program will be contingent upon space availability for each nursing course: NURS 7, NURS 9, and NURS All clinical facilities are requiring students to complete a background clearance for criminal history prior to placement in the clinical facility. Students who cannot obtain this clearance will be unable to complete the program objectives and the offer of space will be withdrawn. Candidates are strongly encouraged to consider their personal background prior to submitting a LVN-ADN Prerequisite Verification to the nursing program. The criteria which will be utilized to determine clearance, is identified in Appendix A. The clearance application and fees must be submitted upon tentative acceptance into the program. 3 B. Requirements to apply as a 30-unit candidate: 1. Completion of prerequisite course with a GPA of at least 2.5 and a grade of C or better for each prerequisite course listed above. 2. Current and valid California Vocational Nursing License at time of entry into the program. Candidate must provide copy of license or official notification from the Board of Vocational Nursing and Psychiatric Technicians of successful completion of the licensing exam with the LVN-ADN Prerequisite Verification. 3. Candidates applying as a 30-unit candidate must schedule a mandatory meeting with the Dean of Health Sciences to discuss implications of the 30-unit option. This must be done prior to submitting a Prerequisite Verification form. C. Prerequisite Verification Process 1. LVN-ADN Prerequisite Verification forms for fall 2009 admission will be accepted April 20 through April 24, 2009 ONLY. Mailed packets must be received by the Health Science Division, Locke Center, Room 213, no later than 4:30 pm on April 24, Acceptance of Prerequisite Verification form does not guarantee that there will be space available in the program. Incomplete or disqualified Prerequisite Verifications will be returned to the candidate. Late LVN-ADN Prerequisite Verification forms will not be accepted. 2. Prior to submitting your LVN-ADN Prerequisite Verification form, schedule a mandatory meeting with the Dean of Health Sciences to discuss implications of the 30-unit option. 3. A current Application for Admission to San Joaquin Delta College must be on file with the college prior to submitting this Prerequisite Verification. If you've never attended San Joaquin Delta College or have not attended for more than one semester, you must apply ONLINE for admission to the College. Only online applications are accepted and may be completed at the following locations: Electronic Enrollment Services Lab, Stockton Campus, Cunningham 226, at the Tracy Learning Center, 1895 W. Lowell Avenue, Tracy; or by accessing the Delta College web site at 4. ALL fees owed to San Joaquin Delta College must be paid in full before you submit a LVN-ADN Prerequisite Verification form and you must be in good standing at San Joaquin Delta College. 5. Submit a copy of a valid, current California Licensed Vocational Nurse license or copy of an official notification from the Board of Vocational Nursing and Psychiatric Technicians of successfully passing the licensing examination with the LVN-ADN Prerequisite Verification form. 6. Official transcripts (in original sealed/unopened envelope) for all lower and upper division courses completed at any and all regionally accredited U.S. institutions other than San Joaquin Delta College must be submitted with LVN-ADN Prerequisite Verification form. Failure to submit transcripts with all grades posted will result in disqualification for fall Course descriptions for prerequisite courses only must accompany the academic transcript for the purpose of determining course equivalency. Course descriptions must come from the specific catalog year in which the course was taken and may be obtained from the college catalog where courses were taken or at Not required if prerequisite courses were taken at San Joaquin Delta College. 8. All required information must be included at the time of submission or your LVN-ADN Prerequisite Verification form will be disqualified. D. 30-Unit Option Admission, Notification and Additional Requirements 1. Prerequisite Verifications for 30-unit candidates will be ranked by use of the prerequisite GPA. In the event two or more candidates have the same prerequisite GPA, the date and time of submission of a complete LVN-ADN Prerequisite Verification will be utilized as the tie-breaker. Candidates with the highest prerequisite GPA will be conditionally offered space after all qualified LVN-ADN and PT-ADN degree-option candidates have been offered space, if space is available. 3. All candidates conditionally offered a space will be required to take the Test of Essential Academic Skills (TEAS) and pass with a composite/adjusted individual total score of 67% as established by the California Community College Chancellor s Office. Please refer to the Prerequisite Verification Information packet for more details on this requirement. 4. Candidates will be notified by of their ranking and admission status by the Health Science Division. A list of qualified candidates to fill space(s) in the program, should any become available, will be created every semester. E. Licensing Information Prior to obtaining a license to practice as a Registered Nurse ALL graduates must report prior felony and misdemeanor convictions along with submission of fingerprints. The Board of Registered Nursing (BRN) may deny licensure based on prior convictions, which include, but are not limited to: Conviction of child abuse Violation of Nursing Practice Act Conviction of a mentally disordered sex offender Crime or act involving narcotics, dangerous drugs, or dangerous devices Conviction of assault and/or battery Any act that is determined to be substantially related to the practice of nursing When considering denial of license, the BRN takes into account the following criteria to evaluate the rehabilitation of the candidate: Nature and severity of the acts or crimes Additional subsequent acts Recency of acts or crimes Compliance with terms of parole, probation, restitution, or other sanctions Evidence of rehabilitation submitted by candidate The Board of Registered Nursing screens applications fairly but cautiously, applying the above criteria. Schools of nursing are encouraged when counseling prospective nursing students to make them aware that there could be potential licensure problems due to serious acts or convictions as described above. In this manner, students have the opportunity to explore other career options prior to investing substantial time in a nursing program if it appears that a prior serious act or conviction may jeopardize licensure due to its substantial relationship to the practice of nursing. ADMISSION FOR LICENSED VOCATIONAL NURSE - ADN DEGREE, OPTION II A. General Information 1. Licensed Vocational Nurses (LVNs) may receive credit for prior education in vocational nursing programs. 2. Admission into the nursing program is on a space available basis. Candidates may complete the ADN Degree option two ways: a. NURS 6, Obstetrical Nursing and NURS 8, Pediatric Nursing may be waived IF the candidate provides documentation of at least 60 clinical hours of employment in the role of an LVN or LVN-ADN Prerequisite Verification Information May 2009, Page 6

7 vocational nursing program verification of 60 hours of clinical instruction in each area with submission of the Prerequisite Verification. Determination of each equivalency will be determined by the Dean of Health Sciences or designee. Candidates who do not provide this information at the time of submission will be required to complete NURS 6 and NURS 8. The candidate who waives NURS 6 and/or NURS 8 will be taking courses out of regular semester sequencing. Progression through the program will be contingent upon space availability for each nursing course: NURS 7, NURS 9, and NURS 10. b. The candidate may complete all required Nursing courses which includes NURS 4, NURS 5, NURS 6, NURS 7, NURS 8, NURS 9, NURS 10. Candidates who apply with this option are guaranteed space in each nursing course. 3. All General Education requirements must be met to obtain the Associate of Science degree. By waiving the first semester courses, LVN-ADN candidates will not complete 74.5 units. To complete the minimum unit requirements, LVN-ADN degree candidates must complete a minimum of 74.5 units, which must include nursing core courses, program prerequisites, general education courses, and other coursework from regionally accredited U.S. colleges or universities. 4. All clinical facilities are requiring students complete a background clearance for criminal history prior to final acceptance into the program. Students who cannot obtain this clearance will be unable to complete the program objectives and the offer of space will be withdrawn. Candidates are strongly encouraged to consider their personal background prior to submitting a LVN-ADN Prerequisite Verification to the nursing program. The criteria which will be utilized to determine criminal history "clearance" is identified in Appendix A. The clearance forms and fee information will be provided to student with tentative acceptance into the program. The clearance process must be completed prior to final acceptance into the program. 5. Selection process and formula: A numerical value will be obtained by use of four criteria and the formula below: College GPA of all lower division courses (excluding credit/non-credit courses) English 1A GPA Core biology GPA (anatomy, physiology, and microbiology courses) Repetitions of core biology courses (D, F, I, W grades) Formula: Exp( (ColGPA)+.3139(EngGPA)+.267(BioGPA) (BioReps)) (1+Exp( (ColGPA)+.3139(EngGPA)+.267(BioGPA) (BioReps))) A worksheet to calculate the numerical score, is available at nursing.deltacollege.edu. B. Requirements to apply for LVN- ADN Degree options: 1. Completion of all prerequisites of the Nursing Program with a cumulative GPA of at least 2.5 and a grade of C or better for each prerequisite course listed above. HS 43, Nursing Synthesis, is a prerequisite for LVN-ADN degree option but the grade is NOT utilized to factor the cumulative prerequisite GPA. Candidates must also have a minimum of 70% probability of passing the program. This score is determined utilizing the selection formula detailed in II.A.5 above. 2. Current and valid California Vocational Nursing License at time of entry into the program. Candidate must provide copy of license or official notification from the Board of Vocational Nursing and Psychiatric Technicians of successful completion of the licensing exam with the Prerequisite Verification. 3. NURS 6, Obstetrical Nursing and NURS 8, Pediatric Nursing may be waived IF the candidate provides documentation of at least 60 clinical hours of employment in the role of an LVN or vocational nursing program verification of 60 hours of clinical instruction in each area with submission of the Prerequisite Verification. 10 APPENDIX A The criminal history background clearance will identify any felony or misdemeanors within the entire state of California for the past seven years. If any of the following appear on the criminal history background report, the offer of space in the nursing program will be withdrawn. Criminal History Background Screen Scoring Definitions Non-Conviction: Any disposition other than a plea of guilty, no contest or a finding of guilt. Non-Convictions can be one of three categories. o o o Passing: Non-Conviction leading to charge being dismissed, Nolle Prosse, Nolle Prosequi, Expunged, Not Guilty verdict or acquittal of defendant. Failing: Any adjudication withheld/deferred where the charge was not dismissed, expunged, Nolle Prosse or Nolle Prosequi. Provisional: Any active or pending case. Passing Disposition: Any Non-Conviction disposition leading to the case being dismissed, Nolle Prosse, Nolle Prosequi, Expunged, Not Guilty verdict or acquittal of defendant. Failing Disposition: Any disposition resulting in a Conviction or Non-Conviction (adjudication differed/withheld) that has not led to the case being dismissed. Healthcare Related Misdemeanor: Any Misdemeanor crime related to the following categories: Theft Embezzlement Forgery Fraud Misuse of Credit Card Unauthorized Use Shoplifting Receiving Stolen Property Unemployment or Worker s Compensation Fraud Worthless Check Assault and/or Battery Simple Assault Domestic Violence Resisting Arrest Obstructing Officer Evade Officer/Lawful Arrest Weapons Charges Stalking Menacing Child Abuse/Neglect Child Endangerment Contributing to Delinquency Any child-related sexual activity Any nonconsensual sexual activity with child or adult Sale and/or possession of a controlled substance Public intoxication/drunkenness Elder abuse/neglect Pass Score all candidates as Pass for the following conditions: Any misdemeanor or felony offense with a disposition date older than 7 years, with the exception of any misdemeanor or felony violent crimes, nonconsensual sexual crimes, and crimes against children with a failing disposition. Any misdemeanor or felony crime with a passing disposition. Any misdemeanor marijuana offense over two years old in the State of California. Any misdemeanor (or lower) traffic violations. Fail Score all candidates as Fail for the following conditions: Any case with a Failing Disposition for Misdemeanor or Felony violent crimes, nonconsensual sexual crimes, and crimes against children regardless of elapsed time from disposition date. Any other Felony crime with a Failing Disposition within the last 7 years. Any Healthcare Related Misdemeanor crime with a Failing Disposition within the last 7 years (except in California, exclude misdemeanor marijuana convictions more than two years old). Any Controlled Substance Offence with a Failing Disposition within the last 7 years (except in California, exclude misdemeanor marijuana convictions more than two years old.
