2 1. CRITERIA for filling out the Scholarship Packet 1. Traditional BSN and RN-BSN or CARE students only. There are no scholarships for Graduate students at this time. 2. Applicant must have a GPA of 3.0 or above. 3. Applicants receiving grant money are not eligible for these scholarships. 4. Applicants who have loans are eligible to apply. TOPS recipients are eligible to apply. 5. Applicants must submit a Financial Aid Award Notice or FASFA statement to prove financial need. 2. CRITERIA for Scholarships 1. Lori Norton Scholarship- Entering student in either the traditional BSN, RN-BSN Program or CARE program and must have a GPA of 3.0 or above. Students already receiving grant moneys are not eligible, students that have loans may apply. Student must supply a copy of their Student Financial Aid Award Notice. 2. Nursing School Scholars - Entering student in either the traditional BSN, RN-BSN Program or CARE program and must have a GPA of 3.0 or above. Students already receiving grant moneys are not eligible, students that have loans may apply. Student must supply a copy of their Student Financial Aid Award Notice. 3. Suzanne Theriot Scholarship - Student in the traditional BSN program and must have a GPA of 3.0 or above. Students already receiving grant moneys are not eligible, students that have loans may apply. Student must supply a copy of their Student Financial Aid Award Notice. 4. Laura Ryan Endowment Scholarship is for non-traditional students (older students, single parent of minor children, or parents of minor children, please include the number of children, elder care provider, students with 2 nd degrees, etc.) must have a GPA of 3.0 or above 5. Northrup Scholarship Fund - Entering student in either the traditional BSN, RN-BSN Program or CARE program and must have a GPA of 3.0 or above. Alumni Scholarship Students (traditional BSN, RN-BSN Program or CARE program) that are active in student government or student class officers, must have a GPA of 3.0 or above. Students already receiving grant moneys are not eligible, students that have loans may apply. Student must supply a copy of their Student Financial Aid Award Notice. 7. Donielle Doni Doucette Memorial Scholarship This scholarship is to be awarded to an undergraduate nursing student who is a non-traditional student defined as working with/without dependents. A Junior/Senior level student in traditional BSN, RN-BSN, or CARE programs, must have a GPA of 3.0 or above. 3. Loss of Scholarship: The scholarship will be withdrawn for any of the following reasons: 1. Failure to maintain the required average. 2. Resignation or dropping out of the School of Nursing. 3. Being placed on disciplinary status or on academic probation. ***Scholarships will be distributed based on availability of funds. *** 2

3 4. Financial Aid Award Notice: One of the scored criteria for all the scholarships is the student s debt accumulated from loans taken while going to school. This criteria is verified through paperwork submitted. Please include a copy of the financial aid notification received from the Student Financial Aid Office called Financial Aid Award Notice. A student may access their Financial Aid Award Notification through Academic Self Service. A screen shot of the View Award Notification link is below. The Student Financial Aid Office may provide statements for non-applicants and for those who have not yet been awarded. If you cannot obtain this form from financial aid, you need to apply to the federal financial student aid program. Use the LSUHSC code Financial aid department will process the FASFA application. This may take a few days; don t delay in submitting. Please do not attach your student invoice. If you have any difficulty, please contact the Student Financial Aid Office for assistance at Scholarship Notification of Award: Students awarded a scholarship will be notified by Karen Smith in the Dean s Office. An notification will be sent to all undergraduate students stating the process has ended. 6. Ineligibility: If your application is missing any part of the three forms for entry, you will be ineligible for a scholarship award. Please ensure you submit a completed application to the Office of Student Affairs by deadline. (Forms I, II, and III of the Scholarship Packet (pages 4, 5 & 6) and the Financial Aid Award Notice). No applications will be accepted after the deadline of Friday, September 4, 2015 at 4:00 p.m. 3

You may check more than one scholarship. Only one Scholarship Packet is needed.
